    Working as a Community Support Specialist is a unique learning opportunity. Through care planning, you will be an integral part of a client’s growth and recovery.     The Care Coordinator, CCBHC’S (Certified Community Behavioral Health Clinic), in collaboration with the CCBHC Outpatient team, is responsible for coordinating referrals and linkages to help members navigate and access services. The Care Coordinator’s role supports members through community outreach and case management to improve health and recovery outcomes. Furthermore, this role is responsible for maintaining the quality of data tracking and reporting in alignment with CCBHC requirements.

    ESSENTIAL DUTIES & RESPONSIBILITIESMonitors and facilitates access, engagement and linkage to healthcare services and resources for Thresholds CCBHC members.Provides case management by phone and in person to refer and link members to external healthcare providers and community resources to facilitate and improve continuity of care, promote progress toward recovery goals, and address social determinants of health. Depending on the member’s case management needs, activities could include a wide range of things like helping someone apply for Medicaid, follow-up with Social Security, enroll for classes, apply for affordable housing, find food pantries, learn to ride public transportation, learn ways to track appointments, find activities to do in line with the member’s interests and values, find out where to go for things like tax filing, affordable child care, and legal support, and other similar things necessary for a member’s success.Provides community outreach to members to assist with linkage to mental health, physical health, substance use services and other resources.Follows up with collateral providers to facilitate improved continuity of care.Reaches out to members who miss appointments to address barriers to care.Identifies gaps in care for members and arranges appropriate and timely resolution, providing support to remove common obstacles and barriers to outpatient care and resources.Attends and participates in team and branch meetings.Documents member care and related communication in the electronic health record in accordance with state, federal, and Thresholds requirements.Establishes and oversees a process for communicating pertinent member information and status reports to designated Thresholds and community healthcare providers.Advocates for members who may be vulnerable or need additional support.Creates and develops relationships with community providers to find resources for our members by attending networking meetings, visiting or contacting agencies, and attending information fairs.Participates in regularly scheduled caseload consultation. These consultations may focus on members who are new to treatment, difficult to engage, or who are not improving as expected.Identifies and links members to Thresholds Supported Employment IPS program and assists members with applications for financial and/or medical benefits.Completes the NOMS (National Outcome Measures) and submits them in a timely manner.

    MARGINAL DUTIES & RESPONSIBILITIES

    May be required to work flexible schedules and be available to work on weekends or evenings as
    required.

    EDUCATION

    Must, at a minimum, have a high school diploma and at least two (2) years of mental health and/or community engagement experience.Mental Health Professional designation (meeting Office of Mental Health Rule 132 credential requirements) is strongly preferred.High school diploma or GED certificate and 5 years of supervised clinical experienceBachelor’s degree in Psychology, Social Work, or related fieldBachelor’s degree in an unrelated field and 2 years of supervised clinical experienceQualified Mental Health Professional including a Master’s degree in social work, counseling, or closely related field is a plus.

    EXPERIENCE

    Experience working with persons diagnosed with mental illness preferred.Experience engaging with the community to promote services is a plus.Ability to document services provided in an Electronic Health Record in a timely manner.Daily access to a personal vehicle is required. Must obtain and maintain $100,000/$300,000/$100,000 bodily injury and liability insurance. Subject to validation every six months.May be required to provide transportation for members of the program using a Thresholds provided vehicle or personal vehicle. Must be 23 years of age and must meet all insurance carrier’s requirements. Subject to annual Department of Motor Vehicles license verification.

    Are you ready to take the next step in your career? As a Team Leader with Thresholds Emerging Adults team on Chicago's North Side, you will supervise a dynamic team of Community Support Specialists (CSS) as they provide mental health services to young adults in the community. This includes sharing case management and counseling duties within the team and leading by example while working directly with clients, providing guidance, and growing the clinical skills of your CSS team.

    Team Leaders hold a unique position within Thresholds: they provide direct service while keeping an eye on the overarching goals of the program. The Emerge program uses evidence-informed coordinated care to support young adults ages 21-27 struggling with serious mental health challenges. Emerge promotes wellness, healthy development, community and vocational engagement, and effective symptom management, and it is nationally recognized for its innovative approach.

    You and your team will remove barriers to mental healthcare through case management: connecting our clients with resources and helping them navigate complex systems. You will help clients strengthen daily living skills as you drive them to doctors’ appointments, to benefits offices, or to the grocery store. You will collaborate with clients to create plans for symptom management and teach coping skills that support clients in reclaiming their lives.

During regular team meetings and supervision, you will engage and motivate your staff, provide direct feedback, coach staff to meet billing targets, drive collaboration on client care, and help staff work through challenges. You will also manage and participate in a rotating on-call schedule to respond to clients' needs.     The Transition Coordinator serves as the primary transition facilitator for youth and young adults who are discharging from the Young Adult Residential and Transitional Living Program. They provide the coordination of mental health treatment, housing, benefits, and linkage to other services and resources that will be needed post-discharge. They also assist in skill-building through coaching and one-on-one work with young adults as they prepare for the transition.

    ESSENTIAL DUTIES & RESPONSIBILITIES

    Provides direct service to members through transition, including case management, crisis intervention, and psycho-social rehabilitation services.Develops and implements the transition/discharge plan, in cooperation with other members of the transition/treatment team.Connects member with community and Thresholds resources, including appropriate housing, mental health and psychiatric services, physical health services, public assistance benefits, resources for social/leisure activities, and developing connections to family and others.Provides direct assistance to help members move, obtain household items, and explore their new community at time of discharge from residential program. Maintains contact with members post-discharge from the residence to provide continuity of care and track outcomes.Primary contact between agency, families, and other collateral resources to assist with members' transition goals and needs.Performs and participates in crisis intervention using Therapeutic Crisis Intervention (TCI). Performs and supervises physical management when necessary, according to Thresholds policy. Thresholds provides and requires training in TCI. Attends all transition-related court appearances, DCFS meetings, agency staffings, team meetings, treatment team meetings, and other meetings as assigned. Ensures all Thresholds procedures regarding the handling of members' funds are followed and meet the highest standards.Completes all assigned paperwork in a timely fashion. Produces Medicaid compliant service notes. Completes reports for Thresholds and other stakeholders when required. Facilitates group programming related to transition in both the YAP residential program and the REACH program.Teaches financial literacy curriculum to members.May be required to work flexible schedules and be available to work on weekends or evenings as needed. May be required to be on call.

    EDUCATION & EXPERIENCE

    Bachelor's degree in psychology, social work, or a related field required. Master's degree preferred.Previous experience working with young adults strongly preferred.Previous experience working with housing assistance programs and/or public assistance benefits systems strongly preferred.

    The Service Coordinator, Continuum of Care reports to the Team Leader, Service Integration and is responsible for coordinating referrals and linkages to clinically indicated services supporting housing retention and eviction prevention within the Quality department for the Continuum of Care (COC) Program. The Service Coordinator’s role supports providers through an integrated approach to case management and community outreach while implementing program improvements that strengthen care management, member access, and health outcomes. This role additionally ensures quality of data reporting and project performance outcomes, including increasing income and non‐cash benefit measures.

    ESSENTIAL DUTIES & RESPONSIBILITIES

    Supports the mental and physical health care of members on an assigned caseload.Closely coordinates care with member’s mental health, physical health, and/or substance use providers.Works closely with community resources and organizations to coordinate member services.Provides support and attempts to remove common obstacles and barriers to outpatient care.Facilitates member outreach, engagement, linkage, referrals, coordination, and follow up for mental health, physical health, and substance use services and supports.Identifies gaps in care for members and arranges appropriate and timely resolution with internal and external agencies.Identifies risk factors related to relapse and develops relapse prevention plans and strategies in conjunction with clinical consultation.Advocates for members who may be vulnerable or need additional support.Provides member education on common mental health, physical health, and substance use disorders.Provides brief interventions using evidence based psychosocial treatments, (CBT, MI) as clinically indicated.Participates in regularly scheduled caseload consultation. These consultations will primarily focus on members who are new to treatment, difficult to engage, or who are not improving as expected.Ensures grant compliance with program targets by identifying and linking participants who are in need of income, employment, or are eligible for benefits and assists in connection to employment services and coordinates mainstream benefits applications.Ensures compliance with service delivery, data quality, and program outcomes for permanent supportive housing programs.Collaborates closely with Thresholds Housing Administration, HMIS data administrator and Grants Accounting Team through the submission of partner service reports.Completes eligibility and needs assessment of referrals from the Coordinated Entry System (CES).Monitors service delivery of project service partners to program participants, including reviewing personnel activity reports.Conducts annual audits and files reviews for partner agency service providers.Provides accurate and timely information to their supervisor(s) on all topics and issues related to the project operations and wellbeing.Other duties as assigned.

    MARGINAL DUTIES & RESPONSIBILITIES

    May be required to assist in evaluation of clinical performance outcomes through reporting data to systems integration specialist and team.May be responsible for assuring that all appropriate safeguards are in place with regard to the confidential files maintained within the Housing Administration. All records are to be maintained in such a fashion as to prevent unauthorized access as well as to reasonably protect them from fire, water damage or other hazards. Electronic records will be backed‐up in accordance with agency guidelines.

    EDUCATION

    Bachelor’s degree in social work, psychology or closely related field required.Master's degree in social work, counseling or a closely related field preferred.

    EXPERIENCE

    Two (2) years of relevant work experience in mental health is required.Case management knowledge and experience in a healthcare setting, including utilization management is preferred.Demonstrated knowledge of community service providers.Experience with Motivational Interviewing.
