    The Resident Service Coordinator organizes, implements and evaluates the Social Services program to meet the psycho-social needs including but not limited to: resident assessment, maintenance of resident records, coordination of services to meet the needs of residents and relocation when necessary.  As a representative and team member of the company, this position is expected to present oneself in a manner that reflects professionalism and ensures resident satisfaction.


    Work Duties

    ·         Assesses resident needs based on admission social services history, resident’s health, psychological and social needs;

    ·         Develops a Resident Care Plan based on the above assessment;

    ·         Visits and interviews residents at quarterly intervals and when requested by staff, physicians, residents or family members to determine recreational/activity history, needs, and interests, as well as social services’ needs;

    ·         Counsels and assists families with issues or concerns relating to caring for a resident;

    ·         Develops measurable social service goals and revises goals and care plan quarterly;

    ·         Attends resident care plan and resident evaluation committee meetings for resident’s in

    ·         areas of responsibility;

    ·         Completes documentation regarding social service needs as required by regulations;

    ·         Coordinates, facilitates and organizes educational programs for residents regarding health, safety, and quality of life concerns;

    ·         Monitors Medicare eligibility period and provides required notices to residents/responsible parties;

    ·         Establishes relationships with individual residents, their families and neighborhood groups

    ·         Provides general case management (including intake) and referral services to all residents needing such assistance;

    ·         Provides individual casework, case management for frail or at risk residents to include assessment, development of individual care plan, coordination of services and monitoring;

    ·         Serves as liaison with appropriate social and health agencies and service providers in the community;

    ·         Refers and links the residents of the community to local service providers, such as, case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, visiting nurse, preventive health screening/wellness, and legal advocacy;

    ·         Educates residents, and families on service availability, application procedures, client rights, etc., providing advocacy as appropriate;

    ·         Monitors the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual.

    ·         Manages the provision of supportive services where applicable;

    ·         Helps the residents build informal support networks with other residents, family and friends;

    ·         Works with residents, families and outside agencies when need for a higher level of services has been identified;

    ·         Report any suspected cases of abuse, neglect or exploitation of a resident to appropriate agency;

    ·         Report problems of service provider to the appropriate agency;

    ·         May set up volunteer support programs with service organizations in the community;

    ·         Maintains accurate records and files in the most confidential manner;

    ·         Promotes and protects the rights of each resident;

    ·         Attends in-service training workshops and meetings as required;

    ·         Provides effective and courteous service to all residents, guests and co-workers;

    The Social Worker I is responsible for conducting in-depth intakes and assessments covering psychological, social and physical, and environmental concerns for frail, elderly clients; Holocaust survivors and their families. S/he will evaluate clients, identify the potential limitations which impede independent living and activate resources to allow client to remain safely in the home setting.


    Qualifications:

    •Bachelor’s degree in social work or related health or service-related field

    •Current unrestricted CA Driver’s License and proof of auto insurance

    •Prior experience with client engagement, customer service, or social service interventions, and working with the LGBTQ community is desired.

    •Demonstrating front-facing customer service skills, being culturally sensitive, and being able to work as part of a team and maintain professional boundaries.

    •Experience working with older adults, caregivers, and mentally and physically disabled clients is highly desired.

    •Excellent verbal and written communication skills and ability to utilize telehealth platforms.

    •Must be computer literate in MS Office and have the ability to learn new computer applications.

    •Knowledge of safety issues when working with at-risk clients

    • Provision of information and referral, concrete services and advocacy, short-term case management based on a targeted case management assessment, resource coordination, and benefits/ forms/applications assistance for individuals and families residing in contracted buildings to promote health, psychological, and social well-being.

    • Work closely with the housing building managers from all four sites to coordinate services for interested residents. Outreach and education to building managers and residents on types of programs and services available in the community.

    • Monthly check-in and ongoing monitoring of service delivery and outcomes.

    • Manage multiple tasks and prioritize effectively.

    • Provide excellent, timely, and responsive customer service.

    • Participate in weekly individual supervisory and team meetings.

    • Appropriate documentation in the agency’s database, tracking statistics, and promptly preparing and submitting quarterly reports.

    Mostly remote and community based, home office is San Leandro. Caseload is mainly in in Alameda County but this person needs to be able to travel to surrounding counties as needed. Will need to go to office a few times/ month for paperwork.


    Key Responsibilities:
    1. Provide social work supervision for up to eight social workers.
    2. May function as a social worker if fewer than seven social workers are being supervised, with the ability to carry three cases in place of one social worker.
    3. Conduct and document regular visits (weekly, biweekly, monthly, or as needed) with assigned social workers for caseload supervision and professional development.
    4. Offer guidance and support to social workers and agency personnel, ensuring professional growth and compliance with regulations.

    5. Monitor and evaluate the quality of services provided by social workers, ensuring alignment with agency goals and state requirements.
    6. Conduct annual and ongoing performance evaluations for supervisees, documenting these evaluations thoroughly.
    7. Review and approve supervisees' timesheets, mileage logs, expense reports, and time-off requests for submission.
    8. Review and oversee social worker caseloads and casework to ensure compliance with laws, regulations, and agency policies.
    9. Ensure documentation and case management meet professional standards of practice.
    10. Monitor child and family binders for quality and completeness.
    11. Review and approve reports such as Needs and Services Plans (NSPs), biannual reports, safety plans, and home assessments.
    12. Monitor placements to ensure the safety, compliance, and well-being of clients.
    13. Conduct home inspections and unannounced visits as assigned, ensuring compliance with licensing and safety standards.
    14. Implement corrective action plans to address compliance issues or improve service delivery.
    15. Assist the Program Director in educating staff on state and agency regulations and monitor compliance by staff and resource families.
    16. Respond promptly to crises, providing effective intervention and support to social workers as
    18. Provide comprehensive orientation and ongoing training to new and existing social work personnel.
    19. Participate in and conduct Resource Family trainings to enhance caregiver skills and knowledge.
    20. Approve Resource Family reimbursements such as enrichment forms for submission.
    21. Assist with placement calls, intake assessments, and on-call support as needed.
    22. Act in the Program Director's absence, assuming relevant duties and responsibilities.
    23. Accurately report incidents, issues, and concerns to the Program Director in a timely manner.
    24. Act as a liaison between the social work team, Resource Families, and external agencies to ensure effective communication and collaboration.
    25. Actively participate in Child and Family Team (CFT) meetings, staff meetings, planning sessions, and program evaluations.
    26. Meet monthly, or as needed, with the Program Director for supervision and professional development.
    27. Represent the agency professionally in all interactions.
    28. Perform additional duties as assigned by the Program Director to support agency objectives.
    29. Abide by the agency’s professional Code of Ethics, maintaining the highest standards of integrity and accountability.

    Candidate must have eviction prevention experience, information and referrals, event and activities planning and execution, strong customer service skills, exceptional communication, newsletters and calendars, work experience with families and children, the ability to be flexible and pivot when needed, as well as be a team player. This is a full-time temporary opportunity, 40 hours per week assigned at 3 local properties in Palo Alto/Mountain View areas.


    Resident Services Coordinator is responsible for assessing resident needs, developing and/or identifying programming to meet those needs, making resource information available to residents, and providing linkages to needed services for low-income residents living in affordable housing developments. This position is responsible for the resident services coordination at assigned properties designated for families, seniors and/or adults with special needs (i.e. developmental or physical disabilities, chronic mental health issues, chronic health conditions, etc.). The ability to effectively coordinate services for residents directly impacts the organization’s mission statement in that it allows residents to maintain their housing for longer periods of time, enhances their quality of life and provides for greater incidence of safe, affordable housing for the populations we serve.



    EDUCATION and/or EXPERIENCE

    · 2 years of experience and/or training in community development, social services or related field; OR equivalent combination of education and experience required.
