    Your role:

    The call center schedules appointments and distributes information to STRIDE CHC clients regarding services and programs while providing patients with quality customer service.


    Essential Duties/Responsibilities:

    Answers up to 100 calls per day calls from patients and partners regarding scheduling medical and dental appointments for new and established patients, medication refills, general questions, etc.Identifies customer needs, clarifies information, builds sustainable relationships and engages customers by going the extra mile.Keeps accurate records of conversations and communicates effectively with clinic staff.Ensures one call resolution whenever possible and provides a connection to clinic staff when needed.Provides excellent customer service to all internal and external customers.Meet personal and team qualitative and quantitative requirements.Schedules and cancels appointments according to preset scheduling guidelines.Answers general questions about STRIDE CHC.Informs and guides patients to resources within or outside of STRIDE CHC .Assists callers to STRIDE CHC with medical records requests and specialty referral requests.Routes messages to clinic staff following STRIDE CHC policies and procedures.Maintains communications and collaborate with patient, physician, payer representative and STRIDE CHC staff.Makes outbound calls to patients as directed by CCC Management.Responds to messages in the STRIDE CHC Patient Portal.Enters and updates patient information into patients Electronic Health Record.Meets required monthly metrics and goals.Stays up to date on all organizational and departmental means of communication (e-mail, Teams, Instant Messaging, etc.)

    Education or Formal Training:

    High school diploma or GED required.

    Experience:

    3 years related experience or customer service experience preferred.

    Work Schedule

    We offer a competitive hourly range of $20.00-$21.81, depending on your experience.

    This range reflects STRIDE’s good faith estimate of potential compensation at the time of posting. The final salary for the selected candidate will be determined based on several factors, including experience, education, budget, internal equity, specialty, and training.

    Your role:

    The Supervisor - Medical Assistant provides supervision, training, and coaching to Medical Assistants under the supervision of the Clinical Nurse Director. This role includes direct patient care time (75%) and administrative time (25%). Administrative time includes assisting with timecard review and approval, scheduling, staffing, training, progress reviews, disciplinary action, and site meetings.


    Essential Duties and Responsibilities:

    Supervisory Responsibilities

    Collaborate with other site-based leaders in establishing, implementing and maintaining quality patient care, improve quality metric standards and meet the external client expectations. Direct training/supervision/coaching of Medical Assistants including performance appraisals, in consultation with other site-based leaders. Review and monitor EHR tasks (e.g. Individual and Pool In Baskets, Schedules) to ensure accurate and timely completion of patient care and follow-up. Responsible for completing monthly competencies for staff under their supervision. Demonstrate knowledge of all policies and procedures pertinent to their work area and staff under their supervision. Assist Clinical Nurse Director with schedule efficiency, coordinating vacation requests and sick calls to ensure appropriate clinic coverage. Ensures opening and closing duties are completed (including laboratory and medications) and understands quality controls in the laboratory. Ensure clinical support/back-office manuals and logs are maintained. Has knowledge of the quarantine process. Knowledge of storage, handling, ordering, and reconciliation Vaccine for Children supplies. Supports the ordering of clinic supplies. Other duties as assigned.

    Non-Supervisory Responsibilities

    Performs examination, diagnostic, and treatment room procedures within scope of practice and in accordance with organization policies, procedures, and workflows. Maintains safe, secure, and healthy work environment by following standards and procedures; complying with legal regulations; appropriately responding to emergency codes; communicating safety issues/concerns timely and appropriately.Maintains patient care and non-patient care records in a confidential, accurate, and timely manner.Prepares paperwork for provider signature by completing as much information as possible before submitting to provider. Escalates patient/customer concerns to professional/licensed staff when appropriate. Performs opening and closing lab procedures including quality control, autoclaving, and packaging of instruments.Examples of routine work/tasks include but are not limited to: coordinating a pre-visit planning session (i.e. huddle), rooming patients in preparation for their visit, pre and post visit patient care in accordance with STRIDE policies, performing “Nurse” or “Medical Assistant” visits for specimen collection or protocolized care, scheduling internal/external patient appointments, instructing/educating patients on topics appropriate to scope, maintaining adequate inventory of supplies and notifying clinic leader when supply ordering is required. Registers, checks-in, and checks-out patients for all services.Collects and uploads all necessary documentation and consents.Promptly and accurately updates patient records including financial and demographic information.Collects copayment and account balances at time of service.Completes annual competency assessment.     General Purpose: This Licensed Master’s Level Clinician (LCSW, LPC, LMFT) provides clinical mental health services to patients in our highly integrated primary care setting. Behavioral Health Providers (BHPs) work collaboratively with primary care and dental providers to reduce barriers to mental health care. Much of the day is spent providing brief screenings, assessments, psycho-education and brief therapeutic services in the medical exam room or an in-clinic consultation room. BHPs carry a small caseload of patients for therapy services. BHPs may also work in collaboration with internal and external psychiatric providers to enhance the care of patients taking medication. This position is entirely clinical in nature and responsibility.

    General Purpose:

    In order to ensure efficient, quality healthcare services, the Nurse Practitioner/Physician Assistant provides general patient care. Additionally, the NP/PA provides clinical leadership to other providers around services within their scope of practice.


    Essential Duties/Position Responsibilities:

    Provides direct patient care to pediatric patients age 1 day to 20 years of age.Conducts well child checkup visitations and addresses acute and chronic concerns.Arranges referrals and consultation for patients with healthcare needs beyond the scope of practice.Communicates and coordinates with hospitals for transfer of care.Offers clinical consultation and arranges referrals to other providers and clinical staff.Reviews labs, x-rays, and ultrasound reports and develops a plan of care when abnormal.Prescribes medications to patients, including counseling patients regarding the proper use and contraindications of medications as well as detect and diagnose adverse drug reactions.Evaluates medical charts to determine appropriate patient care follow up and education regarding acute and chronic illnesses.Accurately and completely charts information regarding patient interactions into the Electronic Health Record (EHR) in a timely fashion.Works within and supports a team based and integrated care model, which includes medical assistants, other medical specialties, dental, behavioral health, patient navigation, and care coordination.Attends clinic meetings, specialty meetings, provider meetings, all staff meetings, and all other meetings as assigned.Helps determine topics of interest for specialty meetings to evaluate the quality and efficiency of patient care needs.

    General Purpose:

    In order to assist clinical staff with patients’ health related needs, the Radiology Technician administers x-rays to patients for diagnostic purposes.


    Essential Duties/Position Responsibilities:

    Operate and oversee operations of radiology equipment and produce images of the body for diagnostic purposes.Explain procedures and observe patients to ensure safety and comfort during X-ray procedure.Position and immobilize patient on examining table.Position imaging equipment and adjust controls to set exposure time and distance, according to specification of examination.Use safety measures and protection devices to comply with government regulations and to ensure safety of patients and staff.Review and evaluate developed x-rays, video tape, or developed computer generated information to determine if images are satisfactory for diagnostic purposes.Review and evaluate to determine if images are satisfactory for diagnostic purposes.Set up examination rooms, ensuring that all necessary equipment is ready.Record, process and maintain patient data and treatment records, and prepare reports.Coordinate work with clerical personnel and other technologists and consult with clinical staff.Demonstrate new equipment, procedures, and techniques to staff, and provide technical assistance.Perform scheduled maintenance and minor emergency repairs on radiographic equipment

    Education and Experience:

    Required: valid ARRT certificationRequired: Valid and current Radiologic Technologist license2+ years’ experience in a clinical care settingAssociates degree in X-Ray technology required; bachelor’s degree preferred.
