    Direct Care Worker: In-Home Do you have a heart for helping others live safely and independently in their own homes? As a Home Health Aide (HHA) / Direct Care Worker, you'll provide compassionate personal care and support to individuals who rely on you daily. Whether you're certified or have relevant skills, this opportunity allows you to grow while making a real impact. We're seeking dedicated, friendly caregivers ready to deliver top-quality, in-home assistance in accordance with each client's personalized care plan. Your responsibilities may include, but are not limited to: Assisting with personal care: bathing, toileting, grooming, dressing, feeding, oral hygiene, nail care, shaving Mobility support: ambulation, transferring, positioning clients in bed, using assistive devices Supporting physical therapy plans, passive and active exercises under supervision Monitoring and communicating changes in physical or emotional health Performing light housekeeping: laundry, tidying bedrooms, kitchens, and bathrooms Preparing meals (including special diets), shopping for groceries, and household supplies Maintaining a clean, safe, and comfortable living environment Using PPE (Personal Protective Equipment) when appropriate Documenting care and submitting timely reports via telephony system Who We're Looking For: Required Qualifications: At least 18 years of age Completion of a formal certification program or ability to pass a skills/competency test Ability to read, write, and follow a written Plan of Care Good verbal, written, and interpersonal communication Neat appearance and good personal hygiene Dependability, reliability, and sound judgment Friendly, patient, and respectful with a sincere interest in caring for others Ability to lift up to 50 lbs and perform physical caregiving tasks Maintain confidentiality and comply with HIPAA regulations Preferred Qualifications: Valid driver's license, insured vehicle in good repair, and clean driving record Ability to travel independently to clients' homes (unless exempt) Must be able to work at least two weekends per month as scheduled What We Offer: Bi-weekly pay Flexible scheduling - work around your life Paid training and support Work one-on-one in the comfort of clients' homes A chance to build meaningful relationships and make a difference every day Physical Read Less

    As we continue to grow, we are seeking a dedicated, experienced Certified Nurse Practitioner to join our Family Health Center team and help expand access to comprehensive, patient-centered care.

    Position: Certified Nurse Practitioner (Full-time or Part-time)
    Location: [Insert Clinic Location - e.g., Abington, Norristown, Pottstown or other VNACS Family Health Center site]
    Reports to: Clinic Medical Director / Family Health Center Leadership

    What You'll Do

    Provide comprehensive primary care services to patients of all ages (infants through seniors), including wellness exams, acute care, and chronic disease management.

    Conduct patient assessments, diagnose illnesses, order/inter­pret diagnostic tests, and develop and implement treatment plans.

    Prescribe medications and therapies, including managing ongoing medication regimens.

    Provide patient education, counseling, preventive health guidance, and support for health maintenance.

    Collaborate closely with other healthcare professionals - including RNs, support staff, and administrative team - to ensure coordinated, high-quality care.

    Maintain accurate patient records and documentation, including use of electronic health record (EHR) systems.

    Participate in ongoing professional development, training, and quality-improvement initiatives to support our mission of compassionate, patient-centered care.

    Who You Are - Required Qualifications

    Master's or Doctoral degree in Nursing (or equivalent) from an accredited Nurse Practitioner program.

    Active Nurse Practitioner license in the state of Pennsylvania (or relevant state licensure) and national certification (e.g., family practice or adult-gerontology).

    Strong clinical judgment, diagnostic ability, and comfort managing a broad spectrum of health conditions and patient populations.

    Excellent communication skills, with a patient-centered and compassionate approach.

    Capacity to work independently and collaboratively as part of a multidisciplinary care team.

    Familiarity with electronic health records (EHR) and documentation best practices.
