Nursing Team Roles: Care is relationship-based, focusing on physiologic monitoring, patient education, and discharge planning, while utilizing a collaborative multidisciplinary practice approach. Nursing provides extensive education to newly transplanted heart and lung patients. This includes medications, lab and vital sign monitoring, lifestyle modifications, promoting independence in activities of daily living, and emotional support. All patients are ECG monitored with rhythm identification and interpretation responsibility assumed by nursing staff. Additional monitoring to meet patient needs include SpO2 monitoring, non-invasive blood pressure, and arrhythmia detection. Nurses manage equipment such as temporary pace makers, chest tubes, central lines, nasogastric tubes, wound cares, and more depending on the needs of the patient.
