General Purpose:
The Clinic Nursing Director provides strategic and operational leadership over all clinical nursing services, ensuring the delivery of safe, efficient, and high-quality care. This leader oversees licensed nurses, unlicensed assistive personnel, and clinical support teams, and partners closely with medical and operational leadership to align nursing services with organizational goals.
This role supports the advancement of team-based care, population health management, and integrated service delivery across STRIDE sites. The Nursing Director models leadership behaviors, coaching, and accountability to build high-performing, values-aligned teams.
Essential Duties/Position Responsibilities:
Provides supervisory functions within the department including hiring, training, evaluation, workflow, discipline, standards, and overall performance evaluation in accordance with STRIDE policies, procedures, and applicable laws. Oversees compliance with all applicable regulations and standards pertinent to the organization and care environment. Assists in the development, implementation, and maintenance of clinic and patient care processes and procedures. Provides direct service to patients in the conduct of various clinical procedures within the approved scope of nursing. Assists in determining required levels of medical supply inventories, conveying needs to the Purchasing Department, and overseeing other supplies and equipment needs and maintenance requirements. Responsible for developing nursing staffing schedule based on workloads of providers and productivity targets established in the budget. Under the general direction of the Senior Leadership Team, develops and maintains approved policies and procedures for Nursing and clinical operations and helps ensure compliance with all federal, state, and local regulatory and accreditation bodies. Education and Experience:
Baccalaureate degree in Registered Nursing from an accredited school or training program, master’s degree in nursing or related field preferred. An active, unrestricted Registered Nurse license in the State of Colorado or appropriate compact licensing arrangement. 2 years of full-time experience as a Registered Nurse in a hospital or community health setting. Previous ambulatory care nursing experience is preferred. Previous clinical leadership experience is preferred.Skills & Expertise

Work Schedule
40 hours a week
This role will provide leadership and oversight across multiple sites, including Englewood, Conifer, School-Based Health Centers, and Triage services.
We offer a competitive salary range of $92,624 - $136,700 depending on your experience.
This range reflects STRIDE’s good faith estimate of potential compensation at the time of posting. The final salary for the selected candidate will be determined based on several factors, including experience, education, budget, internal equity, specialty, and training.

General Purpose:
The RN Care Coordinator (RNCC) meets comprehensive health and assessing needs, coordination and external resources to promote quality cost-effective care. The RNCC must be skilled in the assessment, evaluation and interventions of persons and families with multiple and/or complex care needs. The RNCC must be organized, detail-orientated, prioritizing clinical issues and managing a caseload of patients with varying degree of health and psycho-social needs.
The RNCC will develop individualized care plans in collaboration with the STRIDE care team and external partners to ensure goal directed care. The RNCC will coordinate and provide comprehensive care, including chronic disease management and medication titration, within established protocols.
Essential Duties/Position Responsibilities:
Assessment
Conducts a comprehensive patient and support system assessment at initial visit and at intervals necessary to update goals and plan of care.Comprehensive review of patient health record, external health records, health status and risk factors. Identifies family and support system strengths and needs. Use multiple modalities for communication including face to face visits, written communication (MyChart) and telehealth such as telephone encounters or video visits. Identify patients for care coordination using various tools.Promote patient autonomy and self-management of conditions.Assist with safe transitioning of care to most appropriate level of care. Terminate care coordination when client reaches most appropriate level of care.Utilizes data and metrics to meet patient and organizational needs. Maintains high level of skill and responsibility, including but not limited to: Symptom management. Review and act on test results. Work Schedule:
Monday-Friday, 8 hours per day. This position is hybrid, with 3 days in the office, and 2 working from home.
