    We have a new opening for a Certified Registered Nurse Anesthetist - CRNA at Nellis Air Force Base in North Las Vegas, NV.

    Duties:

    Independently (without physician supervision) manage patient anesthesia experience, perform preoperative evaluations and preparations, administer monitored anesthesia care, regional, general and neuraxial anesthesia, monitor and manage patient reactions anesthesia and surgery, manage fluid therapy, perform and manage invasive monitoring, and perform post-operative evaluations as privileged by the facility Credentials Committee.Perform a pre-anesthetic assessment and evaluation of the This includes obtaining an informed consent from the patient, requesting consultations and/or diagnostic studies and ordering pre-anesthetic medications.Perform an assessment immediately prior to induction of anesthesia of the patient. This shall include a review of the medical record with regard to completeness, pertinent laboratory data, time of administration and dosage of pre-anesthesia medication, together with an appraisal of any changes in the patient's condition from that noted on previous visits. This note shall include any previous anesthetic experiences and complications thereof noted in the medical record.Maintain the patient's physiologic homeostasis and corrects abnormal responses to the anesthesia and/or surgery.Collect and interpret patient physiological data by selecting, applying or inserting noninvasive monitoring modalities.Manage the patient's airway and pulmonary Manage the patient's emergence and recovery from anesthesia by maintaining homeostasis, providing relief from pain and anesthesia side effects and preventing and managing complications through selecting, obtaining, ordering, and administrating medications, fluids or ventilator support.Record all pertinent events taking place during the introduction of, maintenance of and emergence from anesthesia, including the dosage and duration of all anesthetic agents, other drugs, intravenous fluids and blood or blood components on all patients that have received anesthesia care.Record post-anesthetic visits, including at least one note describing the presence or absence of anesthesia related complications of the procedure.All licensed health care providers shall provide patients with the utmost care and attention. All patients shall be assured of their privacy and personal dignity. Employee shall present a professional, conservative, and neat appearance. Contractor personnel shall report for duty in a professional manner, in appropriate attire befitting a health care setting, and having complied with socially acceptable standards of personal hygiene expected of health care workers. The employee will arrive to the start of the duty day earlier enough to prepare for the 1st case start to be on time (ex. in the OR room at 7:30am); and be flexible to be relieved or complete with the duty day within 15 mins before or after 3:00pm (or 5:00pm if on a 4-day /10-hour shift schedule). The employee shall ensure that all contract employees participate in the facility orientation procedures for newly assigned medical employees to include regulations specific to their professional specialty, hospital and facility policy and procedures. Contract employee training will include any in-processing and out processing, MHS Genesis, Joint Legacy Viewer, or job specific data training as required by the 99th Medical Group.The employee will abide by medical staff bylaws upon acceptance of of privileges.

    Schedule:

    The medical center is a 24-hour 7-day a week, inpatient treatment facility.

    Shift Structure: Daily shifts will begin at 6:30am and will be assigned as 8, 10, or 12-hour durations to meet variable surgical and patient care demands. All shifts include a minimum of a 30-minute uncompensated lunch break.

    Schedule Management: The specific shift assignment is determined by the Government supervisor.

    Requirements

    Minimum Qualifications:

    Degree: Possess a Masters or Doctorate degree in nursing. Graduate from an academic program approved by the Council on Accreditation of Nurse Anesthesia Programs (COA).

    Experience: As required to meet clinical competency requirements specified in the CRNA/Anesthesia-specific credentialing instructions, minimum 2 years of active practice as a CRNA, current within the last 6 months of application; practice must have included cases specifically in General Anesthesia and Obstetric (Neuraxial) Anesthesia.

    Licensure: Current, full, active and unrestricted license as a Registered Nurse and CRNA license from any state.

    Board Certification: Current Registered Nurse Anesthetist from the National Board for Certification and Recertification for Nurse Anesthetists (NBCRNA), AND a current state (any state in the U.S.) Registered Nursing (RN) License and CRNA License.

    Life Support Certification: Possess current BLS/ACLS/ALS/PALS certifications through the American Heart Association or American Red Cross. Must also possess a current Neonatal Resuscitation Program (NRP), and Advance Trauma Life Support (ATLS) certificates.

    Security: Must possess ability to pass a Government background check/security clearance.

    Duties:

    Provides discharge planning services to all individuals admitted to the medical treatment facility as inpatients, same day surgery, and emergency department patients.Triages discharge planning needs to permit DP to meet the most urgent, unusual needs. Develops template for repetitive DP processes/procedures to maximize efficient delivery of services when DP is overtasked.Accurately predict discharge/transfer needs to avert unnecessary delay in discharge, or extended Lengths of Stay (LOS) yet achieving a safe/timely discharge.Facilitates the smooth transition from an inpatient setting to the next appropriate level of care through effective professional coordination with the patient, the patient’s family/caregiver, and the multidisciplinary healthcare team. This coordination includes patients needing consideration for, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; patients having experienced combat stress, residuals of traumatic brain injury; persons experiencing community adjustment problems; addictions; psychiatric illnesses and other health problems.Collaborates with utilization management, case management, Medical Discharge Planning Social worker and other members of the healthcare team to achieve a timely, appropriate, safe discharge that meets the patient’s needs and is IAW local facility/ Department of Defense/War (DoD/W) as well as local and national discharge Assesses, plans, monitors, and evaluates patient care through discharge planning Applies Interqual (McKesson)/Milliman or other facility endorsed in- and out-patient evidence-based programs to monitor Length of Stay (LOS) as well as admission and discharge criteria.Actively participates in arranging transfers both into the Mike O’Callaghan Military Medical Center (MOMMC) and to other medical facilities either for care not available at the MOMMC or to a higher level of care.     • The quality of Emergency Services Physician service shall meet or exceed reasonable community standards of professional practice for the health care concerned as determined by the authority that governs military medical professionals in the same discipline.
    • Be capable of performing the full range of emergency medicine services to include the ability and experience to adequately diagnose and treat diseases and injuries.
    • Be able to adequately perform medical procedures to include, but not limited to, the following:
    o Incision and drainage
    o Nail Trephination
    o Sling or swath of injuries
    o Bandage
    o Reduce dislocations and fractures when appropriate.
    o Stabilize and evaluate cervical spine injuries as appropriate.
    o Lumbar puncture
    o Pericardiocentesis
    o Removal of foreign bodies as appropriate
    o Deliver newborn/perform emergency vaginal deliveries.
    o Perform venous punctures for lab studies and interpret results.
    o Draw arterial blood gases for evaluation and interpret results.
    o Suture or staple simple and multilayered lacerations
    o Remove sutures, surgical staples.
    o Tube thoracostomy
    o Open thoracotomy
    o Secure and maintain an adequate airway, by such means as:
    o oral airways; oral tracheal intubation; nasal/tracheal intubation; crico-thyrotomy
    o (Needle and surgical)
    o Splint and stabilize traumatic injuries to extremities.
    o Administer intravenous, intramuscular and subcutaneous injections.
    o Prescribe and administer medications as appropriate.
    o Secure and maintain adequate intravenous access by peripheral and by central.

    • Be able to perform tasks to include, but not limited to, the following:
    o Coordinate with transfer/accepting facility & other Emergency Departments
    o Request consults appropriately
    o Complete all required paperwork
    o Obtain an adequate history, physical, assessment and plan in a timely and appropriate manner on each patient and develop treatment plan
    o Utilize paraprofessional staff as appropriate, including non-physician providers
    o Respond to cardiopulmonary arrest as appropriate
    o Interpret electrocardiograms and laboratory tests
    o Preliminary interpretation of imaging studies to include CAT scans
    o Direct medical care through radio communication with an ambulance
    o Provide discharge instructions
    • Examine patients, render a medical assessment of their condition, and take appropriate action.
    • Provide immediate evaluation and management of emergency patients.
    • Provide definitive medical care to reduce the emergency medical condition and enable the patient to safely pursue follow-up care as an outpatient as required. Follow-up of abnormal laboratory and/or imaging study, reports is the responsibility of the ordering physician. Review each report in accordance with established facility procedures.
    • Respond to "Code Blue" (cardiac or respiratory arrest) medical emergency alerts within the hospital and provide resuscitation treatment until more specialized treatment arrives. Should resuscitation efforts be unsuccessful, the physician shall pronounce the patient dead and notify the next-of-kin, if the next-of-kin is physically present. Although completion of the death certificate is primarily the responsibility of the patient’s primary physician, the Emergency Services physician can complete this document if all the appropriate information is available.
    • Reassure and treat non-acute conditions in priority as established by medical triage.
    • Prepare appropriate patient profile changes.
    • Prepare and document appropriate history, physical examinations, and preoperative diagnosis. Maintain documentation of all treatment provided in accordance with facility directives.
    • Dispatch and provide medical control for ambulances in accordance with facility procedures.
    • Accompany emergency services technicians on ambulance runs when indicated by the nature of the emergency and in accordance with facility leadership guidance and facility procedures.
    • Perform emergency room procedures compatible with the facility operations capacity, supplies and equipment.
    • As workload permits during scheduled duty, attend the Emergency Service's monthly staff
    meetings and any additional committee meetings as required by the individual facility. The physician may also attend such meetings scheduled during other than their assigned duty shift.

    Work Schedule:

    Sunday - Saturday, 10 hour shifts, days, swings, and nights as needed.

    Pick up 5-8 shifts per month as needed.

    Requirements

    Minimum Qualifications:

    Education: Shall have successfully completed an approved Emergency Medicine (EM) residency acceptable to the USAF Surgeon General and be board eligible/certified by the American Board of Emergency Medicine (ABEM) or American Osteopathic Board of Emergency Medicine.Experience: Minimum of 3 years experience, training, and certification requirements for the appropriate level of emergency medical staff physician in accordance with regulations.Licensure: Current, unrestricted Nevada medical licenseBoard Certification: Board eligible/certified by the American Board of Emergency Medicine (ABEM) or American Osteopathic Board of Emergency Medicine.Life Support Certification: Possess current BLS, ACLS & PALS certificationsSecurity: Must possess ability to pass a Government background check/security clearance.
