Progressive Rehabilitation Associates, LLC is seeking a full-time Physical Therapist (PT) to join our collaborative and experienced multidisciplinary team at University of Iowa Home Health Care. This position includes the evaluation and treatment of adult patients in their homes in Johnson and surrounding counties to address mobility, strength, balance, and functional independence. The ideal candidate is a motivated clinician who values autonomy and flexibility while providing high-quality, evidence-based care. This role offers the opportunity to work closely with other physical, occupational, and speech therapists, nurses, and physicians to optimize patient outcomes in a home health setting. Responsibilities: Perform comprehensive evaluations and develop individualized treatment plans for home health patients. Provide skilled interventions to improve mobility, strength, and safety. Educate patients and families on exercises, home safety, and fall prevention. Collaborate effectively with an interdisciplinary care team to ensure coordinated care. Complete timely and accurate documentation in accordance with regulatory and company standards. Qualifications: Doctor of Physical Therapy (DPT) or Master s Degree in Physical Therapy from an accredited program. Current Iowa Physical Therapy license (or eligibility for licensure). Prior home health experience preferred, but not required. Strong interpersonal, time management, and communication skills. Valid driver s license and reliable transportation required.
