    Under the supervision of the Director of Operations, we provide supportive living support services to adults with developmental, mental, and behavioral disabilities in their homes.

    Your General Duties:

    ● Assist clients to reach their daily goals and provide support based on their needs.

    Be able to remain calm and deescalate a situation when behavioral issues occur.

    ● Help with all independent living skills, such as cooking healthy meals, household tasks, grocery and necessity shopping, money management, and public transportation.

    ● Ensure the highest level of independence for the people you support by assisting in day-to-day tasks.

    ● Monitor medication and accompany the people you support to medical appointments.

    ● Assist with hygiene when necessary.

    ● Develop and maintain positive relationships with our individuals’ families and third-party agencies.

    Vision Life is currently seeking a compassionate, organized, and motivated Case Manager to join our growing Supported Living Services (SLS) team. If you are a natural leader who enjoys helping others and working collaboratively, this may be the perfect opportunity for you.

    Vision Life provides services to adults with developmental, intellectual, and behavioral disabilities, who are committed to promoting independence, dignity, and meaningful community living. The Case Manager will work closely with caregivers, families, Regional Centers, Program Supervisors, and other team members to coordinate quality services and support for our clients.

    Responsibilities


    Under the direction of the SLS Program Supervisor and Program Director, duties include but are not limited to:

    Coordinate and schedule client services and staff coverageEnsure authorized service hours are implemented appropriatelyConduct intake assessments and participate in client screenings and referralsProvide case management services using individualized support approachesMentor, support, and help manage Direct Support Professionals (DSPs)Complete monthly, quarterly, and annual reports and documentationParticipate in home visits and community-based support when necessaryProvide crisis intervention and behavioral support when neededMaintain client files, at-home binders, and updated documentationCollaborate with multidisciplinary teams and community resourcesCoordinate with Regional Centers, vendors, behavioral teams, and outside professionalsParticipate in on-call support rotation in accordance with company policyDeliver exceptional customer service and client-centered careQualifications

    Schedule:

    We are currently hiring for various weekday, weekend, and occasional overnight shifts to support a 24-hour care plan. Shifts include afternoon, evening, weekend daytime, and scheduled overnight coverage.

    Consistent hours are available for candidates with flexible availability. Due to the structured behavioral support plan in place, reliability and schedule consistency are highly valued.

    Primary Responsibilities
