    Job Summary

The Physical Therapy Assistant provides skilled, compassionate physical therapy services under the direction of a licensed Physical Therapist.     Essential Functions

    Patient Care

    Deliver physical therapy interventions as directed by a qualified Physical Therapist and in accordance with the physician's plan of care and the state Physical Therapy Practice Act.Treat patients to help relieve pain, maintain function, and support maximum safe performance.Lead patients through active and passive therapeutic exercises, muscle re-education, gait training, functional mobility activities, transfer training, and prosthetic training.Utilize physical agents such as heat, cold, water, light, electricity, ultrasound, and massage in alignment with state practice guidelines.Observe and report patient responses, progress, or changes in condition to the Physical Therapist and appropriate agency personnel.Instruct patients in the safe use and care of mobility and supportive devices, including wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.

    Communication & Documentation

    Document clinical notes and updates in a timely and accurate manner; ensure all documentation is incorporated into the patient's clinical record as required.Participate in interdisciplinary care discussions and collaborate with the hospice care team to support coordinated service delivery.Additional ResponsibilitiesAttend home care team meetings, in-service programs, and interdisciplinary group (IDG) meetings.Participate in quality assessment and performance improvement activities.Perform additional duties as assigned to support high-quality patient care.QualificationsGraduate of an accredited Physical Therapy Assistant program with an associate degree in science or applied science (approved by APTA).Current PTA certification or licensure as required by state law.Strong verbal, written, and organizational skills.Current CPR certification.Licensed driver with a reliable, insured automobile that meets organizational requirements.     Job Summary At VitalCaring, our team members transform lives and foster hope through genuine caring. As a Hospice Aide/CNA, you are a valued member of the interdisciplinary team, providing essential personal care and comfort to patients at the end of life. Working under the supervision of a Registered Nurse, you help maintain dignity, ease, and peace for patients and their families. Guided by our valuestrustworthy, capable, compassionate, proactive, and calledevery visit is an opportunity to make a meaningful difference.

    Essential Functions Direct Patient Care Perform assigned personal care tasks promptly and compassionately; report any tasks you are unable to perform to the Case Manager immediately. Provide personal hygiene care, including bathing (bed, sponge, tub, shower), back rubs, oral care, shampooing, and linen changes. Assist with toileting, elimination activities, dressing, and undressing. Help with ambulation and exercise as instructed by the hospice nurse or therapist. Offer support with bedpans, urinals, and light laundry needs. Provide observation and care to prevent skin breakdown, especially over pressure areas. Assist with self-administered medications as allowed under state regulations (e.g., reminders).

    Comfort, Nutrition & Environment Prepare nutritious meals and shop for groceries as assigned. Assist with feeding when needed. Maintain a clean, safe, and orderly patient living environment. Provide respite to the family/caregiver when appropriate. Demonstrate respect for the patient's home, surroundings, and personal needs.

    Monitoring & Reporting Observe and report changes in the patient's condition, mood, or environment to the Case Manager. Be aware of caregiver dynamics and interpersonal issues within the home. Take and record vital signs (temperature, pulse, respirations, blood pressure) when ordered and when competency has been demonstrated.

    Job Summary

The Physical Therapy Assistant provides skilled, compassionate physical therapy services under the direction of a licensed Physical Therapist. Guided by our valuestrustworthy, capable, compassionate, proactive, and calledyou will play an important role in supporting comfort, function, and quality of life for hospice patients and their families.     Essential Functions

    Patient Care

    Deliver physical therapy interventions as directed by a qualified Physical Therapist and in accordance with the physician's plan of care and the state Physical Therapy Practice Act.Treat patients to help relieve pain, maintain function, and support maximum safe performance.Lead patients through active and passive therapeutic exercises, muscle re-education, gait training, functional mobility activities, transfer training, and prosthetic training.Utilize physical agents such as heat, cold, water, light, electricity, ultrasound, and massage in alignment with state practice guidelines.Observe and report patient responses, progress, or changes in condition to the Physical Therapist and appropriate agency personnel.Instruct patients in the safe use and care of mobility and supportive devices, including wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.

    Communication & Documentation

    Document clinical notes and updates in a timely and accurate manner; ensure all documentation is incorporated into the patient's clinical record as required.Participate in interdisciplinary care discussions and collaborate with the hospice care team to support coordinated service delivery.

    Additional Responsibilities

    Attend home care team meetings, in-service programs, and interdisciplinary group (IDG) meetings.Participate in quality assessment and performance improvement activities.Perform additional duties as assigned to support high-quality patient care.

    Qualifications

    Graduate of an accredited Physical Therapy Assistant program with an associate degree in science or applied science (approved by APTA).Meets hospice personnel qualifications per 418.114(b)(8).Current PTA certification or licensure as required by state law.Strong verbal, written, and organizational skills.Current CPR certification.Licensed driver with a reliable, insured automobile that meets organizational requirements.
