Job Summary
Reporting to the Director of the Empowerment, Healing, and Wellness Center, Case Managers play a crucial role in the intake and discharge processes of our clients. They are responsible for executing client action plans to support individuals on their journey to empowerment and healing. Case Managers offer compassionate guidance to clients and their children, fostering a safe and nurturing environment.

Responsibilities
Document dietary and allergen requirements for clients and their children.Ensure access to essential toiletries, food, bedding, and other necessities for clients.Provide support and reassurance to clients while developing and addressing client action plans.Maintain up-to-date knowledge of the legal and social service systems at the county and New York State levels.Communicate client needs with team members and relevant service providers.Keep case management files updated using the agency's Empower program.Stay informed about local social services, rental procedures, tenant rights, and responsibilities.Utilize the UNITAS referral and Safe Shelter collaboration web-based tools to assist clients as required.Document services provided and submitted reports as assigned.Assist clients with daily routines and shelter chores.Provide transportation to clients for appointments as necessary.Maintain a clean reception area and perform light cleaning of the shelter to ensure a well-maintained program space.Attend YWCA NENY staff meetings and mandatory training.Assist and cover for Hotline Operators by answering calls and offering empathetic and trauma-informed guidance to clients when needed.Perform other duties as assigned.Required Skills and Qualifications
An associate degree in human services or a related field preferred.Two or more years of experience in crisis-centered or intervention work.Proficiency in Microsoft Office (Word and Outlook).Effective and clear communication skills through case notes.Ability to work and communicate with clients who may have animals.Physical Requirements
Extended periods of desk work.Ability to lift 25 pounds without assistance.Capability to work in a noisy environment.Occasional bending, standing, reaching, and stair climbing.Driving of own vehicle.Additional Requirements

Job SummaryThe Shelter Benefit (DSS) Specialist provides vital support and advocacy to domestic violence survivors in our shelter. This role includes assisting clients with Department of Social Services (DSS) paperwork, coordinating benefits, maintaining accurate records, and ensuring clients' basic needs are met. The DSS Specialist also provides emotional support, facilitates access to community resources, and fosters connections with local agencies.

Job Summary
YWCA NENY Rapid Rehousing program serves individuals with varying levels of mental health diagnoses, women recovering from addiction, those recently released from incarceration, individuals living in extreme poverty, chronically homeless individuals and other marginalized populations. Reporting to the Rapid Rehousing Coordinator, the Case Manager is responsible for a caseload of approximately 30 participants/households who are homeless or at risk of homelessness due to domestic violence.

Compensation: $19 - $21.50 Hourly.
Responsibilities:
Conduct intake screening with client once referral received to determine housing needs.Assess client’s physical and mental needs and refer for assistance as needed.Assist with household budgeting and lease management for apartments. Refer individuals for financial counseling as needed.Provide ongoing case management as needed after family has found housing, to help maintain safety, security and mediate landlord tenant dispute as needed. Case Management may be located in client’s home (within the capital region) or YWCA NENY office.Encourage and provide guidance to clients with services such as social services, SSI/SSDI, finding employment, continuing education, obtaining childcare etc. Participate in apartment inspections with clients.Maintain files and case notes (empower database data entry) for individuals/families in the program. Participate in conferences, staff meetings, trainings. Other duties as assigned.Required Skills/Abilities:
Knowledge and experience managing program budget and expenses.Knowledge and experience with NYS tenant and/or landlord rights.Knowledge of Housing and Urban Development (HUD) requirements and standards.Qualifications:
1-4 year’s experience working with individuals needing assistance with low-income housing preferred.Associates degree in Human Services or related field preferred.Valid NYS Driver’s license and ability to use own vehicle for work related travel.Proficiency with Microsoft Office, Internet use, and database.Physical Requirements
Prolonged periods of sitting at a desk.Ability to use computer and necessary agency software.Able to lift up to 25 pounds without team assistance.Occasional bending, standing, reaching, stair climbing.Additional Requirements
