    NPORT Staffing is seeking a dedicated and skilled Registered Nurse (RN) to join our healthcare team at a skilled nursing facility focused on rehabilitation services and post-acute care. The RN will play a critical role in delivering high-quality compassionate nursing care to residents, ensuring compliance with clinical standards and supporting overall resident well-being in a skilled nursing environment.

    Key Responsibilities:

    Provide direct nursing care to residents in accordance with individualized care plans and physician orders and facility protocols

    Monitor and assess document residents' condition, reporting any changes in health status to the appropriate medical professionals.

    Administer medications and treatments and therapies in a safe and timely manner.

    Collaborate with physicians, therapists, social workers, and other healthcare team members to ensure coordinated and effective care.

    Maintain accurate, timely, and complete documentation in resident medical records

    Supervise and support Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs), ensuring quality of care and adherence to best practices.

    Maintain accurate and complete nursing documentation in compliance with the state and federal regulations

    Uphold infection control, safety, and patient rights standards at all times.

    Educate residents and their families on health conditions, care procedures, and post-discharge instructions when applicable.

    Qualifications:

    Active and unencumbered Registered Nurse (RN) license in the state of New York.

    Current CPR/BLS Certification (required).

    Minimum of one year of experience in a skilled nursing facility or long-term-care setting preferred.

    Strong clinical assessment, communication, and critical-thinking and communication skills.

    Ability to lead, delegate, and work effectively within an interdisciplinary team.
