Evaluates effectiveness of treatment and adjusts the treatment to achieve maximum benefits.Monitors all patients under his/her care: supervises Physical Therapist Assistants and/or Technicians who are involved with treatment, and assures appropriate treatment and documentation; submits a modified plan of care to the physician for altered plans of care; reports any adverse reactions to treatment or changes in condition to the physician as indicated.Institutes or assists with emergency measures for sudden adverse developments such as cardiac arrest or medical condition requiring activation of a code, rapid response team, or activation of 911.Implements and assesses appropriate patient and caregiver education per Plan of Care and phases of progress.Establishes and maintains collaborative communications with physicians, other healthcare professionals, and facility departments. This would include team conferences, ICU rounds, other patient care rounds or assigned meetings.Consistently meets department productivity standards (70%)Completes required reports, notes and other client-oriented documentation in a timely manner according to policy and department standards, including G-code functional reporting and associated modifiers.Complies with changing regulations related to insurance, The Joint Commission, state/federal and other requirements as directed.Documents medical necessity and skilled need for continued treatments in evaluations/re-evaluations and progress notes.Meets facility standard for Focus on Therapeutic Outcomes (FOTO) status and discharge surveys (minimum of 10 completed per quarter). Uses FOTO recommendations appropriately or documents reasons for exceptions (i.e. progress to date and expectation of significant progress if patient extended beyond FOTO recommendations). (outpatient)Enters charges accurately into Meditech or on appropriate charge slip.Schedules patients in an efficient and appropriate manner, with assistance of support personnel.
    Qualifications
    License/Certification:Physical Therapist licensed to practice in the State of Oregon.  BLS is required within 30 days of hire. Experience:Clinical experience dealing with a broad base rehabilitation facility is preferred. Must have strong interpersonal and communication skills, both verbal and in writing.Education:Must have graduated from an accredited program.Overview
