Job Description
We are seeking a dedicated and compassionate Early Intervention Service Coordinator to support infants and toddlers (birth–3 years) with developmental delays and their families. The Service Coordinator plays a vital role in helping families navigate the Early Intervention (EI) system and access appropriate services.
Service Coordinators provide case management, coordinate evaluations and services, and ensure compliance with New York State Early Intervention Program guidelines.
Responsibilities
Coordinate and manage a caseload of children enrolled in the Early Intervention ProgramFacilitate development and ongoing review of the Individualized Family Service Plan (IFSP)Conduct intake visits and ongoing family assessmentsCollaborate with therapists, evaluators, and service providersMonitor service delivery to ensure timely and appropriate servicesMaintain accurate and timely documentation in compliance with EI regulationsProvide families with information, referrals, and advocacy supportAttend meetings with families and multidisciplinary teams
Qualifications
Minimum 2 year experience working with children or families (Early Intervention)Knowledge of NYS Early Intervention Program and IFSP process requiredStrong organizational, communication, and interpersonal skillsAbility to work independently and manage multiple casesBilingual (Spanish) a plus
