    Job Summary We are seeking a compassionate, organized, and dedicated Care Coordinator to join our team in supporting individuals living with mental health and/or substance use disorders. As a key member of our care team, you will play a central role in helping individuals work toward their goals, access resources, and overcome life’s challenges. This position is ideal for someone who is deeply passionate about making a difference, skilled in communication and documentation, and committed to walking alongside others in their recovery and wellness journey. Essential Duties and Responsibilities • Build trusting, respectful relationships with individuals, meeting them where they are emotionally and physically. • Develop, implement, and monitor community treatment plans in collaboration with people served, natural supports, and recovery team. • Coordinate support around accessing mental health, substance use, housing, medical, vocational, and other community services utilizing the CCS Service Array. • Advocate for individuals’ needs and ensure timely connection to appropriate supports. • Maintain regular communication with people served through in-person visits, phone calls and written follow-ups. • Coordinate with healthcare providers, therapists, case managers, and other community partners • Track progress, update treatment plans, and document interactions accurately and thoroughly • Empower individuals to build on their strengths and pursue self-identified goals. • Provide psychosocial rehabilitation, symptom monitoring and management, and supportive coaching to people served • Establish individuals’ eligibility for CCS by using the Mental Health/AODA Functional Screen • Respond with empathy and professionalism to crises and setbacks. • Full-time hours 8-4pm, some flexibility, no nights, or weekends. The Clinical Milieu Supervisor is focused on supporting youth and guiding staff in creating a positive, motivating treatment milieu. This role supports supervising staff, promoting the agreements and assumptions of Dialectical Behavior Therapy (DBT), and fostering staff’s clinical skill development. This role includes providing critical support to staff, helping them navigate the evolving nature of their daily responsibilities while ensuring a collaborative and supportive program culture. This includes teaching and role modeling skills, staff supervision, providing training, and serving as role model for staff. Essential Functions: Provide clinical leadership for operations team and role model for Lead Youth Treatment Specialists and Youth Treatment Specialists. Supervise the milieu implementation of therapy by supporting therapeutic interactions through coaching staff on skill use throughout daily activities, modeling appropriate behavior, teaching and reinforcing use and generalization of skills through a token economy system.     Position Summary: The CLTS Waiver Program is one of Wisconsin’s Home and Community Based Services (HCBS) Medicaid Waiver programs, federally authorized under § 1915(c) of the Social Security Act. HCBS waiver programs provide Medicaid funding that may be used to fund allowable support and services that promote individuals in the waiver access and inclusion in their home and community. Eligible participants include youth between the ages of 0-22 with developmental, severe emotional, and/or physical needs, who are also functionally and financially eligible for Medicaid. The Support and Service Coordinator (SSC) coordinates and facilitates access to all services and support, both formal and informal, which are needed by the child and family to meet their identified needs. Support and service coordination assists a child or youth and his or her family to achieve an inclusive, interdependent, and self-empowered life. The child’s or youth’s SSC uses their knowledge of available programs, resources, and services to create an individualized service plan that best supports the child and family in pursuit of their desired outcomes. This includes connecting families to support throughout the community and promotes family members’ self-determination and involvement in all facets of community life. A primary responsibility of the SSC is promoting the child’s health, safety, and welfare in their home and community.
