The Palliative Care RN works collaboratively with patients, families, and interdisciplinary teams to develop individualized care plans that address physical, emotional, psychological, and spiritual needs.\n\nKey Responsibilities:\n\nAssess and manage patients’ physical, emotional, and psychological symptoms, including pain, fatigue, nausea, anxiety, and depression.\nCollaborate with interdisciplinary teams, including physicians, social workers, chaplains, and therapists, to develop and implement comprehensive palliative care plans.\nProvide education and support to patients and families regarding illness management, treatment options, and end-of-life care decisions.\nAdvocate for patients and families, helping them navigate complex medical decisions, treatments, and care options.\nDevelop and maintain individualized care plans focused on symptom control, comfort, and quality of life.\nAdminister medications and treatments, including palliative therapies such as pain management, comfort care, and symptom control.\nMonitor and evaluate patients’ progress, adjusting care plans as needed to meet changing needs and preferences.\nProvide emotional support and counseling to patients and families, addressing concerns related to illness, grief, and end-of-life care.\nSupport advanced care planning, including advance directives, hospice referrals, and coordination of care across settings.\nEducate patients, families, and caregivers about palliative care services, goals, and resources available.\nCollaborate with other healthcare professionals to ensure continuity of care and smooth transitions between care settings.\n\n\nQualifications:\n\nEducation: Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.\nLicensure:\n\nValid Registered Nurse (RN) license in the state of practice.\n\n\nCertification:\n\nBasic Life Support (BLS) certification required.\nAdvanced Cardiac Life Support (ACLS) preferred.\nCertified Hospice and Palliative Nurse (CHPN) preferred.\n\n\nExperience: Minimum of 2-3 years of experience in acute care, oncology, or hospice nursing preferred.\nWellspring Nurse Source Job ID #31257329.
