    Job Summary

    The Executive Assistant provides high-level administrative, operational, and project management support to Executives and members of the Leadership Team. This position is responsible for ensuring efficient day-to-day business office operations, coordinating strategic initiatives, managing complex projects, and facilitating communication across internal and external stakeholders. The Executive Assistant serves as a trusted partner to leadership by anticipating needs, identifying and resolving issues, streamlining processes, and ensuring organizational priorities and deadlines are achieved. This role upholds the mission of the Company by enabling leaders to focus on strategic goals, regulatory compliance, and resident/participant-centered care.

    Duties & Responsibilities

    Provide high-level administrative and operational support to Executives and the Leadership Team.Manage complex calendars, schedule meetings, coordinate appointments, and prioritize competing demands on executive time.Lead and coordinate assigned projects and organizational initiatives from planning through completion, ensuring timelines, deliverables, and objectives are met.Oversee daily business office operations to ensure efficiency, organization, and effective workflow management.Anticipate executive and organizational needs by proactively identifying challenges and implementing solutions.Prepare correspondence, reports, presentations, agendas, and meeting materials with accuracy, professionalism, and attention to detail.Support leadership and governance initiatives, including scheduling meetings, preparing agendas, recording minutes, and maintaining corporate records.Serve as a liaison between Executives, internal departments, external partners, vendors, regulatory agencies, and community organizations.Coordinate travel arrangements, expense reporting, reimbursements, and other executive support activities.Manage facility security administration, including overseeing employee badge access, issuing and deactivating credentials, maintaining door access permissions, and ensuring building security protocols are followed.Monitor project timelines, compliance reporting requirements, action items, and organizational deliverables.Research, analyze, and recommend solutions to operational issues and process improvement opportunities.Maintain confidentiality of all sensitive organizational, participant, employee, and business matters.Organize and maintain electronic and paper records in accordance with organizational and regulatory requirements.Assist with special events, meetings, and organizational initiatives as assigned.Provide mentorship, guidance, and support to administrative staff and team members as appropriate.

    Qualifications & Requirements

    EducationAssociate's or Bachelor's degree in Business Administration, Healthcare Administration, Project Management, or a related field preferred.ExperienceMinimum of 3–5 years of experience supporting senior executives, managing business operations, or leading projects in an executive administrative capacity. Healthcare, PACE, nonprofit, or senior services experience preferred.

    Exceptional organizational, planning, and time-management skills with the ability to manage multiple priorities simultaneously.Strong project management skills with the ability to coordinate cross-functional initiatives and meet deadlines.Proven ability to anticipate needs, exercise sound judgment, and resolve issues independently.Excellent written and verbal communication skills with the ability to interact effectively at all levels of the organization.Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and electronic records management systems.Ability to work independently while collaborating effectively with diverse teams and stakeholders.High degree of professionalism, discretion, and confidentiality.Knowledge of healthcare regulations, PACE program structure, or senior care services preferred.Demonstrated ability to adapt in a fast-paced environment and manage changing priorities.

    Job Summary: The Physical Therapist (PT) is a key member of the interdisciplinary team (IDT) at St. Paul's PACE, responsible for designing and implementing a physical therapy program to help participants maintain independence and live safely within the community. The PT conducts participant assessments and collaborates with the IDT to develop personalized care plans, focusing on either acute restorative or maintenance levels of care, and determining the most effective strategies to optimize function. The PT also oversees the execution of these care plans, performs reassessments as needed, and works closely with the IDT to ensure the highest quality of care. Services are delivered across a variety of settings, including the Day Center, PACE Center, nursing homes, and in-home environments. Assists in the ordering, inventory, distribution and maintenance of durable medical equipment for participants.

    Education Required:

    Graduate of a PT curriculum accredited by the American Physical Therapy Association, the Committee on Allied Health Education and Accreditation of the American Medical Association, and the Council on Medical Education of the American Medical Association.

    Experience Required:

    At least one-year experience as a PT working with geriatric populations.

    Certifications Required:

    Licensed Physical Therapist in the State of California (CA)

    Job Summary: Under the direction and supervision of the Clinical Services Manager, the RN Case Manager is responsible for coordinating comprehensive participant care in collaboration with an interdisciplinary team. This role ensures timely assessments, medication management, care planning, and participant education in accordance with CMS guidelines and organizational standards. The nurse serves as a key clinical liaison between participants, families, PCCs, providers, pharmacies, and home care services to promote safe, effective, and continuous care.

    Duties and Responsibilities:

    Performs the administration of medications, including oral, topical, intravenous, vaccinations and injectable forms in the clinic in accordance with nursing standards. Records all medications and treatments administered.Coordinate comprehensive participant care in collaboration with the interdisciplinary team which includes attending IDT meetings and participating in Plans of Care meetings. Conduct comprehensive initial, semi-annual, annual, and change-of-condition assessments in compliance with CMS guidelines. Coordinates with the interdisciplinary team to develop a comprehensive care plan for participants. Travel to the participant’s location as required.Maintains participants’ medical record and fulfills CESD charting and reporting requirements. Minimally provision of Quarterly narrative nursing case management note, unless a participant’s condition requires a more frequent note, indicating participant’s progress toward achieving health goals.Determine nursing level of care in accordance with regulations as delegated by Clinical Services Manager.Case Management of participants on end-of-life services to complete initial admission visits. Complete in-home visits for nursing services outside the scope of LVN In-home Service nurses.Develop, update, and maintain accurate nursing care plans that reflect nursing services provided.Manage medication-related processes including executing new orders, reordering medications, coordinating pre- and post-operative medications, coordinating pharmacy services and medication deliveries.Communicate updates to Care Kinesis regarding participant contact information changes, including address updates.Maintain and update participants’ Medicare Part A status, including completion and submission of the Part A Notification Form.Complete monthly medication verification and reconciliation for participants receiving medication management servicesFacilitate discharge planning from hospitals or skilled nursing facilities, including management of new prescriptions, ordering DMW, pharmacy coordination, and medication delivery.Assist with utilization reduction by reviewing hospital documentation for discharge appropriateness and referring to the End-of-Life program when appropriate.Collaborate with homecare team members to coordinate supplies, medications, and in-home nursing services.Shall attend in-service education program, including orientation and skill training and continuing education.Shall receive in-service training in first aid and in cardiopulmonary resuscitation within the first six months of employment.Follows all St. Paul’s PACE Policies and Procedures and OSHA safety guidelines.Maintains the confidentiality of all company procedures, results, and information about participants, clients, or families.Practices Universal precautions and follows appropriate Infection Control procedures.Maintains a safe working environment.     Essential Duties and Responsibilities:

    Review requested work for Maintenance & Housekeeping and assign to appropriate staff

    Review requested work and change customer assignment as needed.

    Assist with Quality Control and Clarity Assurance. Add notes and/or change values to work orders as required. Clarifications, roadblocks, time values.

    Monitors incoming work requests and contacts appropriate Facilities Leadership to discuss repeat requests that have potential risks or hazards.

    Creates work orders for vendor work to track activities related to costs, completion, need for follow-up, and to ensure asset history is maintained.

    Creates work orders for items identified during rounding and internal inspections.

    Reviews completed work orders to verify completeness, identify possible issues, and the need for vendor assistance.

    Routinely reviews requested or Corrective Maintenance work order status to ensure they are on time.

    Communicates with departments to give status updates.

    Runs Preventive Maintenance tasks reports related to regulatory compliance.

    Maintains regulatory compliance manuals ensuring up to date records for audit readiness.

    Ensures weekly Preventive Maintenance work orders generate appropriately related to assignment, and asset detail.

    Ensures unassigned Preventive Maintenance work orders are assigned to staff based on availability and appropriateness.

    Creates new Preventive Maintenance tasks and cycles typically following audits or inspections

    Routinely reviews Preventive Maintenance work order status to ensure they are on time.

    Creates work orders shared by email or verbally.

    Assists with special projects as needed.

    Qualifications and Requirements:

    Education: High school graduate or its equivalent. Experience: General knowledge of building maintenance operations and terminology is preferred. Must be able to understand basic technical concepts and tasks. Must be able to function independently with minimum guidance and direction. Must have strong computer skills and be able to utilize appropriate application software.

    Help develop and implement activity programs within the framework of the needs and ability of memory impaired residents and State and Federal regulationsCoordinate programs in a team manner with related Villa services and staff. Will implement activities that are both group and individual participation in nature.Develop activities providing the opportunity for the participants to experience sensory input, touch, smell, taste, etc. as well as group interaction and personal achievement.Include activities which shall encourage participants to make decisions, participate in planning and assume some degree of responsibility and independence.Help develop weekly schedule of all activities, to be coordinated with Activity Coordinator.Evaluate each participant according to his/her background, interest, abilities, physical and mental limitations and needs.     PHYSICAL REQUIREMENTS:

    St. Paul’s PACE is seeking a Primary Care Physician that can:

    Coordinate with the interdisciplinary team to develop a comprehensive care plan for each participant.Integrate the primary care treatment plan into the overall plan of care developed by the interdisciplinary team.     Key Responsibilities:

    As a Primary Care Physician at St. Paul’s PACE, you will:

    Deliver high-quality, comprehensive primary care to frail older adults in coordination with an interdisciplinary teamPerform initial, semi-annual, and ongoing assessments to develop individualized care plansIntegrate the primary care treatment plan into the broader plan of care developed by the interdisciplinary teamEvaluate and manage chronic conditions and acute illnessesProvide preventive care, including immunizations, screenings, and health maintenanceParticipate in care planning and discharge decisions for hospitalizations or long-term placementCommunicate effectively with nursing staff, specialists, and other providers to ensure seamless, coordinated careMake home, assisted living, and nursing facility visits as neededProvide ongoing clinical support and training to staffParticipate in team meetings, peer review, quality improvement, and compliance effortsContribute to clinical policy development and program evaluation under the guidance of the Medical Director

    Education: MD/DO with interest in geriatrics. Board certified or Board eligible (if newly graduated from residency) in Internal Medicine, Family Practice, or Geriatric Medicine desired. Current state license and DEA registration required. Meets requirements of 42 CFR Part 410.20.

    Experience: A minimum of one year experience working with a frail or elderly population required. Shall have either training or related experience in the assigned job.
