    Job Summary:

    The ACT Registered Nurse (RN) is an integral member of the ACT multidisciplinary team and is responsible for conducting psychiatric assessments, assessing physical health needs, making appropriate referrals and collaborating with community providers, providing management and administration of medication in conjunction with the psychiatrist, delivering a range of treatment, rehabilitation, and support services, and sharing case management responsibilities with other staff. The Registered nurse provides chart management for an assigned group of clients, including responsibility for developing, writing, and revising overall treatment goals and plans in collaboration with other team members. This position also includes educating and supporting clients’ families and advocating for clients’ rights and preferences.

    Duties/Responsibilities:

    The Registered Nurse provides nursing care and health education to individuals served by the ACT team, with the majority of services delivered in the community (e.g., individual’s home, hospitals, shelters, etc).


    Services include:

    Manage the medication system, administer and document medication treatment.

    Transcribe and fill prescription orders.Oversee and monitor medication storage and inventory. Assemble medication deliveries, manage IM injections, and maintain accurate documentation (e.g.,MAR). Communicate medication changes with the team in real-time.

    Screen and monitor individuals for medical problems/side effects.

    Conduct regular screenings and assessments for medical conditions and medication side effects. Monitor and response to pre-existing medical conditions.

    Communicate and coordinate services with other medical providers .

    Document services in the Electronic Medical Record. Participate in daily team meetings and treatment planning reviews. Attend medical appointments with clients when possible. Maintain communication with inpatient/outpatient providers using health communication forms.

    Engage in health promotion, prevention, and education activities.

    Provide symptom education to improve self-awareness and self-management. Assess and support behavior change around risky behaviors. Offer education and support around chronic illness, preventative screenings, and wellness. Promote smoking cessation and health self-management strategies.

    Educate team members about psychiatric symptoms and side effects.

    Provide formal and informal training to team members regarding medication side effects and symptom management. Contribute to team-wide understanding during daily meetings.

    Support medication adherence strategies.

    Collaborate with the psychiatrist and team to improve adherence using tools such as behavioral tailoring, pill organizers and reminders, long-acting injectable medication when appropriate, etc.

    Miscellaneous:

    Participate in rotating on-call responsibilities. Provide psychiatric rehabilitation and ADL support in the community, including, but not limited to maintaining housing, supporting personal hygiene, nutrition, and household management, assisting with accessing entitlements or employment, coordinating primary and specialty medical care, and completing miscellaneous duties and projects based on client needs. Maintain accurate and timely documentation of clinical records in accordance with Federal, State, County, and treatment program requirements under the direction of the clinical supervisor. Meet performance expectations for time and billing standards. Participate fully as a member of the Spero Assertive Community Treatment Team as a supportive, reliable, positive, collaborative, and professional team member during the development of individual treatment planning, daily team meetings, and crisis intervention. Participate in continuing education and professional development activities to maintain certification and stay current with best practices in peer support.

    Required Knowledge, Skills, and Abilities:

    Understanding of approaches and interventions used to support others in recovery and resiliency, demonstrating efforts as self-directed recovery. Knowledgeable of community resources and how to connect individuals to those resources. Ability to understand and carry out directions and instructions; ability to maintain confidentiality; interest and skill in providing services to individuals and groups; ability to speak, read, and write English. Ability to provide documentation consistent with department standards for quality and timeliness; willingness to work hours conducive to reaching the identified population.Ability to establish and maintain cooperative working relationships with individuals, coworkers, and other agencies. Ability to work independently and as part of a multidisciplinary team. Valid Minnesota Driver’s license and ability to transport clients.     Education and Experience:

    The Registered Nurse may have a master’s, bachelor’s, or associate degree in nursing, must have an unencumbered license to practice as a registered nurse in the state of Minnesota and should have at least one year of experience in an inpatient or outpatient psychiatric setting.


    Physical Requirements:

    Operates a motor vehicle; sees using depth perception, stands; sits; moves about the work area; clients 1 floor of stairs; lifts, pushes, pulls, moves carts and carries items weight up to 5 lbs.; operates a keyboard; speaks with a clear and audible voice; reads a 12pt. font; hears a normal speech level; works in clients homes or other areas exposed to dust or second-hand smoke; may work in uneven areas.

    This position is responsible for carrying out the job duties of Children’s Mental Health Case Management. The case manager is responsible for assessing client needs, planning for service implementation, coordinating services on behalf of clients, and monitoring services for consistency and efficiency. Case managers are expected to interact professionally with internal and external community providers, perform related work tasks as required or assigned and comply with Minnesota Rule 79 in delivery of Children’s Mental Health Case Management services.

    Case managers may be assigned to perform job responsibilities in one or more than one of the counties under the jurisdiction of Minnesota Prairie County Alliance. Under the direct supervision of Director of Children’s Services - SPERO and assigned Minnesota Prairie County Alliance Child and Family Supervisor(s).

    Deliver services according to SPERO and State of Minnesota guidelines for Children’s Mental Health Case Management.Maintain a variety of records, including individual client records, time records and travel records in a timely manner. Assure that client insurance coverage is requested at intake and updated on an ongoing basis. Notify and consult client’s assigned mental health professional, Minnesota Prairie County Alliance supervisor, and SPERO Clinical Supervisor when client experiences a significant change in status. Review and update the Individual Treatment Plan (ITP) according to changes in client status, or at least every 6 months, documenting clients’ progress during the previous 6 months. Coordinate referrals of client and family to community resources needed to meet client needs and the ITP goals and objectives.Develop and/or review Crisis Response Plans /Safety Plans with clients and families semi-annually or more frequently as needed.Assist other Case Managers with urgent tasks as needed.     b. is fluent in the non-English language of the cultural group to which at least 50% of the practitioners recipients, completes 40 hours of training in the delivery of services to recipients with mental illness, and receives clinical supervision from a mental health professional at least once a week until the requirements of 2000 hours of supervised experience are met

    B. Has at least 6000 hours of supervised experience in the delivery of mental health services to recipients with mental illness. Hours worked as a mental health behavioral aide I or II under CTSS may be included in the 6000 hours of experience for child recipients.

    C. Is a graduate student in one of the mental health professional disciplines and is formally assigned by an accredited college/university to an agency or facility for clinical training.

    D. Holds a master’s or other graduate degree in one of the mental health professional disciplines from an accredited college/university.

    E. Tribally certified mental health practitioner who is serving a federally recognized Indian tribe.

The peer specialist utilizes their lived experience as a recipient of mental health services for severe and persistent mental illness to provide essential peer support. This role focusses on fostering hope, promoting recovery, and assisting individuals in achieving their personal goals through direct support, advocacy, and collaboration. The role works closely with other team members to promote a team culture that recognizes, understands, and respects each client’s point of view, experiences, and preferences.

    Duties/Responsibilities:

    The Mental Health Certified Peer Specialist I and/or II will provide advanced support, encouragement, and assistance to individuals and groups enrolled in Spero’s Assertive Community Treatment Team, with the majority of services provided in the community (i.e., individual’s homes, hospitals, shelters, etc.). Services include:


    Coaching and Consultation to promote recovery and self-direction

    • Mentoring through appropriate self-disclosure of personal recovery experiences to develop trusting and meaningful relationships with members, while also modeling self-help and wellness strategies.

    • Provide outreach to engage participants and assist them in overcoming barriers to participating in services.

    • Offer support to develop personal recovery and wellness goals, explore self-help strategies, review dimensions of wellness, engage in wellness strategies, and act as a peer advocate. Provide skills training in daily living skills or other areas as indicated by the participant’s treatment plan.

    • CPS Level II: Assist in the development and implementation of individualized recovery plans.


    Facilitating wellness management and recovery strategies

    • Facilitate and connect individuals to services and activities with a focus on building self-efficacy.

    • Facilitate weekly Wellness Recovery Action Plan groups.

    • Assist in linking peer and community resources to build natural support.

    • Promote skills building activities in the local community to support personal wellness and ongoing recovery.

    • Provide supportive navigation, information, and coaching while acquiring services related to medical, social, educational, transportation, housing, education, employment, and other applicable services.

    • CPS Level II: Facilitate peer support groups and workshops.


    Modeling skills for, and providing consultation and collaboration to fellow ACT team members

    • Provide expertise, experience, and consultation to the team to promote a culture where an individual point of view is recognized, respected, and integrated into treatment.

    • Provide cross-training to other ACT team members in recovery principles and strategies at least bi-annually.

    • CPS Level II: Collaborate with other mental health professionals to ensure comprehensive care.


    Miscellaneous

    • Maintain accurate and timely documentation of clinical records in accordance with Federal, State, County, and treatment program requirements under the direction of the clinical supervisor.

    • Meet performance expectations for time and billing standards.

    • Participate fully as a member of the Spero Assertive Community Treatment Team as a supportive, reliable, positive, collaborative, and professional team member during the development of individual treatment planning, daily team meetings, and crisis intervention.

    • CPS Level II: Participate in continuing education and professional development activities to maintain certification and stay current with best practices in peer support.


    Required Knowledge, Skills, and Abilities:

    Understanding of approaches and interventions used to support others in recovery and resiliency, demonstrating efforts as self-directed recovery. Knowledgeable of community resources and how to connect individuals to those resources. Ability to understand and carry out directions and instructions; ability to maintain confidentiality; interest and skill in providing services to individuals and groups; ability to speak, read, and write English; ability to provide documentation consistent with department standards for quality and timeliness; willingness to work hours conducive to reaching the identified population; ability to establish and maintain cooperative working relationships with individuals, coworkers, and other agencies. Ability to work independently and as part of a multidisciplinary team. Valid Minnesota Driver’s license and ability to transport clients. Ability and willingness to discuss recovering experiences during the interview process.


    Education and Experience:

    As defined by Minnesota State Statue 256B.0615 Subd. 5:


    A certified peer specialist II is qualified as a mental health certified peer specialist, as defined in section 245I.04 Subd. 10, which states a mental health certified peer specialist must:


    1. Have been diagnosed with a mental illness;

    2. Be a current or former mental health services client; and

    3. Have a valid certification as a mental health certified peer specialist


    CPS Level II:

    4. Be qualified as a Mental Health Practitioner level as defined by Minnesota Statute 245.462, subd. 17:

    a. Have at least 6,000 hours of supervised experience in the delivery of peer services to persons with mental illness.

    b. Alternatively, have at least 4,000 hours of supervised experience in the delivery of services to persons with mental illness and an addition 2,000 hours of supervised experience in the delivery of peer services to persons with mental illness.

    Operates a motor vehicle; sees using depth perception, stands; sits; moves about the work area; clients 1 floor of stairs; lifts, pushes, pulls, moves carts and carries items weight up to 5 lbs.; operates a keyboard; speaks with a clear and audible voice; reads a 12pt. font; hears a normal speech level; works in clients homes or other areas exposed to dust or second-hand smoke; may work in uneven areas.

    This position operates within a dynamic and collaborative Assertive Community Treatment (ACT) team. You will work alongside a multidisciplinary team of professionals, including case managers, nurses, therapists, and psychiatrists, in a supportive and respectful environment. The ACT team provides services primarily in the community, including clients’ homes, community settings, and the office. This role requires flexibility and ability to adapt to changing situations. The team emphasizes a client-centered, recovery-oriented approach, valuing the unique experiences of each individual served.
