Responsibilites: * Conducts ongoing and individualized nursing care duties as assigned and as dictated by the patient's condition in accordance with physician orders and facility protocols. * When administering PRN medications, assess for confirmed med for PRN, documents need (rationale) and its effectiveness in the medical record. * Reports change in patient physical, mental status or episodes of seclusion or restraint to nurse manager or supervisor during each shift. * Ensures family and physician notification is completed and documented if a patient is transported to Emergency Room, Injured, Falls or is Placed in Seclusion or Restraints. * Collaborates with other team members in the development, implementation, and evaluation of an individualized plan of care. * Performs treatments and therapies as ordered by physician in an appropriate and timely fashion. * Documents patient care, findings, and psychosocial responses to nursing interventions. * Documents PRN medications given, reason and effect * Initiates emergency measures according to age appropriate CPR and client resuscitation protocols. * Participates in emergency crisis interventions according to age appropriate techniques. * Maintains confidentiality related to patient, family, client facility and staff in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
