    REPORTS TO: Senior Program Manger

    HOURS: 40 hours per week

    CLASSIFICATION: Exempt

    SUMMARY: The Assistant Program Manager is responsible for the daily program operations of Step Up’s DMH FSP, OCS, HSSP, and CGF programs. The Assistant Program Manager leads, administers and oversees program services and directly supervises team members who assume responsibility for providing services to Transitional Aged Youth (TAY) and Adults experiencing serious mental health issues. Services are in accordance with DMH/Medi-Cal services: comprehensive mental health services and psychosocial rehabilitation, targeted case management, housing first, medication management, and crisis intervention in a field and office based setting. The program provides 24/7 on call coverage to clients and family members to provide the highest quality of life in the least restrictive community setting possible.

    DUTIES: The following reflects essential functions for this job but does not restrict other tasks, which may be assigned:

    Leadership, Administration, Oversight

    Direct day-to-day clinical operations. Supervise, manage and support team members to ensure appropriate coverage and quality clinical services to the client census.Oversee program outcomes to meet contract budget and revenue requirements.Signature Authority for direct reports, timesheets, mileage, check requests and performance evaluations.Participate in DMH, QA, Continuum of Care (COC) committees and meetings as requested.Collaborate and strategize with Mental Health and Homeless Network providers and communicate with non-Step Up providers to engage in interventions strategies.Attend regular Step Up management level meetings as well as local service area meetings as assigned.

     

    Direct Service and Support

    In collaboration with Director of DMH Clinical Programs, takes a lead role in staff hiring and on boarding process. Ensure team members have proper training and understand their job and role on the team. Connect staff to QM or clinical supervisors as needed for additional training and guidance to ensure staff’s work is in line with Step Up’s Core Values.In coordination with the Directors, communicates with County DMH and community referrals; coordinates client outreach and admission process.Works in collaboration with a dynamic and progressive array of Community Partners focused on ending homelessness in Los Angeles.Coordinates day and on call schedule and coverage. Provides clinical direction and guidance to team members on progress of all cases.Assume responsibility for any outside communication written or verbal, with team members.Provide clinical support and supervision to team members, referring any administrative and/or clinical issues outside scope of practice to Director of DMH Clinical Programs, Vice President of Programs, or Chief Programs Officer.Coordinate Nursing and Psychiatrist services with Director of Nursing.Coordinate Education and Employment Services with IPS/Vocational Program Manager and Chief Vocational Officer.

     

    Documentation and Data Collection

    In coordination with the Quality Assurance Manager, implement and follow Quality Assurance activities to meet Medi-Cal Standards and protocols; monitor Electronic and paper documentation and charts; determine and review appropriate use of program services, accuracy of documentation and third-party payment requirements compliance and ensure staff mastery of QA requirements. Authorize or co-sign paperwork as warranted.Will become or is LPS designate for program for emergency psychiatric assessments, 5150 evaluations and hospitalizations. Serve as backup on call and assume primary “on call” as needed.Other Duties as assigned.

     

    SKILLS: Knowledge of or experience with DSM 5 diagnoses, assessment of level of functioning, DMH documentation, EBP’s such as Motivational interviewing, DBT, Cognitive Behavioral Therapies, Trauma Informed Care, Housing First, etc. Ability to work independently and on a collaboratively team. Initiative and solution focused practice. Uses good time management skills and resources to balance case load direct service and paperwork.

     

    QUALIFICATIONS: Licensed Clinical Social Worker required or LMFT a minimum of 3 years experience working with adults recovering with chronic mental illness in psychosocial trauma informed program, knowledgeable about the (P)ACT and Positive Youth Development (PID) models preferred. Minimum of 3 years experience working with adults recovering with chronic mental illness. Experience supervising staff and interns, extensive knowledge of entitlements, excellent case management skills, communication skills and experience working on a team with paraprofessionals. Candidate must be computer literate with fluent Windows applications. Candidate must have own transportation and current California driver’s license and insurance. Position requires frequent driving and transporting.

     

    SUPPLEMENTAL INFORMATION: Work with people experiencing homelessness whether on the streets, in shelters or other places of habitation or services, may present inherent challenges and difficulties such as: exposure to bed bugs or other infestations, unpleasant smells or odors, individuals who may have poor hygiene or unclean homes due to homelessness, mental health symptoms, or poverty. Additionally, in working with our members, employees may encounter instances of profanity, sexually explicit or derogatory language, or verbal or physical expressions of anger and trauma. Very rarely do these situations ever escalate to physical contact, and negative or derogatory communication patterns can often be negotiated successfully. All direct care staff are provided adequate training to develop skills to deescalate crisis situations that may arise and Step Up has established procedural safeguards for all employees to mitigate (but not eliminate) these inherent risks of employment.

     

    PHYSICAL REQUIREMENTS: While performing the duties of this job, the employee is regularly required to sit; use hands and fingers; handle or feel; reach with hands and arms; talk; and hear. The employee is frequently required to walk, balance, stoop, kneel, and/or crouch. The employee must occasionally lift and/or move up to 15 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus. Keyboard data entry required.

    SUMMARY: Partners for Home Rapid Housing Initiative Permanent Supportive Housing Program provides mental health services and care coordination to Adults (18+ years of age) in community-based settings. We serve individuals who have been diagnosed with a mental illness, substance abuse and who have experienced chronic homelessness, have experienced homelessness in the recent past or are in danger of losing their home. The Rapid Housing Initiative Permanent Supportive Housing Program aims to provide linkage to resources including mental health services, housing, benefits, and is geared toward promoting wellness, mental health management and community integration. The program provides 24/7 on call coverage to members to provide the highest quality of life in the least restrictive community setting possible. Our work at Step Up is guided by the principles of Housing First, Mental Health Recovery and Trauma Informed Care.

    DUTIES

    Leadership, Administration, Oversight

    Assess homeless adults for mental illness, co-occurring substance use and other health care needs in the community until linkage and services are accepted.Attend all staff and community meetings as requested by Program Manager.Engage in all scheduled supervision with management to consult about caseload and discuss any pertinent issued related to consumer care and/or documentation.

     

    Direct Service and Support

    Completes comprehensive initial and ongoing assessments for diagnosis and evaluation of level of functioning, support network, adequacy of living arrangements, financial status, physical health, level of self-care.Completes intake paperwork including biopsychosocial assessments, treatment plans, safety plans, service notes and other required paperwork within 72 hours or contracted time frame.Provides clinical therapeutic and case management services for adults with psychiatric disabilities to assist them in improving their current level of functioning in the community.Attend medical or mental health appointments with clients to advocate and ensure comprehensive care and case coordination occurs.Make home visits based on members’ service level to support independent living skills and subsidized housing certificates. Assist members with relevant skill-building and self-care to support independence and housing retention.Monitor for compliance, stability, and report any warning signs of decompensation and crisis.Assist members in identifying needs, setting goals, establishing concrete objectives and developing a Coordinated Care Plan with a set timeframe from enrollment and annually.Actively outreach clients on caseload who have not been in contact and ensure engagement in services.Provide therapeutic interventions needed: individual, group and crisis intervention, to address symptoms as defined in the assessment and to improve level of functioning or develop insight to reduce defined distress or stressors.

     

    Documentation and Data Collection

    Complete all program-required paperwork such as ClientTrack and Welligent, within prescribed timeframes as outlined in SUOS documentation policy.Maintain member’s chart on EHR system by ensuring all relevant documents pertaining to client care have been scanned into attachments.Work cooperatively with other program staff including sharing responsibility for 24 hour on-call coverage.Perform all other duties as assigned.

     

     

    SKILLS: Knowledge of or experience with mainstream benefits and services, mental health diagnoses and level of functioning, EBP’s such as Critical Time Intervention, Motivational Interviewing, Trauma Informed Care, Housing First, Harm Reduction, etc. Ability to work collaboratively with others and on a team. Initiative and solution focused practice. Uses good time management skills and resources to balance case load direct service and paperwork. Works within skills and scope of practice asking for support and training to improve abilities when needed. Ability to engage individuals in a meaningful manner which imbues trust enough to accept services offered. Understands and promotes safety first to self and team members.

     

    QUALIFICATIONS: Master's degree in human services and license eligible. Must have experience working with mentally ill adults and those who have experienced chronic homelessness in a community setting. Case management experience is essential, also having the ability to chart patient progress to meet documentation requirements. Interview will involve a review of this ability. Must have the flexibility to be “on call” as part of rotating schedule. Candidate must have own transportation and current Georgia driver’s license and insurance.
