Overview:
The Director of Clinical Services, Hospice provides coordination of the entire interdisciplinary team by coordinating all disciplines including social work, counseling and volunteer services for the provision of patient care, particularly through coordination of the interdisciplinary team (IDT) meetings. The Director of Clinical Services assists with the oversight of safety and infection control, chart audits, and medical supplies. Assures medical records are maintained appropriately and that all clinical disciplines function according to licensure, state law and Medicare standards. Assist with coverage for the Director of Operations during vacations, leaves or as needed. This position supports the Daytonn, Lebanon and Cincinnati Markets.

Responsibilities:
Planning, Organizing & Implementation
Provides direction, supervision and consultation to the clinical staff.Serves as a resource to all agency personnel regarding clinical activities. Assists in the oversight of the nursing, hospice aide, social work, counseling and therapy services to patients in Hospice consistent with standards of practice, community need, agency policies and procedures, Medicare guidelines and State Hospice licensure laws.Provides clinical nursing care to patients when needed.Works with clinical staff to assist as needed with difficult care issues and to alter patient plans of care.Advises, assists and collaborates with clinicians on an as-needed basis to assess patient situations, staff problems, and coordinate physician referrals.Assures that each patient receives quality care consistent with agency policies based on patient need and state/federal guidelines. Utilizes nursing knowledge and skills in overseeing the provision of nursing care to hospice patients on a high quality, consistent basis in accordance with the mission of agency.Assists with the development and maintenance of nursing and clinical policy and procedure manuals.Assures that hospice nurses, social workers, counselors, therapists and hospice aides/homemakers receive supervision.Demonstrates basic keyboarding and word processing skills and is able to use agency software/computer systems.Assists Executive Director in seeing that there is proper maintenance of medical records.Human Resource Management
Assures that Hospice Nurses and Hospice Aides/Homemakers receive supervision, either directly or via other appropriate nursing staff in a manner that reflects patient care standards.Supervises employees for performance relative to patient care and case management, providing feedback and on-site supervision as required.Conducts employee performance evaluations appraisals at the completion of the probationary period and annually thereafter. Assists in setting annual performance goals and provides feedback.Informs the Executive Director of patient care and/or staffing issues or personnel issues on a timely basis.Works with Executive Director to assure that on-call schedules and services are maintained, coordinated and care is available through Hospice RNs 24 hours a day, 7 days per week.Fiscal Management
Understands the need for effective staff and contractor utilization and productivity.Assists in the investigation of patient and customer concerns and complaints.Approves requests for time off assuring that there is coverage for all clinical positions/services.Focuses on maintaining nursing, therapy, counseling, social work and aide staffing to parallel census needs for Hospice.Appropriately manages staffing to accommodate referrals.Works with the Executive Director to recruit, train, and maintain staff based on agency and community needs.Oversees that clinician's function within clinical caseload guidelines and productivity standards as determined through agency management, budgetary and regulatory processes.Leadership/Professionalism

Manages a caseload as needed to deliver care in a cost-effective manner that demonstrates an understanding of Hospice reimbursement methodologies.Assures proper maintenance of records per agency guidelines.Participates in evening/weekend on-call as required, conducting on-call services in a clinically competent and responsive manner.Maintains regular attendance.Functions independently but seeks and accepts guidance from other members of the IDT, as necessary and appropriate.Complies with local, state and federal laws, Medicare Hospice Conditions of Participation and agency policies and procedures.Conducts all business activities in a professional and ethical manner.Qualifications:
Education: Graduate of an accredited registered nursing program, bachelor's degree in nursing preferred.
Licensure/Certification: Must have a current RN license in good standing in the state of Ohio or in accordance with the Board of Nursing. Valid driver's license and reliable transportation that is insured in accordance with Ohio requirements
Experience: 2+ years as a RN of direct patient care experience in hospice, a home care setting or other related health program required; 2+ years of supervisory and/or administrative experience required.

OVERVIEWThe Social Services Director for Rehabilitation Services reports to the Executive Director. The Social Services Director identifies and facilitates social and emotional needs of the residents, families, and responsible party. Will assist with admission paperwork and inquiry process as needed.Ensures social services issues comply with all facility policies and procedures as well as State and Federal requirements and guidelines.Complete annual requirements eligibility re-determination by work with the local Department of Child Services and Division of Family Resources.Provide consultation to staff concerning resident rights and needs of the elderly, and others relating to social service needs.Make contacts with family members or residents as needed to discuss Medicaid application process, eligibility requirements document that are needed to apply and who to contact to initiate the Medicaid application process.Maintain a close working relationship with all departments to assure appropriate and efficient decisions about services provided and discharges.Maintain a working knowledge of federal and state regulations.Education: College degree or High School diploma or general education degree (GED), or one year or more of related experience and/or training or equivalent combination of education and experience.
Experience: Previous experience in long-term health care facility preferred. The holder of this position should have relevant experience and social service and or customer service training.

Responsibilities
Be responsible for directing and coordinating medical care provided by the Hospice
Assist in the development of transfer and emergency treatment policies and procedures
Participate in client care conferences and guide administrative staff in the development and implementation of medically related Hospice programs and client care policies and activities
Assist in developing written rules and requirements for all licensed independent practitioners who have privileges at the Hospice
Consult with attending physician and the Interdisciplinary Team in establishing a written plan of care for each client, and in reviewing and updating the plan of care at intervals specified in the plan
Obtain necessary certification of a hospice client as terminally ill as required by state and federal regulations
Verify credentials of physicians who seek appointment as members of the medical staff of the Hospice and then subsequently approve the physicians' credentials with the Hospice Program Director
Ensure that an alternate credentialed physician would be available to respond to the needs of the Hospice in his/her absence due to vacations, illness or other extenuating circumstances
Provide emergency medical coverage in cases of emergent situations requiring physician intervention for all clients including when the clients own attending physician may not be available
Participate in appropriate planning and presentation of the Hospice's in-service education program for associates and volunteers.
Provide consultation to Hospice staff in the development of an adequate medical record system
Participate, as appropriate, in the Hospice's performance improvement and utilization review activities
Provide the Program Director with a written summary of appraisals, recommendations, plans relative to resident care activities, appropriateness of the Hospice's scope of services, medical equipment and staffing on a quarterly basis.
Consult with Hospice administration regarding the Hospice's ability to meet the clients' needs and about future client care programs
Evaluate occurrence worksheets reported to risk management, assist in identifying safety hazards and make recommendations to improve the Hospice's service delivery.
Keep current with policies and programs of public health agencies that may affect client care
Represent Hospice to both the medical and non-medical community through presentations, meetings, and media to better its understanding and acceptance by the community
Monitor employee health and advise the Program Director on employee health policies
Comply with the policies, rules, and regulations of and with federal, state and local laws and rules and regulation of any governmental agency or public authority and the appropriate accrediting agencies
Qualifications
TRAINING/EXPERIENCE REQUIREMENTS:
Must hold a medical degree and have advanced knowledge of hospice philosophy and concept

Responsibilities
Be responsible for directing and coordinating medical care provided by the Hospice
Assist in the development of transfer and emergency treatment policies and procedures
Participate in client care conferences and guide administrative staff in the development and implementation of medically related Hospice programs and client care policies and activities
Assist in developing written rules and requirements for all licensed independent practitioners who have privileges at the Hospice
Consult with attending physician and the Interdisciplinary Team in establishing a written plan of care for each client, and in reviewing and updating the plan of care at intervals specified in the plan
Obtain necessary certification of a hospice client as terminally ill as required by state and federal regulations
Verify credentials of physicians who seek appointment as members of the medical staff of the Hospice and then subsequently approve the physicians' credentials with the Hospice Program Director
Ensure that an alternate credentialed physician would be available to respond to the needs of the Hospice in his/her absence due to vacations, illness or other extenuating circumstances
Provide emergency medical coverage in cases of emergent situations requiring physician intervention for all clients including when the clients own attending physician may not be available
Participate in appropriate planning and presentation of the Hospice's in-service education program for associates and volunteers.
Provide consultation to Hospice staff in the development of an adequate medical record system
Participate, as appropriate, in the Hospice's performance improvement and utilization review activities
Provide the Program Director with a written summary of appraisals, recommendations, plans relative to resident care activities, appropriateness of the Hospice's scope of services, medical equipment and staffing on a quarterly basis.
Consult with Hospice administration regarding the Hospice's ability to meet the clients' needs and about future client care programs
Evaluate occurrence worksheets reported to risk management, assist in identifying safety hazards and make recommendations to improve the Hospice's service delivery.
Keep current with policies and programs of public health agencies that may affect client care
Represent Hospice to both the medical and non-medical community through presentations, meetings, and media to better its understanding and acceptance by the community
Monitor employee health and advise the Program Director on employee health policies
Comply with the policies, rules, and regulations of and with federal, state and local laws and rules and regulation of any governmental agency or public authority and the appropriate accrediting agencies
