The CCM Navigator organizes, supports, and implements various Unio Health Partners (UHP) programs under the direction of the Team Lead and Program Director. This role helps to coordinate patient care and ensures that the patient understands and is knowledgeable about their specific condition(s). The CCM Navigator provides and documents non-face-to-face care coordination, mostly over the phone.
