Responsibilities Provide high-quality nursing care to long-term care patients. Participate in ongoing professional development and training. Collaborate with healthcare teams to develop patient care plans. Monitor and document patient progress and changes in condition. Educate patients and families on health management and care plans. Adhere to safety and infection control protocols at all times. Utilize performance metrics to assess and improve care delivery. Engage in regular communication with patients and their families. Assist in the training and mentoring of new nursing staff. Manage time effectively to meet patient care deadlines. Participate in quality improvement initiatives within the facility. Maintain accurate and up-to-date patient records and documentation. Experience/Qualifications Valid RN license in New Jersey with long-term care experience. Proven experience in patient care and monitoring in long-term settings. Strong communication skills to engage patients and families effectively. Willingness to participate in ongoing professional development and training. Ability to collaborate with healthcare teams for patient care planning. Familiarity with infection control protocols and safety procedures. Experience in utilizing performance metrics for care improvement.
