    Therapeutic Certified Recreational Therapist

    Part Time Position

    Range: $25 - $32 per hour

    Provide therapeutic recreation-based treatment program opportunities, including individual and group therapy, in a safe, effective manner to all program participants. Complete assessment/evaluation in the areas of Recreational Therapy upon admission for all program participants. Work directly with paraprofessional staff to facilitate training and program participant growth in the areas of Recreational Therapy. Assist in the development of the rehabilitation plan of care to meet the needs of each program participant. Remain current and compliant with federal, state, and accrediting body regulations and standards. Adhere to work practices consistent with Mentor mission, policies, and procedures. Procure and maintain all recreational equipment to assure safe and appropriate use by program participants and staff. Advocate and educate staff, program participants, and families in the areas of recreational therapy and therapeutic recreation. Maintain best practice methods through continued education coursework.

    Qualifications:

    Bachelor's Degree in Recreational Therapy. Two years of experience in a rehabilitation environment and/or work with persons with neurological and developmental deficits. Certification of CTRS (Certified Therapeutic Recreation Specialist). Knowledge of rehabilitative techniques relative to recreation for persons with diverse neurological impairment. Ability to interpret evaluations from various disciplines (Neuropsychology, Occupational/Speech/Physical Therapies) and understand the relevance to your practice. Ability to handle stressful situations in a calm, professional manner. Ability to exercise sound judgment and discretion. Exceptional communication skills with an ability to establish trust and rapport quickly. A good listener with an ability to empathize while still providing guidance.

    Licensed Practical Nurse/Licensed Vocational Nurse

    Full Time Position

    Range: $27 - $31 per hour

    This is a fantastic role to continue your nursing career path and work within a setting that offers variety and rewarding challenges. You will have high-impact work by providing nursing services, training, and supports to address the medical or psychiatric needs of individuals with a brain injury, intellectual and developmental disability, a substance use disorder, and/or has an emotional or behavioral disability.

    Monitor the health conditions of individuals served and ensure quality services are provided. Administer medications and provide documentation as well as oversee and monitor staff members who administer medications. Provide prescribed medical treatment by physician's orders and personal care services. Conduct assessments, implement health care services to be included in each Individual Service Plan (ISP), and respond to changes of condition. Provide training to staff members including universal precautions, infection control, and medication administration. Serve as liaison and advocate with other medical and professional service providers as well as work with external agencies to coordinate services and ensure consistent treatment of individuals receiving care. Provide training for individuals served with topics including self-medication, prescribed treatments, sex education/STD prevention, and smoking cessation.

    Qualifications:

    SUMMARY

Attends programmatic meetings on request for team reports, policy setting, exchange of information, etc.; attends other appropriate meetings/conferences as needed. Attends clinical meetings to exchange, discuss, and evaluate program participant treatment programs. Serves on rehabilitation treatment team(s). Communicates with physicians, staff, professionals/paraprofessionals, consultants, families, and community resources as needed to ensure effectiveness of physical therapy program. Plans and implements in-service programs for staff and other groups as requested/assigned by supervisor. Participates in orientation programs for new staff, program participants, and PT/PTA students. Writes service summaries as requested by case manager/program director and maintains daily billing summaries and turns them into the appropriate staff. Performs other related duties and activities as required.

    Education and Experience:

    Bachelor's Degree in Physical Therapy One year's experience preferred

    Nursing Manager Pay Range: $90,000 - $105,000

    SUMMARY

    Use your RN experience to improve the lives of individuals with complex conditions while providing meaningful support to your co-workers. As Manager of Nursing, you will lead a team of nursing staff and provide nursing services, training, and support to address the medical or psychiatric needs of individuals with developmental disabilities, substance abuse issues, and/or emotional or behavioral challenges.

    Oversees team of LPNs and RNs. Evaluates performance and implements corrective actions as needed. Directs the quality of nursing assessments, nursing diagnoses, planning, implementation, and evaluation. Monitors the nursing delegation process in conjunction with the Medical Director. Ensures required training is completed, reviews delegation decisions, and develops procedures for assigned tasks. Conducts the medical assessment of each individual referred to a program and collaborates with the Intake Coordinator to determine appropriateness for placement in the community. Provides oversight and coordination of Nursing Intake process, service plan, MDS, and other meetings related to the nursing care of the specialized foster care child. Develops and leads medical & clinical trainings in conjunction with the Medical Director. Directs with expert clinical judgment, based on accepted nursing standards, state administrative codes, contract obligation, policies/procedures and TMN Quality of Care Standards. Evaluates nursing staff in knowledge and application of law, regulation, and policies. Conducts on-going quality assurance and improvement audits; develops and implements corrective action plans as needed. Develops and implements nursing related QAI systems to document all performance evaluation processes. Acts as a liaison between DCFS, community health agencies, and providers. Identifies community resources and develops relationships to expand network. Develops systems to effectively coordinate and integrate care through the use of community resources in collaboration with Network Clinical Team, and others. Complies and analyzes QAI data and makes recommendations. Coordinates responses to address widespread QA concerns within a state as needed Participates in the development and implementation of service plans and supervises, in conjunction with the physicians, the medical components of the plan. Reviews documents and revises all nursing/nursing related care plans on a regular basis. Oversees the management of high-risk medical conditions and private duty nursing services. Performs other related duties and activities as required.

    Minimum Knowledge and Skills required by the Job

    Education and Experience:

    Graduate of an accredited RN program Minimum of five years' experience working with medical and/or behaviorally complex children Preferably, experience with children with special needs One year supervisory experience required.

    Certificates, Licenses, and Registrations:

    Current state RN License. Current driver's license, car registration and auto insurance if providing transportation to individuals receiving services. Current CPR/First Aid certification.

    Blood Borne Pathogen Category:

    Job requires performance of duties that involve potential for exposure to blood, body fluids, or tissues. Tasks that do involve exposure are an expectation of employment.

    ESSENTIAL JOB FUNCTIONS

Works with supervisors in the development, implementation, maintenance, and generalization of behavioral change. Provides Behavioral Therapy and Active Treatment programming in the residence job site, and within the community on an individual or group basis. Participates in family education and therapy as needed Participates in treatment planning with the trans-disciplinary team and attends pre-admission meetings as requested. Serves as a member on the rehabilitation team. Ensures evaluation, progress, and discharge reports on each program participant are provided in a timely manner. Completes daily billing summaries on a timely basis. Assists in coordination of behavioral programming within all living settings, community settings, and vocational sites as necessary. Consults with and/or trains staff in behavioral techniques. Oversees work and trains BIT intern and practicum students. Ensures adherence to accreditation standards and ethics of confidentiality.     SUMMARY Position Type: Full-time, non-exempt, hourly supervisor role. Scope of Role: Supervises a single program or unit within a program, typically supporting 4-8 individuals in residential settings or 10-15 individuals in periodic, vocational or day programs. Duties are split between the provision of direct support, professional or program activities and supervision. Key Responsibilities: Supervise Direct Support Professionals Oversee the day-to-day operation of the program including delivery of supports, implementation of person-centered plans, health care, advocacy, customer engagement, regulatory compliance and when applicable, oversight of the physical environment. Generally works 8 to 16 flexible hours per week in a supervisory capacity; and provides direct support for the remaining hours. ESSENTIAL JOB FUNCTIONS

    To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below.


    Services and Supports: Direct Support: Works regularly scheduled shifts as a direct support professional (see DSP position description). Records: Documents as required; reviews individual records, billing and other program documentation routinely for accuracy and completion on a daily basis; maintains Program Planning: Assists the Program Director in development of individual program plans; trains and monitors implementation of plans each day across all shifts. Team Meetings: Attends and assists with routine progress meetings. Compliance: Assists the Program Director in maintaining licensing compliance for program services and supports; implements Sevita compliance plan; manages individual rights implementation. Management of Individuals' Assets: When assigned, assures safety of individual's funds and property; monitors spending and reconciles individual's financial accounts monthly. Incidents: Completes or assists DSPs in documenting incident reports; submits initial incident reports to the Program Director. Billing and Utilization: Compiles or accumulates census or attendance information; reviews data and attests to accuracy; submits accurate data to the Program Director or appropriate billing authority.
    Health Care: General Health Care: Monitors health of persons being served; documents; communicates with medical professionals as appropriate; implements preventive strategies to limit illness and serious incidents; reviews and monitors documentation across all shifts. Appointments: When applicable, schedules or assists in scheduling health care appointments; may accompany individuals to appointments; may assist with communication; documents visits; oversees follow through of orders; communicates results as appropriate with families, guardians, case managers, day programs or others. Medications: When applicable, accurately administers or supports self-administration of medication and documents delivery of medications and treatments; reviews all other medication documentation for accuracy and completeness, daily or weekly as required; reviews medication errors with employees; communicates medication changes and provides training across all shifts; maintains appropriate security of medications and supplies. Medical Supplies and Equipment: Purchases and maintains medical supplies and equipment in consultation with Program Director; communicates with employees and may assist with training on proper use. Dietary planning: Develops menus based on diets, preferences, and occasions, and with the exception of some in-home services, purchases or oversees purchasing of food and supplies in accordance with planned menus and individual choice or need; directs and monitors food service, consistent with individual plans. Stakeholder Relations: Communications: Maintains productive and professional relations with individuals, families, case managers and other stakeholders. Customer Service: Implements Sevita Customer Service Standards; trains DSPs in delivery. Personnel: Recruitment and Hiring: As needed, assists with recruitment; interviews and hires direct support professionals in consultation with the Area Director and Talent Acquisitions teams. Employee Scheduling: Populates and maintains schedule to assure that all shifts are covered, and overtime is minimized. Training: Assists with onboarding and delivers orientation and ongoing training concerning the individuals served, program plans, policies, procedures and physical environment; Verifies training records are accessible in the home and are completed by all staff on the schedule. Evaluation: Prepares and conducts annual evaluations for Direct Support Professionals, in consultation with the Program Director. Discipline and Termination: Coaches employees as needed, correcting or directing employee behavior where required; may remove employees in the event of emergency; may recommend and participate in decisions regarding termination; consults with the Program Director on all matters of discipline. Employee Safety: Responsible for employee safety and workers' compensation within the program; implements and monitors compliance with safety standards. Management: May assist with or lead monthly program meetings for direct support professionals; documents monthly meeting agenda and attendance.
    Financial Management: Purchasing: When applicable, purchases food and household supplies; purchases office supplies and minor equipment; reviews and attests company credit card statements and monthly invoices; directs employees as necessary; monitors and attests to cost effective, accurate and applicable spending. Individuals' Served Funds Management: Secures and safeguards individuals' served funds locally; uses forms for money/debit card transfers; updates financial ledgers weekly and maintains itemized receipts for purchases on behalf of individuals served Billing and Utilization: Reviews census, attendance information or other required documentation to assure accurate billing; monitors utilization between authorization and provision of services and supports. Staffing: Monitors staffing hours for budgetary compliance.
    Census Management: Census Capture: Captures daily census within systems; understands importance of accurate and timely service capture along with required documentation. Census Performance: Develops basic understanding of census, including how census is measured and census performance for locations; Escalates potential service disruptions Placements: Supports location preparations required for expected placements (e.g., room & furniture set-up, safety requirements, etc.).
    Maintenance: Vehicles: When applicable, test drives vehicle monthly; inspects vehicle lifts and seatbelts monthly; reports safety concerns immediately to supervisor or maintenance personnel; assures vehicle is washed and cleaned; trains staff in safe vehicle operation and safely transporting individuals; ensures routine maintenance is performed. Living Environment: When applicable, schedules and monitors daily and seasonal housekeeping; oversees lawn and yard care. Maintains neat, clean and safe environment for individuals served. Maintenance and Repair: When applicable, makes or arranges for minor repairs; reports to and schedules repairs by maintenance personnel as required; monitors environmental safety; monitors water temperature, refrigerator and freezer temperature, and safety alarms and extinguishers on a monthly basis; replaces furnace filters monthly. Equipment: Monitors wheelchairs, lifts and other safety equipment; sees that repairs are made promptly and correctly in the event of faulty equipment.
    Other: Performs other duties and activities as required, including backfilling roles under your supervision. MPA = Mobile Punch Authorized for timekeeping.
