    The IOSS Therapist provides trauma-informed mental health therapy to youth clients of SLMH’s Intensive Community Treatment Services (IOSS) program as well as providing crisis support and intake assessments. The IOSS Therapist is imbedded in the IOSS Treatment team and collaborates with team members on clinical details of treatment for the client and family/community systems.

    Duties and Responsibilities:

    Provide Mental Health Therapy to Clients

    Build therapeutic relationships and provide individual therapy to youth clients of the SLMH’s IOSS program with emotional problems and/or psychiatric disabilities. Help these youth clients identify personal strengths and resources needed to acquire skills and supports while healing from past traumas and assist in developing and implementing culturally appropriate individualized and family plans in collaboration with other IOSS team members.Provide family support as necessary and appropriate, including providing support for parents and other caregivers of clients.Work with clients and IOSS Team members to provide therapy and support in the environment that provides the best opportunity for success. This may include conducting therapeutic sessions in the community or schools, over the phone, and/or conducting home visits as necessary and appropriate.Provide crisis service and assistance to other members of the IOSS team who are providing crisis support as needed during business hours.Conduct in-depth mental health assessments, intakes, and updates; develop individualized support plans; and document services as appropriate, both for assigned IOSS clients and for other IOSS clients as requested.Understand that many individuals have been traumatized and work to provide services from a culturally appropriate, respectful, trauma-informed perspective.

    Participate in the IOSS Treatment Team

    Regularly attend IOSS team meetings, and collaborate with other IOSS team members to identify strategies to provide quality care to IOSS clients.Coordinate care with other members of the IOSS team, other providers, family members, courts, schools, etc. as required to assist the client in successfully meeting individual goals.Participate in Child and Family Team Meetings and work with adult care providers (parents, foster parents, teachers) when appropriate to assist with improving the family system.Coordinate with internal and external team members as necessary.Assess safety of self and others in community/home situations, respond to safety concerns, and coordinate with the team to ensure appropriate safety while seeing clients.

    Complete administrative and continuing education responsibilities

    Schedule appointments with assigned clients, and coordinate and maintain appointment calendar.Complete all required paperwork, including notes on each session, individual client paperwork and annual assessments, in a timely manner.Meet requirements regarding productivity and dependability.Actively participate in both group and individual clinical supervision, as well as team and staff meetings.Engage in continuing education activities and trainings, maintain appropriate continuing education credits as required by board and licensure, and continually build therapeutic skillsParticipate in compliance and quality reviews as required.Positively represent SLMH in the community and with other service providers, and act as a liaison with other organizations or with internal committees/cross-departmental teams.Perform all duties in a respectful and responsible manner, with clients, their families and coworkers, and in a manner that meets all professional ethical standards.Additional duties as requested.

    Qualifications:

    Education and Experience:

    Master’s degree in the related fields of therapy to include social work, professional counseling, or couples and family therapy.LCSW, LPC, or LMFT preferred.

    Skills, Knowledge, and Abilities:

    Skill in building therapeutic relationships with clients in individual, family, or group settings and provide therapy within scope of training.Ability to conduct DSM 5 diagnoses and mental status examinations.Knowledge of theoretical frameworks for psychotherapy, as well as case management and skills training techniques and resources.Ability to determine appropriate treatment modalities and level of care for each client, including identifying when to focus on solution-oriented symptom management and when to focus on addressing underlying causes as the path to symptom reduction. Ability to write and implement an integrated service plan.Skill in de-escalating stressful situations and providing appropriate support to clients in crisis.Ability to clear conceptualize a case, and clearly communicate that conceptualization to others.Knowledge of best practice suicide risk assessment and safety planning techniques, and ability to assess health and safety risks.Ability to conduct mental health assessments according to standards set in Oregon

    Working conditions and physical requirements:

    This is a physically active role, located in partially an office environment and partially in many community locations, including client homes, and requires regular interaction and engagement with youth.     About the position

    South Lane Mental Health is looking for a Certified Peer Support Specialist to provide wellness management and recovery strategies as well as supportive employment and education support as a member of our Assertive Community Treatment team. As a Peer you will support program participants to maintain health, wellness, and stability in the community. ACT team members provide direct support to approximately 40 individuals with severe and persistent mental illness living in the rural communities of south Lane County. Services are provided in the community, individual homes, and at our office location.

    Position Duties Include:

    Provides supportive employment and education, vocational assessment, job development, and job placement and support services for program participants. Provide coaching and consultation to clients to promote recovery and self-direction and facilitate wellness management and recovery strategies for program participants.Provide ADL (Actions of Daily Living)skills training and support to clients of the ACT program as directed by their primary therapist and as part of a collaborative plan developed by the ACT team.Directly support clients to navigate everyday challenges in the community and in their homes including providing transportation and assistance for shopping, doctor appointments, social outings and other activities as directed by collaborative plan developed by the ACT team.Conduct individual and group skills training and development sessions with clients of SLMH’s ACT Program.     Position Summary

    The Center for Recovery Services Coordinator provides day-to-day support for the office and administrative functions of the Recovery Program, including serving as the first point of contact for new clients with a primary diagnosis of substance use disorder, and provide direct client support including individual and group counseling, transportation support, and other planned interventions while delivering direct client services aligned with individualized treatment plans.

    This is a full-time, non-exempt position with general office hours Monday through Friday, 8:30 a.m. – 5:00 p.m., including a 30-minute unpaid meal period, in accordance with Oregon labor law. Some duties require community-based work, including client transport and off-site service delivery.

    Key Responsibilities

    Administrative and Office Duties

    Serve as a trauma‑informed, welcoming first point of contact for clients and visitors, including answering phones, greeting clients, checking clients in for appointments, and maintaining an inclusive, respectful environment.Verify, update, and maintain accurate client demographic, insurance, and payment information in the electronic medical record (EMR).Educate clients about payment options for Recovery Services, including potential Oregon Health Plan (OHP) eligibility, and refer clients to the Outpatient Administrative Specialist for enrollment assistance as needed.Schedule and track client assessments and required updates; coordinate with clinical staff to support timely documentation and submission.Ensure clinical records, episodes, assignments, and closures are accurately entered in the EMR, including conducting periodic file health and completeness reviews.Maintain current Releases of Information and ensure compliance with documentation and reporting requirements, including MOTS and required status reports.Support team meetings by ensuring client updates and care coordination decisions are properly documented.Coordinate billing-related documentation and support timely submission to the SLMH billing department.Maintain office operations, including inventory and ordering of supplies, petty cash management, equipment upkeep, and ensuring shared spaces are safe, welcoming, and functional.Represent the Center for Recovery Services in interagency meetings and collaborative efforts as directed by SUDs program leadership.


    Peer Support and Direct Client Services

    Provide initial brief screening assessments for new SUD clients.Deliver peer support, recovery coaching, and skills-based interventions that promote recovery, autonomy, and self-direction.Provide individual and group skills development sessions consistent with ethical scope and credentialing.Support clients in developing and practicing alternatives to substance use, in coordination with the client’s primary counselor and care team.Assist clients in navigating community-based needs, including providing transportation support as identified in treatment plans.Apply evidence-informed, harm reduction–oriented approaches tailored to each client’s goals, readiness, and lived experience.Monitor client progress, adjust interventions as appropriate, and contribute to collaborative treatment planning and reassessment.Participate in complex case reviews and coordinate care with multidisciplinary SUDs team members.Provide services consistently through a trauma‑informed, respectful, and culturally responsive lens.Act as a liaison with community partners and internal teams as assigned.


    Administrative, Ethical, and Professional Standards

    Qualifications

    Education & Experience

    Oregon-registered Certified Alcohol and Drug Counselor (CADC) or Certified Recovery Mentor (CRM) required.Bachelor’s degree in social services, behavioral health, or a related field OR a minimum of three (3) years of professional experience in mental health or substance use disorder services.Valid Oregon driver’s license, acceptable driving record, and ability to pass a criminal background check, as required by law.

    About the position

    Assertive Community Treatment (ACT) is an Evidence-Based Practice designed to provide comprehensive treatment and support services to individuals who are diagnosed with serious and persistent mental illness (SPMI). All ACT services are provided by a multidisciplinary team and are designed to be provided in the most integrated setting possible to maximize independence and community integration. ACT Therapists/Social Workers are Masters level Qualified Mental Health professionals (QMHP) who provide direct support to program participants living in rural Cottage Grove and South Lane County. Therapist/Social Workers help ACT participants develop and implement individual plans to overcome barriers and to work through challenges they face in everyday life while also supporting participants to focus on long-term treatment goals for stability and well-being.

    Principal Duties and Responsibilities

    Provide therapeutic support to participants of the ACT Team

    Build therapeutic relationships with ACT program participants and provide individual therapy and case management services to individuals with severe and persistent mental illness living in the community. Help to identify personal strengths and resources needed to acquire skills and support while healing from past traumas and assist in developing and implementing culturally appropriate individualized plans in collaboration with other ACT team members.
