    Under the direction of the appropriate licensed clinician, and in keeping with unit specific standards, provides patient care to meet the personal needs and comfort of patients; and assists members of the healthcare team. In order to become a PCT II a caregiver must meet the full competencies outlined by the Nursing Professional Practice. As an integral member of the patient care team, serves both internal and external customers including patients, nurses, physicians and other colleagues who support the delivery of care and services.

    Major Responsibilities:

    Provides clear, timely communication to the RN regarding observations and changes seen in patients.Provides constant observation of high-risk patients as assigned by the Resource Nurse or designee.Participates in the development and maintenance of a safe environment.Assists patient with daily living activities such as personal hygiene, ambulating, meals and nourishment.Assists in moving, lifting, positioning and transporting patients by utilizing proper safety techniques and body mechanics in all work-related activities.Maintains patient safety by performing patient safety checks, utilizing fall prevention alternative devices/strategies as directed, and following the Restraint Policy standards when caring for a restrained patient.Takes and records vital signs (temperature, pulse, respirations, blood pressure and pulse oxymetry). Measures and records patients' height and weight.Performs EKG's, performs Point of Care (POC) tests and bladder scanning according to hospital standards. Collects, labels and prepares specimens for transport to lab.Phlebotomy in certain locations.Performs other selected unit specific procedures such as battery changes for telemetry units, checking status of unit equipment, cleaning equipment between patients and proper storage of supplies.Assists RN/Licensed Independent Practitioner with treatments and procedures by preparing patient; obtaining and setting up equipment and supplies, such as intravenous and hemodynamic lines, and utilize sterile technique when indicated. Help in positioning the patient and providing the patient with emotional support. Clean up equipment and supplies following the treatment and/or procedure.Observes, measures, records oral food and fluid intake. Sets up full strength tube feeding solutions and refills tube feeding bags as directed. Empties and records output, as approved, from a variety of sources, e.g., Foley catheter, ostomy appliances and drainage systems. Performs ostomy care on established ostomate patients as directed.Demonstrates sensitivity and responsiveness to patients' needs by acting as a source of information and approaches all interactions with the intention of building positive rapport and relationships.Provides respectful, culturally and age-appropriate communication with patients, staff, family, visitors and others.Replenishes patient and unit supplies and equipment.Requests assistance from identified resources when more direction is needed.Keeps current with hospital and unit changes by reading communication boards and/or books, bulletin boards, posted notices and reads and responds to UMMHC e mails on a regular basis.Has the ability to precept new Patient Care Technicians.

    Supports the day-to-day clinical activities of the practice including patient care, communication, documentation, phlebotomy, coordination of tests and consultations, equipment maintenance, and adherence to safety and compliance protocols.

    I. Major Responsibilities:

    Prepare for patient visits by reviewing schedules, checking for test results, and ensuring exam rooms are stocked and clean.Escort patients to rooms, confirm visit reasons, document interviews, and assist with mobility as needed.Take and record vital signs accurately, alert providers of abnormal findings.Assist providers during exams, anticipate needs, and help maintain schedule flow.Perform clinical procedures as ordered, including but not limited to: EKG, spirometry, immunizations, throat cultures, phlebotomy, and dressing changes.Handle patient communications related to prescriptions, test results, and triage; follow up appropriately per protocol.Document all patient interactions and care activities in the EMR.Schedule and coordinate testing/consultations, providing written and verbal instructions to patients.Maintain and sanitize equipment, monitor inventory, rotate medications, and check temperature logs.Comply with OSHA, HIPAA, and internal policies; support infection control practices.Provide high-quality phlebotomy services and manage specimen handling and transport.Promote positive patient experiences and demonstrate professionalism in all interactions.Collaborate effectively with team members and support a positive, efficient workplace.Display initiative, adaptability, and use resources responsibly.

    I. Major Responsibilities:

    Escorts patients to examination rooms, take and record vital signs such as blood pressures, temperatures, height, weight, etc. Collects and documents patient health status, history, and documents and monitors patient progress following standard procedures.Performs EKG's, phlebotomy, Point of Care (POC) tests according to clinic/office standards.May prepare, document, and administer non-intravenous injections as directed or required by office/clinic, within scope of practice.Assists in performing routine physical examination or clinical procedures.Reviews laboratory results and ensures abnormal results are immediately brought to the attention of the physicians and others, as appropriate.

    II. Position Qualifications:

    License/Certification/Education:Required:High School Diploma or equivalent.Current Basic Life Support (BLS) or Heart Saver (HS) certification from the American Heart Association. If not available upon hire, a BLS/HS must be obtained within 3 months from date of hire. If certification is not obtained within 3 months of hire, the employee may be terminated or employee may apply for another position for which they meet the minimum qualifications.Medical Assistant credential, approved by the Massachusetts Department of Health (ARMA, NHA, etc.), must be obtained within 1 year from date of hire/transfer. If credentialing is not obtained within 1 year of hire/transfer, the employee will be terminated, or employee may apply for another position for which they meet the minimum qualifications. Strong interpersonal skills to gather information from individuals, build relationships that support collaborative work within the department and across the organization and provide exceptional service to patients. Must have proven problem-solving abilities for routine situations and requests assistance from identified resources when more direction is needed. Assists in deep-cleaning of all transport equipment on a biannual basis. Sanitizes transport vehicles and ancillary equipment between patient use.Transports patient specimens to and from the Lab, Pathology, and blood products to/from the Blood Bank in accordance with established infection control guidelines. Transports contaminated equipment to Sterile Processing within same infection control guidelines.Delivers other materials such as medical records, pharmaceuticals, X-ray films, sterile items, and miscellaneous reports and items as required.Checks E-cylinders (portable O2 tanks) for content; distributes/exchanges E-cylinders throughout the hospital.Interacts with a web-based dispatch system TDSS via alpha paging for job assignment; maintains individual Transporter Trip Log and other statistical records as required.May participate in Emergency Department triage transport assignment during disaster situations including CODE HELP, assist in transport of trauma patients and transport of deceased victims to the morgue. Guides and assists patients through decontamination unit.May assume the role of a dispatcher if trained and as directed.
