    The urologist is responsible for providing medical and surgical urologic care to patients.  Physicians follow federal/state laws and regulations to ensure excellent patient care.  In this role, the urologist will be responsible for the evaluation and management of patients with urological disorders through office and hospital consultations and surgical procedures.  Effective communication with other physicians, staff, and patients is crucial in this position to ensure that patient needs are met efficiently and effectively.

As part of California's largest gastroenterology medical group, the Gastroenterologist will evaluate and diagnose patients, develop treatment plans, and ensure appropriate follow-up care in a private practice setting. This role will also monitor the progress of patients and adjust treatment plans as needed. Communication with other physicians, staff, and patients is paramount in this position to ensure that patient needs are met effectively and efficiently.

    What you will be doing

    The CCM Navigator organizes, supports, and implements various Unio Health Partners (UHP) programs under the direction of the Team Lead and Program Director. This role helps to coordinate patient care and ensures that the patient understands and is knowledgeable about their specific condition(s).  The CCM Navigator provides and documents non-face-to-face care coordination, mostly over the phone.  Communication with physicians, staff, and patients is paramount in this position to ensure that patient care needs are met effectively and efficiently.

    This is a remote position, and worker location must be within Unio Health Partners authorized states: AR, AZ, CA, CO, CT, FL, ID, IL, IA, MI, NV, NM, OH, OR, SC, TX, VA and WI

    What you will be doing

    Provide ongoing care coordination and patient navigation for patients in the Chronic Care Management (CCM) program over the phone and other non-face-to-face modalities.Function as a liaison between patients, physicians, staff, and outside medical offices to improve communication and patient care.Coordinate patient-specific needs that arise including, but not exclusive to, access to funding programs, psychosocial support, transportation or legal assistance, and clinical trial participation.Coordinate cancer and medical care for patients with cancer and/or chronic conditions, including patient education, program enrollment, appointment management, coordination of ongoing diagnostics, medication adherence and adverse effect monitoring, and provider communication.Coordinate treatments for patients, including enrollment, scheduling, and benefit verification.Initiate, monitor, and coordinate care planning for assigned patients, including patients receiving immunotherapy infusions and those in the Medically Integrated Dispensing Program.Place and receive high volume of patient calls daily, while remaining organized and diligent with any identified follow-up tasks.Provide a significant amount of patient/family education regarding cancer and medical treatments.


    What you will bring to the team

    Bachelor’s degree (BA, BS, or BSN) strongly preferred.Education/Experience: Clinical patient-care exposure typically gained as an LVN, MA, EMT, or similar role; or equivalent combination of training and experience.Preferred Credentials: Active LVN/LPN license (any U.S. state) or other clinical certification (e.g., MA, EMT).Skills: Care coordination, patient education, EMR proficiency, communication across multidisciplinary teams.1+ years of customer service/call center experience preferred.Knowledge of medical terminology and vocabulary.Bilingual in English and Spanish preferred

    What you will be doing

    What you will bring to the team
