    Position Overview

    What Will I Do?

    You will observe your clients, implement individualized treatment interventions, and administer appropriate case management when necessaryYou will provide group and family sessions in the school, home, and community under the direction of the Mental Health Professional (MHP) and participate as needed in treatment team meetings that offer support and input from all areas of the client's life.You will coordinate care with other service systems and assist with the delivery of in-service training and liaison activities with school staffYou will record client data, track progress on treatment objectives, and complete required chart documentation and quarterly chart audits as assigned by your supervisor.You will meet with the MHP for staff meetings and individual and group supervision as required.You will maintain a high level of professional confidentiality standards and uphold all regulatory licensing requirements of the position.You will attend training courses per program requirements.

    What You'll Do

    Assesses, monitors, and documents patient conditions, responses to treatment, and changes in status throughout the continuum of care.

    Administers medications, treatments, and nursing interventions in accordance with provider orders, established policies, and professional standards of practice.

    Qualifications

    Education: Associate Degree in Nursing or RN Diploma-Required. Bachelor’s Degree in Nursing-Preferred

    Licensure: Current, active, and unrestricted Registered Nurse (RN) license in the state of employment or a multi-state compact license, as applicable.

    Certifications: Basic Life Support for Healthcare Providers required and Crisis Prevention Institute required as a part of the general hospital orientation.

    The PHP Clinician is expected to spearhead day-to-day program activities, ensuring the timely completion of scheduled clinical events and assignments and the implementation of continuing care plans. The role demands seasoned experience in supervising a diverse clinical staff and monitoring all clinical activities for optimal patient service delivery.

This is a hybrid role — you will be in-office M–Th during program hours (9:30 AM–3:30 PM) to lead the clinical team and serve clients directly, with the remaining hours completed remotely on your schedule (30 hours total). That means assessments, note review, treatment planning, and QA work can be done from home — no commute required for those tasks.

    • Develop and review individualized treatment plans (ISPs)  | • Complete monthly ISP reviews (all 6 ASAM dimensions) 
    • Conduct continued stay reviews and MCO authorizations  | • Complete discharge summaries and step-down referrals 
    • Coordinate weekly psychiatric/medication management  | • Review and approve PHP Facilitator progress notes 
    • Conduct chart audits — DBHDS/DMAS compliance  | • Maintain documentation tracker and due-date log 
    • Co-facilitate PHP groups as needed for coverage  | • Manage referrals to/from higher and lower levels of care 
    • Conduct prescreens for program admission  | • Provide 24/7 on-call crisis support (shared rotation)
