    JOB TYPEClassification: Non Exempt Supervises Positions: JOB SUMMARY

    The RN Case Manager serves as a leader of the complex discharge planning process and a valued member of the healthcare team. They collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive/complex/ safe discharge plans. Collaborates with MSW for guidance with psychosocially complex cases. The RN Case Manager is also responsible for facilitating coordination of care. The RN Case Manager along with the other members of the healthcare team accepts responsibility, authority, and accountability for management of care. This shall be in accordance with the current policies and procedures, mission, vision, and values of UVA Health Northern VA Culpeper. Serves as a resource for discharge planning and care coordination for medically complex cases. Maintains current knowledge of available community resources/post-acute care options.

    QUALIFICATIONSEducation:

    Associate's Degree in Nursing required. 4 Year/Bachelor's Degree, BSN, preferred.

    Experience:

    2 or more years of healthcare experience preferred. Case Management or related experience preferred. Refer to the Life Support Training Policy for additional details.

    Licensure:

    Current RN licensure in state of employment required.

    Additional Skills/Requirements Required:

    Weekend and holiday coverage.

    Knowledge/Competence of Microsoft Office products and keyboarding.

    Competencies:

    Patient Care: Interviews and advises patients regarding health and illness prevention. Synthesizes data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention. Recommends community resources to meet patient and family needs. Quality: Collaborates with other departments or disciplines to achieve desired quality outcomes. Monitors quality of core processes in department. Applies the principles of safety and infection control to ensure the safety of all patients. Communication: Demonstrates superior interpersonal and communication skills. Works within a team to achieve patient and team goals. Shares and initiates regular and professional communication with co-workers. Participates in regular staff meetings. Identifies opportunities to improve processes based on feedback from staff and customers. Medical Knowledge: Provides primary health care and performs selective medical services under the direction of practice physicians. Obtains complete history and physical data on patients. Reviews patient records. Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures needed. Performs a complete physical exam and records findings. Establishes presumptive diagnosis. Orders appropriate laboratory and diagnostic procedures. Performs independent evaluations and treatment procedures in emergency situations. Provides monitoring and continuity of care between physician visits.

    The incumbent may be asked to perform additional duties as assigned.

    PHYSICAL DEMANDSPhysical Demand Code:

    Medium to Heavy Physical Demand: The job requires constant standing, frequently walking, and bending/stooping. Proficient communicative, auditory, and visual skills. Attention to detail and ability to write legibly. Ability to lift/push/pull 100 lbs. For any weight over 35 lbs., use Safe Handling Equipment.     The base compensation range for this role is $32.12 - $50.50 hourly.

    Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education. Please note that the pay range does not include any applicable incentive compensation programs such as shift differentials, clinical ladders, and other incentives that may be available for those in eligible positions.

The Float Pool (FP) Technician demonstrates the knowledge and skills necessary to provide services appropriate for the population assigned. The FP technician will receive assignments as delegated by the RN. The FP Technician will be able to access data reflective of the patient's status and report the information needed to identify each patient's needs relative to this information. The technician will provide care based on established policies and procedures. The FP Technician performs clerical, phlebotomy, safety attendant and other duties as assigned within their scope. High School Diploma or GED required. 2 years of acute inpatient or emergency technician experience preferred. CNA, Phlebotomy, ECG Tech, or EMT preferred. Phlebotomy, ECG course/certification, medical terminology. The incumbent may be asked to perform additional duties as assigned.
