Position Summary
The Medical Assistant provides patient care in the health center setting and assists the provider (s) and care teams in the management and coordination of patient care, with an emphasis on prevention and health maintenance.
Required Qualifications

Perform medical assistant duties, preparing rooms, rooming patients, record medical histories, vital signs and performing venipuncture and immunizations.Assist the medical team including, and not limited to the Health Care Team, physicians, family nurse practitioners, and physician assistants, Health Educators and Integrated Care Coordinators.Monitor Patient Centered Health Home goals.Encourage patient’s wellbeing through health promotion.Refer patients to primary healthcare team, as appropriate.Keep accurate records for reporting to various funding and inspecting agencies.Maintain immunization files for the county registry.Travel when necessary to support operational needs.Work one weekend per month as needed to support operational needs.Performs other duties as assigned within the medical assistant's scope of practice.May be required to work evenings, weekends and holidays.Benefits

Position Summary
The Health Educator supports the Primary care team by providing health education to WellSpace Health patients. The ultimate goal of health education is to provide WellSpace Health patients with the necessary knowledge and skills to improve their well-being and live long healthy lives using a patient centered health approach. These services include health education to prevent and/or manage chronic and/or infectious diseases.
Required Qualifications
· High School diploma or equivalent, Bachelor’s degree preferred.
· Certified Health Education Specialist (CHES) preferred.
· Health education/community health/public health degree or experience.
· Fluency in additional languages preferred (E.g Spanish, Farsi, Punjabi or Hmong).

Essential Responsibilities
· Meet with each patient individually and/or in a group setting and provide health education.
· Implement health education programs within Primary Care Department.
· Implement a range of health education services, including establishing effective referral, scheduling and logistical support, and facilitating use of community services to promote the delivery of health education services.
· Collaborate with the patient and care team in the development of an individual health action plan and support the patient in the execution of their health goals.
· Refer patients to primary healthcare team, as appropriate.
· Compose and document progress notes daily into an electronic patient care system and maintain detailed data records daily.
· Provide support with the development of high quality, culturally appropriate health promotion materials.
· Track patient progress and program data daily for reporting.
· Attend regular monthly meetings with Health Centers and team.
· Perform other duties as assigned.

The Suicide Prevention Crisis Line Program Manager oversees all aspects of the crisis program including staffing, maintaining records, training of department staff, preparing reports and making presentations to the community.

Position Summary
The Clinical Support Specialist (CSS) will monitor patients in a recuperative care environment where unhoused patients come to heal from illnesses. This role will perform vitals, observe patients, monitoring patient self-medication under the protocol of the program and report changes to the nursing staff. The CSS will support patients with coordination of medical appointments, medication refills, and other medical supplies as necessary as well as coordinatination across multidisciplinary teams.
Required Qualifications
· High School Diploma or equivalent required.
· Medical Assistant or CNA training/certification highly preferred, OR an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job.
· Minimum of 1 year of patient care experience in a medical facility, substance use disorder treatment facility or behavioral health environment.
· Organized and attention to detail.
· Must have current BLS Certification or willingness to obtain within 90 days from date of hire.
· Ability to document and maintain detailed records.
· Ability to write clearly, including appropriate punctuation and grammar.
· Experience documenting in an Electronic Health Record (EPIC a plus).
· Proficiency in basic Outlook, Word, Excel and Teams computer skills.
· Conduct patient reported screenings to understand clients' needs and to advocate to nursing and management.
· Excellent customer service experience, including active listening, problem-solving, and calmness in emotional or stressful situations. Ability to communicate patient reported symptoms and concerns to nursing staff.
· Experience with supporting patients with self administration of medication (“medication calls”) with detailed documentation.

· Relay patient vitals, symptoms, behaviors, progress, and risks to emergency services and multidisciplinary care teams.
· Encourage patient well-being through health promotion initiatives.
· Refer patients to primary healthcare teams, behavioral health, and substance use treatment programs, as appropriate.
· Assist patients with admission, transfer, and discharge procedures.
· Conduct medication management by performing medication calls, prompting patients, observing adherence, documenting related changes and reporting any anomalies or discrepancies.
· Perform weekly medication inventory audits.
· Verify the integrity of all medication deliveries and accurately document inventory and schedules in the Electronic Health Record (EHR) system.
· Conduct admission intake for patients, including preparing rooms and beds, coordinating meals, inventorying patient belongings, and performing initial patient reported screenings.
· Review and document patient observations and progress notes.
· Maintain housekeeping duties, such as post-meal cleanup and laundry, as needed, while adhering to internal controls and procedures.
· Monitor patient bed count and ensure facility safety.
· Provide 24/7 support and monitoring for patients experiencing homelessness and co-occurring disorders (mental health, chronic medical conditions, substance use disorders) in a high-acuity care facility.
· Ability to work holidays, weekends, and alternate shifts as needed to maintain required staffing levels.

Position Summary
Provides patient care in the office setting. The Case Manager II supports the program by providing case management service. The ultimate goal of all services provided is assisting the patient with obtaining an income, insurance and housing. These services include assistance with co-occurring disorders (mental health and substance abuse) with the expectation of improving current functioning level and skills acquisition through assessment, engagement, assisting with access to treatment including medical care with a primary care provider, rehabilitation and goals.

Position Summary
The EHR/IT Applications Trainer is responsible for the training, implementation support, and ongoing optimization of enterprise-level healthcare software applications, primarily the Electronic Health Record (EHR) and Electronic Practice Management (EPM) systems. This position serves as a key liaison between clinical/administrative end users and IT, ensuring smooth onboarding, training, and support for all EHR-related functionality.
In addition to training responsibilities, this role includes duties commonly assigned to an application support specialist or system analyst—such as building and maintaining training environments, validating workflows against live system builds, and participating in the design and testing of new workflows and configurations. The trainer must have a working knowledge of how to use the EHR and be familiar with its functionalities to guide users through optimized workflows and resolve system-related issues in collaboration with informatics and technical teams.
Required Qualifications
· Bachelor's degree in an academic or scientific field related to Information/ Systems Management or Technology, or the equivalent in training and experience.
· Minimum of one (1) year work experience in Electronic Health Records systems and training of providers and clinical staff support in these systems and other clinical support functions.
Fluency in additional languages preferred.· Experience working with Federally Qualified Health Centers (FQHCs) and their specific patient populations is preferred.

Position Summary
The Compliance Program Specialist plays a key role in supporting the organization’s full compliance program, including regulatory, OIG/corporate, accreditation, operational, and general business compliance. This position manages policies and assigned compliance projects across all compliance domains, coordinates committee logistics, tracks projects, and supports departmental operations.
As the primary administrator of the policy repository system, the Specialist ensures policies and patient-facing forms are accurately maintained, approved, and distributed. This role works closely with the Compliance Officer and cross-departmental teams to support project execution, reporting, and continuous improvement. Position Summary
The Health Educator supports the Primary care team by providing health education to WellSpace Health patients. The ultimate goal of health education is to provide WellSpace Health patients with the necessary knowledge and skills to improve their well-being and live long healthy lives using a patient centered health approach. These services include health education to prevent and/or manage chronic and/or infectious diseases.
Required Qualifications
· High School diploma or equivalent, Bachelor’s degree preferred.
· Certified Health Education Specialist (CHES) preferred.
