Job Title: Case Manager/Social Worker

Compensation: $28.00 to $32.00- Based on experience
Summary: The focus of this position is to provide specialized and progressive case management including screening, assessing, and developing case plans for individuals and families in need of services from SVdP. This position plays a key role in coordinating supports and processing referrals from multiple entry points (such as referrals from our helpline, our parish-based Vincentian volunteer, walk-ins at our Georgetown foodbank and our south end Latino core team). It may involve traveling to various sites to support the larger mission and engage in home visits from time to time.
Essential Duties & Responsibilities: The duties and responsibilities of this position shall include but are not limited to the following. Other duties and responsibilities may be assigned as needed.
Provide services such as planning, coordination, and application of progressive case management activities for individuals or families with multiple barriers who are referred by parish-based SVdP team called conferences and/or other agencies. Additional assistance may be given to individuals experiencing homelessness, formerly incarcerated, unemployed, experiencing mental and/or behavioral health concerns. Work within all existing policies and procedures for enrolling new clients and follow-up on existing clients.Maintain client progress notes, assistance and outcomes are up to date, which includes caseload reports to determine measurable results for grants. Assist in maintaining records of the disbursement of all check requests, vouchers, bus tickets and any other form of financial aid for clients on caseload by using the client database and developing monthly activity reports.Work in coordination with case management team, a network of St. Vincent de Paul (SVdP) volunteers in parishes and parish staff to assist individuals and families in receiving holistic, progressive and individualized support services towards self-sufficiency.Assist in the development and coordination of Council social service programs at the SVdP Food Bank and other satellite offices in King County.Use Cognitive Behavior Therapy and Motivational Interviewing with open-ended questions, affirmations, reflective listening, summarizing, coaching, supporting, and guiding the individual and family to achieve their goals.Stay up to date on other evidence-based practices and participate in subsequent trainings and adhere to those practices. Develop working relationships with other providers and service agencies, especially those who work with a similar population, and including employment agencies.May interface & collaborate with other Case Managers/SVDP staff and work with SVdP Volunteer Manager to expand opportunities for skills development.Minimum Qualifications: In order to perform the essential duties and responsibilities of this position, an individual must possess the knowledge, training, skills and experience indicated by the following:
Education/Experience
Bachelor’s degree in social work or in a related field. Or AA in related field with 4+ years of relevant work or volunteer experience.
Language Ability
This position requires excellent written and verbal communication skills, including ease in addressing groups of clients and communicating with diverse workgroups. Ability to read, analyze, and understand general business periodicals, journals, technical procedures, safety manuals, and regulations. Applicant must have the ability to write reports.
Computer Skills
Computer proficiency using Microsoft Office software is required. Familiarity with client-based software and email communications will be essential to regular communications and reporting.
Reasoning Ability
Must have the ability and creativity in resolving practical societal problems quickly and in situations where there is limited standardization. Ability to interpret a variety of instructions in written, oral, diagram, or schedule form is needed.
Certificates and Licenses
Social work related trainings and certificates are preferred.
Math Ability
Must have the ability to work with basic mathematical concepts. Must be able to understand common financial reports, payroll records, and cash management reports.

Summary: This position supports the De Colores program, a parenting support initiative through Centro Rendu. De Colores is a program funded by King County Best Starts for Kids, which serves Latinx families with children from pregnancy to age five who are living in South Seattle and South King County. This position will provide focused support to De Colores enrolled families with child development education and navigation support, working closely with parents/caregivers to assess and identify their children’s developmental needs, connect them to appropriate resources, and advocate on their behalf. This position works collaboratively with Centro Rendu, Best Starts for Kids, and leaders from the Latinx community.
Essential Duties and Responsibilities include the following. Other duties may be assigned:
Conduct assessments (ASQ) to determine the support needs of children with disabilities.Recruit program participants utilizing appropriate and useful methods.Coordinate outreach efforts with the program team.Prepare and coordinate community events and workshops, including one workshop per quarter focusing on raising awareness about developmental milestones, delays, and available supports.Develop collaboration with other agencies to create referral processes for new clients.Maintain professional records appropriate for the provision of program monitoring.Ensure that all activities are documented to comply with program and contract requirements, including accurate completion of all records pertaining to life skills education and home visits conducted.Maintain all St. Vincent de Paul (SVdP) required paperwork on all program participants served as well as data on our CRM. Participate in supervision, consultation, department meetings and in-service training.Meet individually with supervisor on a regular basis.Promote the SVdP Mission Statement in the community by attending networking meetings and community fairs as requested by the Program Manager.Liaise advocates for students with disabilities or special needs withother social service agencies as the program advocate.Treat program participants in a culturally sensitive manner, assuring that SVdP’s commitment to diversity and cultural competency is maintained.Qualifications:
Two years’ work experience in social services, working in the Latinx community, Racial Equity, preferably in parenting support; experience in child development and parenting curriculum. Strong understanding of disability laws, rights, and policies.
Must have reliable transportation, a valid driver’s license and automobile insurance; must be able to comply with the requirements of SVdP’s Employee Driving Policy.
Education/Experience
Associate of Arts in Early Childhood Education, Bachelor of Arts in Social Work, Human Development or related field or equivalent experience required; Bachelor of Arts in Early Childhood Education preferred; current CPR/First Aid certificates or ability to obtain; must be current on TB (tuberculosis) test or be willing to obtain test.
Language Ability
Must be bilingual in Spanish and English.
