Position Overview:
We are actively seeking passionate Optometrists to join our dynamic team and contribute to the delivery of compassionate care to our patient population. We have openings for both full-time and part-time clinicians who will conduct comprehensive eye examinations, practice full-scope medically focused optometry, diagnose and treat eye diseases, and make referrals as needed. For candidates interested in Vision Therapy, we have a fully equipped clinic ready for use. The ideal candidate should demonstrate the ability to practice independently and confidently.
Qualifications:

Attending General Dentist – General Practice Residency (GPR) Program
Position Overview
We are seeking a dedicated and experienced General Dentist to join our General Practice Residency (GPR) program as an Attending Dentist. This is an exceptional opportunity to shape the next generation of dental professionals through clinical supervision, mentorship, and interdisciplinary collaboration in an academic healthcare setting.
Key Responsibilities
Clinical Supervision & Teaching
• Provide direct clinical supervision and chairside teaching to GPR residents

Academic Contribution

Required:

9. Documents results of evaluations, treatment plans, progress and discharge from services in a clear and concise manner and in accordance with established standards.
10. Completes all required documentation for departmental purposes in a timely manner.
11. Develops appropriate home programs to maintain/enhance the performance of patients in their own environment. Properly documents and follows up with these programs.
12. Maintains daily caseload of ascribed number of patients.
13. Evaluates need for splints, ambulation devices and/or adaptive equipment as indicated. Recommends or issues equipment necessary to promote patient’s functional independence.
14. Maintains department facilities and equipment according to environmental safety and infection control guidelines as set forth by the Hospital, Nursing Home, UCHC-OPD and Department of Health.
15. Contributes to ongoing program development by providing in-service contribution of resources. Attends all staff in-services and meetings.
16. Performs all other duties as directed by supervisor(s) / Director.
17. Abides by the professional standards set forth by the New York State Board of Professional Services, JCAHO, APTA and other accrediting agencies.
18. Attends continuing education conferences related to profession and departmental needs.

UNION has a vested interest and is committed to addressing the behavioral health needs
of our patients and the community. Our team was one of the first to incorporate an integrated
(primary care/behavioral health) model of care required in today’s complex world. This model of
care is a holistic approach, which includes dedicated and experienced behavioral health
clinicians and medical providers designing, and delivering care as one unified team for each
patient. This collaboration between these disciplines helps engender the specific aim of
increasing access to care, reducing morbidity of both mental and physical disorders. UNION’s
Behavioral Health team teaches and encourages self-management behaviors using evidenced-
based practices to decrease symptoms and enhance patients’ quality of life. Services are
available for patients ages 5 and older with an array of therapeutic modalities. In 2024 UNION
will be expanding behavioral health services at additional locations and incorporating new
treatment modalities to address the complex needs of the community UNION serves.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
LCSW/LMSW will be part of a team focused on integrating behavioral health treatmentinto primary care by working directly with nursing, primary care physicians andbehavioral health staff treating depression and anxiety in an outpatient collaborative caremodel.Conduct initial screening, evaluation and diagnosing underlying mental illness.Provide short-term psychotherapeutic methods to children ages 5 and above, adolescents,adults, families and couples with familiarity using supportive therapy, motivationalinterviewing, PST, CBT.Participate in daily huddles in family practice, internal medicine and pediatrics.Attend behavioral health team rounds, weekly group supervision and present complexand new cases.Clinical supervision is providedQualifications:

PRINCIPAL MISSION:
Under the direct supervision of the AVP of Pediatric Wellness, the Registered Dietitian plans, directs and provides nutritional care to patients conducts nutritional assessments and provides nutrition education for patients and members of the medical team. Responsibilities include providing appropriate Nutritional Care for the age populations served (infant, pediatric, adolescent, adult, and geriatric).
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Promote a safe, cooperative and professional health care environment to ensure optimal p[patient care.Adheres to all hospital policies, procedures, rules and regulations, including but not limited to, absenteeism, ETIME, cellular telephone usage, dress code and rules of conduct.Makes Customer Service a priority, treating customers (patients, visitors, and co-workers) in a professional manner exercising courtesy and tact. Effectively communicates with patients and others about UCHC services in their area(s) of expertise or responsibility and ensures that concerns, questions or issues reach the right person(s) in UCHC in a timely fashion,Interviews patients, families, and/or significant others to obtain diet history necessary in planning the patient's diet,Gathers appropriate anthropometric, biochemical, and physical data necessary in assessing patients' nutritional status and documents information accurate in the medical record.Assesses patients' nutritional status based on standards of nutritional care and passing in a rate level of nutritional risk.Develops appropriate care plans based on standards of care, including recommending rate diet or nutrition support,Reassesses patients based on level of nutritional risk or change in condition and modifies the plan of care as necessary.Collaborates with members of the health care team on a regular basis in developing and implementing treatment plans and discussions.Assesses patient/family/significant others' educational needs, readiness to learn, and barriers to learning; provides the education and documents information in the medical record.Keeps current in the field of nutrition through continuing education to maintain registration status through the American Dietetic Association (75 Continuing Education Units/5-year period).
