    Exciting opportunity to join our growing practice and work both in an outpatient clinic and on our new Medical Van! The CMA or RMA will provide direct patient assistance in an outpatient clinic and on our Medical Van, serving adults with Intellectual and Developmental Disabilities.

    Medical Assistant Responsibilities:

    Participates in pre-visit planning by identifying applicable preventative health measures; flagging and following up on overdue labs, imaging, and consultations; acquiring outstanding medical documentation; and managing patient correspondence with the provider and care team.Documents/scans into the electronic medical record for the provider and care team.Documents the following comprehensive health assessment items for the provider and care team to include immunizations and screenings; medical, familial, and social history including cultural and personal preferences; behaviors affecting health; and advanced care planning.Assists the provider in the exam room with collection of specimens, procedures and patient exams as needed.Provides instruction and educational resources to the patient and/or caregiver under the provider’s direction. Executes proper technique in patient interactions.Arranges prescription refills, prior authorizations, and pre-certifications as needed.Efficiently maintains patient flow throughout Adult Health Services. Appropriately cleans and organizes exam rooms for all team patients including replenishing supplies from the storeroom, as well as cleaning and sterilizing appropriate equipment.Responsible for other activities as directed, as well as covering for other positions when needed (i.e., care coordination, medical records, reception, etc.).Participates in quality improvement activities as needed.Attends all required meetings.If interested/Optional: Drive Medical Van with a provider to provide healthcare visits in the community for our patients.

    Medical Assistant Qualifications:

    Required:

    Graduation from an accredited Medical Assistant certificate program;Current Certification - CMA or RMAExternship/Internship;Knowledge of electronic medical record software;Experience performing EKG and Venipuncture;

    Summary of Receptionist Position:

    WIHD is recruiting a full time Medial Receptionist to serve as the primary point of contact for patients, caregivers, and visitors, playing a critical role in ensuring efficient front desk operations, patient access, and overall clinic flow.     The medical receptionist is responsible for high-volume scheduling, patient registration, phone management, and real-time coordination of front desk activities. The Medical Receptionist ensures accurate patient information, supports provider workflows, and contributes to a positive, organized, and patient-centered experience.

    MAJOR RESPONSIBIITIES

    Patient Access & Scheduling Coordination

    Manage high-volume appointment scheduling via phone, in-person, and electronic communication. Schedule, reschedule, confirm, and cancel appointments in alignment with provider templates and visit types. Monitor no-shows and cancellations and assist with rescheduling to support continuity of care. Participate in high-volume scheduling projects (e.g., provider transitions, backlog reduction, clinic optimization initiatives). Ensure accurate appointment notes, including visit type and required documentation.

    Front Desk Operations & Patient Experience

    Greet patients and caregivers in a professional, courteous, and efficient manner. Perform check-in and check-out processes, ensuring all required documentation and information are complete. Direct patient flow and coordinate with clinical staff to maintain efficient clinic operations. Assist patients with forms, wayfinding, and general inquiries. Maintain a clean, organized, and safe front desk and waiting area environment.

    Communication & Phone Management

    Answer and manage high-volume incoming calls in a timely and professional manner. Route calls, messages, and tasks to appropriate staff based on patient needs. Document all patient interactions accurately in the EMR. Provide clear instructions to patients regarding appointments, documentation, and next steps.Escalate urgent or complex issues appropriately.

    Documentation & EMR Management

    Enter and update patient demographic and insurance information accurately in the EMR. Take and upload updated patient photos into EMR charts as required.

    Administrative & Operational Support

    Collect and distribute incoming clinic mail and correspondence to Medical Records Support coordination of clinic-wide administrative tasks and front desk workflows. Support internal tracking processes and basic reporting activities as directed.

    Team Collaboration & Process Improvement

    Participate in team meetings, training sessions, and quality improvement initiatives. Contribute ideas to improve front desk workflows, patient access, and operational efficiency. Collaborate with providers, care coordinators, and leadership to support clinic operations. Maintain flexibility to support evolving clinic and organizational needs.

    QUALIFICATIONS

    Education: High School Diploma or equivalent required.

    Experience:

    Minimum of 2 years of experience in customer service, administrative support, or healthcare front desk operations. Experience working in a healthcare or outpatient setting preferred. Strong computer skills, including data entry, email, and basic Microsoft Office (Word, Outlook, Excel). Familiarity with Electronic Medical Records (EMR/EHR) systems and patient registration workflows. Experience working with individuals with intellectual and/or developmental disabilities preferred.

    General Attributes:

    Ability to accurately document and manage patient information in a fast-paced environment. Ability to multitask effectively, including working on a computer while simultaneously communicating on the phone or in person. Strong organizational, time management, and problem-solving skills.

    Essential Functions of the Nurse Practitioner:

    Direct Patient Care

    Directs medical assistants, care managers, nurses, and care coordinators in order to provide efficient and effective patient centered care, as described in the team based policy.Provides patient centered primary medical care by taking full patient histories and performing examinations in order to develop and implement a plan based on diagnostic and demographically specific evidence based guidelines. This process includes, but is not limited to: ordering and reviewing diagnostic labs and imaging; ordering and reviewing consultations; administering procedures including shots, as needed, communicating with patients and caregivers regarding their individualized care management plan; collaborating with internal and external care providers; providing appropriate healthcare instruction and education; and documenting all healthcare communication and action in the electronic medical record.Engages in pre-visit planning with the care team in order to review and plan for the day’s patient panel and their specific care needs.Triages all patients requesting service in a timely manner regardless of the medium (e.g., phone call, patient portal, in-person, clinical advice task, etc.) Responds to emergencies as needed.Provides back up coverage to other provider panels as needed.Provides extended hours care, as well as after hour’s clinical advice as determined by the Adult Health Services schedule.Follows office policy and procedures and demonstrates knowledge of the requirements of a NCQA Patient Centered Medical Home.Perform any other activities as needed to ensure the best patient care is provided.

    Care Management

    Collaborates with the care manager in stratifying primary care patients according to medical, behavioral, and social determinants of health using population management tools in the electronic health record. Also stratifies patients according to the number and type of internal/external referrals and their overall healthcare utilization to identify high risk primary care patients in need of care management plans.Develops and executes individualized care management plan using comprehensive health assessments with the care team in order to provide appropriate patient centered care that meets the needs and goals of the patient.Consistently revaluates the individualized care plan and a patient’s progress towards their goals and assesses barriers when objectives are not met.

    Quality Improvement

    Participates in clinical quality initiatives that are based on State or Federal programs (i.e., Quality Payment Program, MIPS, MU), as well as organizational initiatives. Agrees to data transparency, as well as the sharing of and accountability for clinical quality metrics.

    Meetings

    Participates in quality improvement meetings and activities as needed.Participates in care management meetings.Attends all required medical home and Adult Health Services meetings.

    Qualifications:

    Education Level: Graduation from an approved Nursing Program with a Master’s degree in a relevant area of specialization (i.e. Family Practice); Possession of a license to practice medicine in New York State; Board Certified Family Nurse Practitioner

    Knowledge of: Electronic medical records; Awareness of current local, state, and federal laws governing the delivery of care.

    Kind and Length of Experience: Minimum of three years’ experience as a Nurse Practitioner in the outpatient setting. Preferred knowledge or experience caring for individuals with developmental disabilities.

    About the Role

    We are seeking a detail-oriented and proactive Care Coordinator to support our Chronic Care Management (CCM) program. In this role, you will work directly with patients, caregivers, and providers to help close care gaps, improve health outcomes, and ensure patients receive timely, coordinated care.

    You will be responsible for conducting structured CCM outreach calls, reviewing patient charts, coordinating follow-up care, and documenting all interventions in the electronic medical record (EMR).

    About Us:

    Child Welfare Services at WIHD provides an array of services, supports and advocacy for children and families involved with the child welfare system in Westchester County. These services include clinical assessments of children and their birth parents, pediatric medical care, educational coordination and advocacy, and community-based parent training services for parents with intellectual disabilities and services for families reunifying from foster care or at risk for foster care placement.

    Summary of Socio Therapist:

    Plan, provide, and participate in therapeutic recreational activities and outings for and with children and families involved in the child welfare system. Conduct group drop-in activities in the community and/or virtually encourage prosocial interests and talents. Coordinate closely with families and others working with the child to plan recreational activities that fit the child’s treatment and social needs and meet the family’s ability to participate. Help families to identify no cost and low-cost recreational resources in their communities that fit their and their child’s needs. Encourage and support independent living skills for older youth.

    Essential Functions:

    Plan, conduct, and actively participate in therapeutic recreational, social, and physical activities with families and children at risk of foster care placement and/or replacement; participate in activities with older youth that support independent living skills.Provide in-home and community-based support and services for families involved with the child welfare system, including group-based activities and identifying no cost/low-cost resources; assist in creating and maintaining a resource guide of community-based recreational resources.Participate in meetings and collaborate with program staff and others to consult on children’s therapeutic needs and family services.Make telephone contacts to arrange appointments, support planning (including budgeting), and complete appropriate documentation of activities.Assist with practical tasks, including dropping off supplies and/or transporting families to appointments.Take responsibility for safe driving with children and for the maintenance of agency vehicles.

We provide the range of diagnostic and rehabilitative services including, diagnostic audiology, electrophysiologic measures (ABR & OAE), auditory processing testing, hearing aids and bone conduction devices. Services are provided to infants, children, and adults, some of whom have neurodevelopmental disabilities.

    Audiologist Qualifications:
