    SUMMARY

    The purpose of this position to provide clean living and working conditions for residents and staff in accordance with state, federal and county regulations along with facility policies and procedures.

    ESSENTIAL DUTIES AND RESPONSIBLITIES include the following (other duties may be assigned):

    Ability to perform cleaning procedures with mop/bucket, machines upon training, and general cleaning with chemicals.Ability to follow oral and written directions.Ability to assist in proper care and use of janitorial equipment.Ability to perform waste disposal procedures in accordance with facility policy this includes regular trash, recycling, and furniture removal.Ability to lift up to and possibly more than 50 lbs. during supply dispersement and furniture moving.Ability to follow Resident’s Rights policies at all times; follow facility safety program.Ability to observe infection control procedures related to the Housekeeping Department.Ability to come to work as scheduled and consistently demonstrate dependability and punctuality; accept assigned duties in a cooperative manner.Ability to perform other duties as assigned.Must be able to speak/understand English fluently.Must be able to work around pets ie: dogs and cats.Tracking the status of all work orders and ensuring timely completions.Effectively communicating updates to all requestors on timeliness of work order progress.Assist with emergency response protocols, including assisting Facilities department during urgent situations.Conducting facility walkthroughs to identify maintenance needs.REQUIRED SKILLS/ABILITIES: Must possess a positive attitude with the ability to interact with various staff, residents and vendors with positive and realistic expectations.Must be able to read and write English.Interpersonal and customer service skills.Excellent organizational skills and attention to detail.Working understanding of Carpet Specialist practices and procedures.Ability to build credibility with all levels of the organization.Superior time management skills with a proven ability to meet deadlines.Ability to function well in a high-paced environment.Basic understanding of building maintenance procedures and systems.

    EDUCATION AND EXPERIENCE:

    High School Diploma preferred but not required.2-4 years of demonstrated carpet cleaning experience preferred.Prior customer service experience preferred.

    SUPERVISORY RESPONSIBILITIES:

    This position is not responsible for supervising other staff.

    About the Role:
    The RN Nursing Supervisor plays a key leadership role in the daily clinical operations of our skilled nursing community.     SUMMARY

    The Nurse Navigator (RN) serves to support the organization’s commitment to a seamless continuum of care by enhancing resident outcomes, promoting independence, and strengthening care transitions. As the clinical liaison and care coordinator for Independent Living residents, the Nurse Navigator guides the residents through the full continuum of care within the community.

    The Nurse Navigator enhances outcomes, improves communication, and supports a person-centered approach to wellness. This role proactively supports residents in maintaining independence, navigating healthcare decisions, coordinating transitions (Independent Living to Assisted Living, Skilled Nursing, Home Care, or external providers), and ensuring seamless communication across care settings. The Nurse Navigator works in partnership with the Director of Nursing and collaborates with Skilled Nursing leadership and coordinates with Social Services Coordinates with Social Services.

    ESSENTIAL DUTIES AND RESPONSIBLITIES include the following (other duties may be assigned):

    Care Coordination & Navigation

    Serve as the primary clinical point of contact for residents and families to help them navigate the healthcare system.Assess resident needs and develop individualized care plans focused on wellness, safety, and optimal outcomes.Coordinate and manage transitions across all levels of care, including hospital discharge planning, short-term rehabilitation, return to Independent Living, and potential movement to Assisted Living or Skilled Nursing. This is done while ensuring continuity, accuracy, and timely communicationCollaborate with physicians, therapists, nursing staff, social workers, and wellness professionals to support comprehensive, integrated care.Facilitate scheduling of appointments, follow-ups, diagnostic testing, and specialist referrals.

    Resident & Family Support

    Provide education on diagnoses, medications, treatment plans, and healthy lifestyle behaviors.Coach residents and families to improve health literacy and confidence in managing conditions.Offer anticipatory guidance, emotional support, and resources to help residents make informed decisions.

    Examples in this area include post-hospital discharge, new chronic diagnosis, high-risk residents, frequent ED utilizers, and cognitive decline concerns

    Clinical Oversight

    Conduct clinical assessments for new admissions, return-from-hospital evaluations, and ongoing wellness check-ins as appropriate.Monitor high risk residents for changes in condition and intervene or escalate concerns promptly.Review medical records, lab results, and provider recommendations to ensure clarity and accuracy.The Nurse Navigator does not provide ongoing skilled nursing services unless ordered under an approved home care plan but facilitates coordination and communication of clinical services.

    Interdisciplinary Collaboration

    Participate in care plan meetings, case reviews, and wellness team huddles.Promote a holistic, person-centered approach by integrating physical, emotional, social, and cognitive wellness.Serve as a liaison between residents, families, healthcare partners, and community resources.

    Quality, Compliance & Documentation

    Maintain detailed and accurate documentation within the electronic medical record (EMR).Escalates urgent clinical concerns according to established clinical protocols and emergency response procedures.Track outcomes, identify patterns, and support quality improvement initiatives.Uphold all regulatory requirements, infection control standards, and nursing best practices: Reduce avoidable hospital readmissionsImprove resident satisfaction with healthcare navigation.Increase timely post-hospital follow-up compliance.Support appropriate level-of-care placement decisions.

    Qualifications

    Education & Licensure

    Current Registered Nurse (RN) license in the state of practice, in good standing.Bachelor of Science in Nursing (BSN) preferred.

    Experience

    Minimum 2–3 years clinical nursing experience (geriatrics, care management, acute care, or home health strongly preferred).Experience with care coordination, case management, or patient navigation is highly desirable.

    SUMMARY

    The Licensed Practical Nurse is responsible for participating in assessing, implementing and documenting resident care as ordered by a physician and with the continuous availability of a registered nurse.

    ESSENTIAL DUTIES AND RESPONSIBLITIES include the following (other duties may be assigned):

Complete incident report and applicable follow up form completely, and accurately in a timely manner following event.Prepares and administers medications, tests and treatments accurately, observing patient response, as ordered by the physician and documents outcomes timely and appropriately in the medical records.Participates in on-going patient assessment and evaluates patient’s responses to nursing actions under the guidance of a registered nurse.Documents changes in patient conditions and notifies physician, responsible party and supervising RN of changes.Completes appropriate documentation and other record-keeping duties as per company policies and relative local, state and federal regulations in patient’s electronic medical records (Point Click Care).Communicate with internal and external physicians regarding patients care and condition.Transcribes, clarifies, co-ordinates and implements physician orders.Communicates with the residents, physician, family members, and other members of the health care team. Complies with company policies, procedures, and related local, state and federal regulations. Maintain accountability for controlled substances and inventories medications at the beginning and end of each shift with another licensed nurse. Notifies RN supervisor of discrepancies. Supports the facilities medical supply system, follows facility procedure for patient charge items. Follows clinic fee schedule with regards to patient charges for time and services rendered. Remains current on nursing techniques and advances as applies to LPNs by attending in-services, workshops, seminars, and continuing education opportunities. 100% attendance to all mandatory in-services/competencies yearly is required. Ensures supplies and equipment necessary for patient care are stored in an organized, safe and efficient manner. Communicates and co-ordinates resident medication orders with Pharmacy as needed, per physician’s order.QUALIFICATIONS:

    The medical Transportation Driver meets the medica; transportation needs of the residents.

    ESSENTIAL DUTIES AND RESPONSIBLITIES

    Represents the company in a positive manner while creating an environment that fosters respect and courtesy for residents, employees, vendors and professional partners alike. Works within the company programs to promote safety and the well-being of all residents and employees.Upholds the company’s Mission Statement.Maintains HIPAA standards in regards to all medical or healthcare information pertaining to residents and employees as confidential.Schedules appropriate routes, parking, etc. prior to departure. Ensures van is clean and gassed prior to trips.Acquires and maintains knowledge of all safety procedures pertaining to the medical van. Utilizes knowledge in times of emergency, breakdowns or accidents.Assists residents with entering and exiting the van. In addition, assistance with any items they bring on board may be required such as a walker, bag, package, etc. Reports needs or concerns of residents and any issues with the vehicle to the Director of VWB or Lifestyle Manager immediately. Reports any driving violation committed during working hours immediately to supervisor and/or director.Maintains/coordinates all scheduling, billing, and driving. Communicates with residents, staff, families and caregivers to coordinate all aspects of the transportation requests. Assists with gassing vehicles and setting up routine vehicle maintenance in coordination with the facilities department.

    OTHER DUTIES AND RESPONSIBILITIES

    Responsible for knowledge of fire and safety procedures including those involving blood-borne pathogens.Remains in proper attire at all times and maintains a high standard of appearance.Interacts well with residents, team members, prospects, vendors, and partners.Maintains excellent resident and employee relations. Handles complaints and recommendations constructively.Performs other duties as assigned by the Lifestyle Coordinator and/or Director of Vitality and Well-being.

    EDUCATION and/or EXPERIENCE

    High School diplomaMedical Transportation Experience Preferred

    SKILLS

    Valid NC commercial driver’s license and a clean driving record.Willingness to work in an environment that changes based on the needs of the residents. Strong verbal communication skills. Ability to work with minimal supervision.Must conduct all business with a professional manner and with a high level of confidentiality.

    PHYSICAL DEMANDS

    Climb up to two flights of stairs occasionally.Lift objects of 25 pounds or less over the head. Move objects of 40 pounds or less.Sit or remain standing for extended periods.Bend, stoop, twist, and perform many maneuvers to safely secure residents into van.Drive lengthy distances as required.Respond to medical emergencies, if necessary.Assist with resident evacuations, if necessary.

    Summary

    The Environmental Services Manager is responsible for overseeing the daily operations of the Environmental Services department, ensuring a clean, safe, and comfortable environment for residents, staff, and visitors. This role includes supervision of staff, management of supplies and resources, and compliance with all applicable regulations and policies. The Environmental Services Manager collaborates with other departments to maintain high service standards and supports the Director of Environmental Services in achieving departmental goals.

    Essential Duties and Responsibilities

    Supervise the Environmental Services team, including interviewing, training, scheduling, and performance evaluations.Ensure compliance with safety, sanitation, and infection control standards in all cleaning and maintenance activities.Manage inventory of cleaning supplies and equipment, including ordering and maintaining stock.Coordinate with vendors and service providers for procurement and maintenance needs.Support the Director of Environmental Services in preparing and managing departmental budgets.Monitor work request through WorksHub, bill residents and process monthly for Accounts Payable.Respond to and resolve resident and staff concerns related to cleanliness or environmental services.Provide hands-on support in cleaning and housekeeping activities as required.Assist with emergency response protocols, including managing Environmental Services during urgent situations.Develop and maintain schedules for cleaning and housekeeping tasks to ensure operational efficiency.Supervises the personal laundry service for the healthcare residents

    Qualifications

    Education and Experience
