Role Description This is a full-time, remote role for a Social Care Manager based in Johnson City, NY. The Social Care Manager will support clients through personalized case management, assist with housing navigation and planning, coordinate with community resources, and provide tailored social care support. The role also involves collaborating with healthcare professionals to address clients' behavioral health and social needs effectively. Monitoring program compliance and training staff in best practices is a critical part of this position. Qualifications Experience in Case Management and Social Services Skills in supporting Elder Care and addressing other age-specific needs Proficiency in Training and development within social care or behavioral health frameworks Familiarity with Medicine and working alongside healthcare professionals to support behavioral health needs Strong organizational, communication, and problem-solving skills Ability to work effectively in a team environment and navigate community-based resources Master's degree in Social Work, Public Health, or a related field preferred Experience with Medicaid programs and regulations is a plus
