Position Overview:
The Case worker, under close supervision, is responsible for supplementing and assisting the AOBH Social Worker in addressing the treatment needs of youth and families. The Case worker participates in: Formulating and implementing appropriate treatment for youth and families on their caseload. Assessing and implementing an appropriate permanency plan for all assigned youth. Providing clinical input into the therapeutic milieu to ensure the most appropriate care for each youth. Documenting all interventions and required reports in a timely and professional manner. Coordinating all treatment provided to each resident to ensure appropriateness, timeliness and effectiveness of interventions. Creating and coordinating IL (Independent Living activities and schedules. Other duties as assigned.
Position Qualifications:
Bachelor’s Degree in Social Work or related field. Prior work experience in social service, mental health or child welfare field preferred. Driver’s license. Solid written and verbal skills, including ability to successfully manage multiple tasks. Bi-lingual preferred.

The Bravehearts will create a new and innovative resource in the Rockland community for young adults to seek support and empowerment from their peers while advocating for the improvement of services and the inclusion of youth voice by expanding Braveheart motivational meetings and support services to youth within the target population as detailed by the grant, within Rockland County. The Braveheart Peer Navigators will used both lived and leadership experience and expertise to nurture, develop and facilitate the activities of the Rockland Bravehearts movement both administrative and programmatically.
In addition, this position will require the ability to coordinate and facilitate biweekly (as need expands, weekly) Braveheart meetings in Rockland for youth. Peer to Peer Navigators need to be facilitate focus groups, forums and activities to engage youth to address systemic concerns.
Position Qualifications:
Candidate must have a GED or high school diploma and have personal experience with the child welfare and/or mental health system in the areas of navigating the educational, employment, wellbeing and housing systems and resources. Must have basic understanding and computer literacy skills with Microsoft office such as Microsoft Word, PowerPoint, and Excel. Must have basic organizational skills and strong oral communication skills. Must have experience and ability to navigate the public transportation system. Demonstrate an understanding of the importance of culture, race and issues facing adolescents of color. Demonstrate ability to effectively use a multi-cultural approach in working with special needs youth. Ability to work as a team member and independently, as well as actively participate in supervision. Ability to establish and maintain effective relationships with youth and staff, as well as ability to partner with professionals. Willingness to speak publicly and participate in trainings. Ability to maintain communication logs.

MST (Multisystemic Therapy) is an adaption of standard MST that provides intensive family and community-based treatment that addresses the multiple behavioral and psychological determinants of serious antisocial behavior in juvenile delinquents and offenders across the key settings, or systems, within which youth are embedded (e.g. family, peers, school, neighborhood). MST strives to promote behavior change in the youth’s natural environment, using the strengths of each system to facilitate change. The Caseworker is a huge support to the family and team and responsible for assisting with basic needs and linkages that connect families to resources will make transformational change. The MST model fits well with the Children’s Village mission statement, current focus towards community-based services and family reunification and with CV values. The MST model promotes personal responsibility, education, work ethic, community responsibility and an intolerance of violence and prejudice.

Minimum qualifications: Bachelors’ Degree in Social Work, Psychology, Childhood Education, or a related subject area of direct clinical experience in treating adolescents or family systems.

Braveheart’s staff are comprised of young professionals who are often referred to as peer navigators, credible messengers and advocates. These individuals harness various levels of lived experience in the NYS child welfare and juvenile justice system of services for youth/young adults with high end needs. The Braveheart Peer Navigators utilize their unique skill set to authentically connect with youth participants in an effort to create plans for sustainable and realistic community connectedness for those youth who are the most at risk of arrest, homelessness and social disconnection. This is achieved by giving youth voice a meaningful platform and establishing the value of partnership with existing professional supports already identified by the client while uplifting the voices and causes of youth.
The Bravehearts Peer Justice Advocate will provide mentoring, advocacy, group facilitation and resource connection to young adults adversely impacted by youth serving justice systems. The Peer Justice Advocate will provide programming on Children’s Village campus to youth who exhibit runaway and high-risk behaviors requiring court activity through the Raise The Age initiative as well as provide programming in the county correctional facility and juvenile detention center. The Peer Justice Advocate will provide advocacy with and on behalf of the youth participant by working in partnership with professionals and systems that the youth is impacted by. Individual will assist in permanency planning for youth alongside support team. Individual will also assist in preparing youth for adulthood and aging out of care/systemic impact Individual will facilitate weekly motivational meetings. Individual may be required to sit in on administrative meetings regarding individual clients and broader systemic advocacy.
Position Qualifications:
High School Diploma or GED required. Strong interpersonal skills and ability to connect with adolescents. Knowledge, experience with system impacted youth preferred. Personal history of foster care as a youth, or experience with the juvenile justice system. Some public speaking is be required. Demonstrated initiative and creativity, particularly with adolescents. Demonstrate understanding of the issues facing Black and Latino adolescents. Ability to work as a team member, independently and bring creative and innovative ideas to program.
