    Job Summary

The Physical Therapy Assistant provides skilled, compassionate physical therapy services under the direction of a licensed Physical Therapist. Guided by our values—trustworthy, capable, compassionate, proactive, and called—you will play an important role in supporting comfort, function, and quality of life for hospice patients and their families.     Essential Functions

    Patient Care

    Deliver physical therapy interventions as directed by a qualified Physical Therapist and in accordance with the physician's plan of care and the state Physical Therapy Practice Act.Treat patients to help relieve pain, maintain function, and support maximum safe performance.Lead patients through active and passive therapeutic exercises, muscle re-education, gait training, functional mobility activities, transfer training, and prosthetic training.Utilize physical agents such as heat, cold, water, light, electricity, ultrasound, and massage in alignment with state practice guidelines.Observe and report patient responses, progress, or changes in condition to the Physical Therapist and appropriate agency personnel.Instruct patients in the safe use and care of mobility and supportive devices, including wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.

    Communication & Documentation

    Document clinical notes and updates in a timely and accurate manner; ensure all documentation is incorporated into the patient's clinical record as required.Participate in interdisciplinary care discussions and collaborate with the hospice care team to support coordinated service delivery.

    Additional Responsibilities

    Attend home care team meetings, in-service programs, and interdisciplinary group (IDG) meetings.Participate in quality assessment and performance improvement activities.Perform additional duties as assigned to support high-quality patient care.

    Qualifications

    Graduate of an accredited Physical Therapy Assistant program with an associate degree in science or applied science (approved by APTA).Meets hospice personnel qualifications per §418.114(b)(8).Current PTA certification or licensure as required by state law.Strong verbal, written, and organizational skills.Current CPR certification.Licensed driver with a reliable, insured automobile that meets organizational requirements.

    Job Summary

As a Care Transition Coordinator (CTC), you will serve as the primary liaison between referral sources, patients and families facing end-of-life or home health needs, and the clinical team. You will support seamless transitions of care by conducting effective sales calls, strengthening relationships, educating partners and families, and coordinating timely referrals. Every encounter reflects our values—trustworthy, capable, compassionate, proactive, and called.

    Essential Functions

    Referral Development & Sales Execution

    Serve as a liaison between referral sources, patients and caregivers, and the clinical team to ensure seamless coordination through the care transition process.Execute effective, structured sales calls using pre-call planning, rapport building, probing questions, objection handling, and closing techniques.Identify and meet the needs of referral partners through consistent communication and service excellence.Successfully execute weekly, monthly, and quarterly strategies to increase market share and drive referral growth.Initiate, manage, and document consistent sales activities across multiple contacts within each referral source.Achieve established referral and admission goals and contribute to overall organizational growth.

    Relationship Building & Collaboration

    Build and maintain collaborative working relationships with referral partners, community stakeholders, and internal teams.Represent the organization professionally and tactfully across a variety of healthcare settings.Work in close partnership with clinical and operational leaders to support timely admissions and appropriate patient onboarding.Participate in community outreach, education, and events to increase awareness of services.

    Care Coordination Support

    Engage patients, caregivers, and hospital care teams to facilitate appropriate, timely home health or hospice transitions.Communicate key patient needs, risks, and updates to internal teams to support safe care delivery.Maintain accurate documentation in CRM and EMR systems to support communication and continuity of care.

    Professional Standards

    Demonstrate strong time management, organization, and follow-through to meet expectations.Maintain accountability to regulatory requirements, agency policies, and compliance standards.Participate in required meetings, in-services, and professional development activities.Perform additional duties as assigned.

    Qualifications

    Bachelor's degree in Healthcare, Business, Marketing, or a related field preferred; equivalent experience accepted.Minimum two years of healthcare or related sales experience; hospice or home health experience preferred.Demonstrated ability to interact professionally with individuals at all levels, both internally and externally.Strong communication, relationship-building, and decision-making skills.Proficient with Microsoft Office (Word, Excel, Teams, Outlook) and CRM systems.Ability to work independently, prioritize competing demands, and manage a dynamic schedule.Reliable transportation with current auto liability insurance.

    Environmental / Working Conditions

    Works primarily in the field conducting visits with referral sources across hospitals, skilled nursing facilities, physician offices, and other community settings.Requires local travel within the service area; occasional adverse weather conditions may be encountered.Flexible schedule required to meet the needs of referral partners.May occasionally participate in patient or family meetings to support education and coordination.

The Palliative Care Nurse Practitioner (NP) provides high-quality, compassionate, patient-centered care to individuals with serious, chronic or life-limiting illnesses in the comfort of their homes. This role focuses on symptom management, advance care planning, identifying goals of care, and coordination of care with existing medical providers and other interdisciplinary services. High focus on supporting patients and families through complex medical and emotional needs. The NP functions with a high degree of clinical autonomy and collaborates closely with patients established medical team and members of the palliative care team.

    Essential Job Functions/Responsibilities

    Clinical Care

    1) Conduct comprehensive in-home assessments, including physical, psychosocial, and spiritual evaluations.

    2) Develop, implement, and adjust individualized care plans focused on symptom control, quality of life, and patient goals.

    3) Provide expert management of pain, shortness of breath, nausea, anxiety, and other distressing symptoms in collaboration with patients medical team.

    4) Prescribe medications, order diagnostic tests, and perform appropriate clinical interventions within scope of practice as needed.

    5) Evaluate treatment responses & modify care plans in collaboration with medical and interdisciplinary team.

    6) Occasional on-call responsibilities may be required.

    7) Provide advanced care planning education and discussions, including but not limited to goals of care and advanced directives.

    Care Coordination

    1) Collaborate with physicians, hospice teams, home health agencies, social workers, spiritual care providers, and other partners to ensure high quality care in alignment with patient and family goals of care.

    2) Facilitate referrals to hospice, home health, community resources or other services when appropriate.

    3) Communicate proactively with patients, caregivers, and providers to educate and anticipate needs with goal of preventing avoidable hospitalizations in chronically ill patients.

    Education & Support

    1) Guide patients and families through advance care planning, goals-of-care discussions, and complex medical decisions.

    2) Provide education regarding disease process, prognosis, symptom management, and treatment options.

    3) Offer emotional support to patients and caregivers throughout the illness trajectory.

    Documentation & Compliance

    1) Maintain accurate, timely, and compliant documentation in the electronic health record (HRE).

    2) Ensure all care follows best practices, regulatory guidelines, collaborative agreement, clinical guidelines, and organizational policies.

    3) Participate in quality improvement, case conferences, and team meetings.

    4) Ensure compliance with monthly and quarterly collaborative requirements set forth by state board of nursing if not in an independent practice state.

    Position Qualifications

    1) Master's or Doctoral degree in Nursing from an accredited Nurse Practitioner program.

    2) Current Nurse Practitioner license in the state of practice.

    3) National certification from AANP or ANCC required.

    4) BLS certification up to date.

    5) Valid driver's license and reliable transportation for home visits.

    6) Strong clinical assessment, critical thinking, and communication skills.

    7) Specialized training or certification in palliative or hospice care (e.g., ACHPN) is highly desirable.

    8) Preferred experience in palliative care, hospice, geriatrics, or related field.

    9) Ability to work independently in the field with strong problem-solving abilities.

    10) Empathy, emotional resilience, and a deep commitment to patient-centered care.

    11) Comfort with complex family dynamics, culturally sensitive communication, and end-of-life issues.

    12) Proficiency with electronic medical records and mobile documentation tools preferred.
