    POSITION SUMMARY

    The Field Supervisor (CNA) will play a vital role in providing compassionate care and support to residents in their daily living activities.     PRIMARY RESPONSIBILITIES

    POSITION SUMMARY

    The Medication Technician (Med Tech) will be responsible for ensuring safe, accurate, and timely medication management for residents. This full-time role is essential to supporting the care team by organizing resident medications, auditing medication compliance monthly, and monitoring for changes in resident condition related to medication use. The Med Tech will collaborate closely with Caregivers, Field Supervisors, Lead Field Supervisors, and Executive Administrators to maintain high standards of resident safety, health, and well-being. Compassion, attention to detail, and strong organizational skills are key for success in this role.



    PRIMARY RESPONSIBILITIES

    Medication Management

    Organize and label medications for residents according to physician orders.Audit resident medications monthly to ensure proper organization, labeling, and compliance with physician orders.Verify accuracy of new medication deliveries from pharmacies and report any discrepancies.Assist in training caregivers on safe medication handling and documentation.

    Resident Care & Monitoring

    Observe residents for side effects, drug interactions, and any changes in condition.Take and record vital signs as directed.Assist with collecting lab specimens when needed.Support caregivers with activities of daily living as appropriate.Report concerns about residents to the Executive Administrator promptly.

    Collaboration & Compliance

    Partner with the Field Supervisors and Lead Field Supervisors to ensure staff adherence to medication safety standards.Assist in preparing resident files and medications during admissions and discharges.Maintain clean and organized medication storage areas.Uphold all state and facility regulations regarding medication management.



    QUALIFICATIONS

    Position Summary

    The Occupational Therapist (Per Diem/PRN) is responsible for providing high-quality, patient-centered therapy services in the home setting. The OT evaluates, plans, and implements individualized treatment programs to improve patients ability to perform daily activities, promote safety, and enhance overall well-being. This position works collaboratively with the interdisciplinary home health team to ensure effective, coordinated care.


    Key Responsibilities

    Conduct comprehensive assessments to evaluate functional abilities, home environment, and rehabilitation potential.Develop individualized care plans and treatment goals that address physical, cognitive, and psychosocial needs.Provide therapeutic interventions to improve strength, coordination, fine motor skills, and independence in activities of daily living (ADLs).Educate patients and caregivers on adaptive techniques, safety strategies, energy conservation, and use of assistive devices or equipment.Recommend and coordinate durable medical equipment (DME) and environmental modifications as needed.Document all evaluations, treatments, and progress notes in compliance with Wellpointe Home Health policies, state, and federal regulations.Collaborate with nurses, physical therapists, speech therapists, and medical social workers to ensure a cohesive plan of care.Participate in case conferences, team meetings, and quality improvement initiatives as requested.Communicate effectively with physicians and care coordinators regarding patient progress and needs.


    Qualifications

    Education: Bachelors or Masters degree in Occupational Therapy from an accredited program.Licensure: Current and valid Occupational Therapy license in the State of California.Experience: Minimum of one (1) year of occupational therapy experience preferred; home health experience strongly desired.Certifications: Current CPR certification.Other Requirements:Valid drivers license and reliable transportation.Proof of automobile insurance.Strong organizational and time-management skills for per diem scheduling.

    POSITION SUMMARY

    The Executive Administrator is responsible for all aspects of resident Care Operations within assigned Fresno Guest Homes facilities. This role ensures that care services meet regulatory standards and align with company protocols, prioritizing resident well-being and satisfaction.


    The Executive Administrators primary focus is on resident care, including medication management and healthcare services coordination, directing Care Team members, compliance, and family communications to maintain high-quality care and resident/family satisfaction. While involved with recruiting, training, staffing/scheduling, HR, maintenance or non-care administrative functions, these functions are led and handled by different teams.


    CARE OPERATIONS (i.e. RESIDENT CARE)

    Involved with hiring process process for Care Team members; coordinate with Staff Development to facilitate ongoing training & development of Care Team members.Ensure all resident care services, including medication management and healthcare services coordination, are delivered in accordance with Fresno Guest Homes standards and state regulations.Oversee resident service snapshots and medication management, including coordination with pharmacy.Responsible for monitoring of resident condition changes and coordinating as necessary with healthcare services, including physicians, specialists, Home Health, Hospice, and DME.Oversee infection control measures, emergency preparedness, and risk management related to resident care.Respond effectively to emergencies and incidents.


    RESIDENT ENGAGEMENT

    Ensure that residents have access to activities, entertainment, and enrichment activities that promote joy, purpose, and personal fulfillment.Work with Staff Development to ensure that Care Team members are facilitating and implementing resident engagement and social interaction activities based on residents abilities and interests.Work with Staff Development and Care Team members to ensure that resident engagement opportunities are tailored to residents abilities and interests.Promote a vibrant community atmosphere by encouraging participation in social and recreational activities that foster meaningful daily interactions and overall well-being.


    COMPLIANCE & QUALITY ASSURANCE

    Ensure compliance with RCFE regulations by adhering to internal policies and practices.Oversee and prepare for licensing inspections; maintain positive working relationships with licensing staff.Maintain accurate resident records and documentation per regulatory requirements.Implement best practices and quality improvement initiatives to enhance care services.


    FAMILY & COMMUNITY RELATIONS

    Serve as the primary point of contact for families, providing regular updates on resident well-being and responding to family questions timely and effectively.Conduct family meetings and care conferences to ensure alignment on service delivery.Address family concerns with professionalism and empathy, using a solutions-based approach driven by active listening.Promote a culture of resident and family satisfaction through all resident/family touchpoints.


    FACILITY OPERATIONS

    FOOD SERVICE & DIETARY OVERSIGHT

    Ensure that meal service is consistent with company menu and standards, including adequate portion sizing and presentation.Address individual resident dietary needs, restrictions, modifications and preferences to promote nutrition, safety, and enjoyment.Liaise between Care Team and Food Service to ensure both orders and deliveries are timely and accurate.Monitor residents for dietary concerns, weight loss/gain, and hydration, implementing adjustments as necessary.


    QUALIFICATIONS

    QUALIFICATIONS

    High school diploma or equivalent required.Active CNA Certification requiredCriminal background clearance is required.Two years of experience (assisted living setting preferred).Knowledge of state elderly care regulations and procedures is highly valued.Reliable transportation.First Aid/CPR Certified.Strong organizational, prioritizing, and analytical skills.Pre-employment physical and TB Test required per state regulations.Ability to read, write, and follow oral and written directions.

    POSITION SUMMARY

    Responsible for assisting residents with their daily living activities and other care needs, as directed by the Executive Administrator and members of the Care Operations team. We seek a self-motivated, compassionate, responsible, and energetic individual to help foster a positive and supportive environment for our residents. The ideal candidate should be flexible and able to adapt to a work environment that requires frequent shifts in direction. Caregivers should have excellent communication skills with an expectation to clearly and timely express resident changes and concerns to the Executive Administrator.


    PRIMARY RESPONSIBILITIES

    Assist residents with activities of daily living (ADLs) such as bathing, grooming, dressing, toileting, and transferring.Assist residents with self-administration of medications.Engage residents with activities.Documentation/communication of resident's activities, ADLs, and changes.Prepare and serve meals according to the menu, with modifications for dietary needsPerform housekeeping tasks to maintain a clean and safe environmentSupport individuals with dementiaEnsure resident confidentiality


    QUALIFICATIONS

    Criminal background clearance (Required by State)Caregiving: 1 year experience in Assisted Living, Skilled Nursing, Home Care or other direct care experience (Preferred)Ability to work with older adults, including willingness to work with those with behavioral challengesExperience with dementia care and Hoyer lifts is a plus
