    Job DescriptionThe Service Coordinator supports residents and families by connecting them with resources, referrals, and case management services that promote employment, stability, and economic self-sufficiency. This role will conduct assessments, develop service plans, maintain documentation, coordinate with community partners, and provide follow-up support to ensure residents receive the services they need.
    Responsibilities:
    • Conduct resident interviews, needs assessments, and service plan development
    • Provide case management support, including referrals, follow-ups, appointment setting, and crisis assistance
    • Maintain accurate resident files, reports, case notes, referral forms, and related documentation
    • Build relationships with community agencies, service providers, resident councils, and local resources
    • Encourage resident participation in employment, training, and supportive service programs while tracking progress
    Qualifications:
    • Bachelor’s degree in Sociology, Psychology, Social Work, or related field preferred
    • Five years of related case management experience with low-income individuals may substitute for degree
    • Strong interpersonal, written, and verbal communication skills
    • Ability to work independently, manage priorities, and maintain accurate documentation
    • Valid Texas driver’s license, dependable transportation, and willingness to travel locally as needed
