  Responsibilities may include:
  Providing direct care and support to individuals with developmental disabilities - our goal is to foster increased independence, choice and empowerment while assuring that their health and safety needs are being met at all times. Working together as a team to ensure the needs of those served are met. Teaching and assisting individuals in all aspects of his/her life. Completing various household tasks including meal preparation and maintaining a clean and safe living environment. Providing personal care assistance.
  Requirements for this position include:
  At least 18 years of age. High school diploma or GED Personal, reliable vehicle that you're willing and able to use to transport individuals served. Valid (Indiana) driver's license and car insurance. Limited "non-driving" positions available. Can push, lift or carry up to 50 lbs. and can climb, balance, kneel and crouch. Experience in the field of developmental disabilities or direct care is preferred but not required. Ability to pass background checks.

We serve residents ages 18 and above, many of whom live with complex mental health needs, chronic illness, or developmental disabilities. Our psychiatric nurse practitioners partner closely with each facility's long-term care healthcare team to deliver individualized, high-quality mental health care for residents.   Responsibilities may include:
  Performing face-to-face assessments and ongoing psychiatric visits with adult clients in long-term care facilities. Maintaining current patient records to clearly document problems, treatment plans, actions, goals, and changes as they occur. Educating and supporting caregivers and facility staff. Managing and adjusting psychiatric medications to promote stability and well-being.
  Requirements for this position include:
  Graduate of an accredited Nurse Practitioner program. PMHNP license required. Experience in psychiatric, mental, and behavioral health services. Long-term care and/or geriatric experience strongly preferred. Outpatient and tele-psychiatry experience a plus. Willingness to travel within assigned service area.
