    Job Summary:

    The Receptionist serves as the first point of contact for visitors, clients, and staff. This role is responsible for greeting guests, answering and directing phone calls, managing the front desk area, and providing general administrative support to ensure smooth daily operations. This position involves interacting with visitors, managing communications, and handling various clerical tasks with efficiency and professionalism. The ideal candidate will have excellent organizational skills, a keen attention to detail, and the ability to manage confidential information with discretion.

    Duties/Responsibilities:
    • Welcome visitors, address inquiries, and manage all reception desk activities while ensuring a positive, welcoming environment.
    • Manage incoming communications by answering phones and directing calls to the appropriate staff.
    • Effectively handle and distribute incoming mail and deliveries, and perform clerical tasks such as copying, faxing, and filing.
    • Provide administrative support such as data entry, filing, and document preparation.

    • Ensure security by following procedures and controlling access with visitor sign-ins.

    Education and Experience:
    • High school diploma or GED required, preferably in a related field of study.
    • 1+ year prior experience as a receptionist, front desk representative, or similar role.

    Required Skills/Abilities:
    • Proficient in Microsoft Office Suite programs including Word, Excel, PowerPoint, etc.
    • Strong interpersonal and communication skills with staff, clients, and visitors to ensure a positive experience.
    • Strong organizational and multitasking skills, and ability to meet deadlines.

    Job Summary:

    The official job title is Community Support Outreach Specialist.

    The Community Support Outreach Specialist cultivates relationships with community programs, members and leaders. They develop assistance programs, offer crisis intervention for the homeless, and provide expert referrals and advocacy to help secure permanent housing, while also providing intensive support to resolve housing barriers for individuals experiencing homelessness. The Community Support Outreach Specialist also implements ongoing efforts to support members after securing housing, ensuring continued development of life skills, financial literacy, and home management skills to promote long-term stability and self-sufficiency. The program we service will include programs and centers from the community support services section below.

    Responsibilities:

    Perform thorough screening and assessments with individual members or families, gathering functional, environmental, financial, employment, housing, educational, and health information. The CS Outreach Specialist aid in assisting them to find and sustain housing, ultimately putting an end to their homelessness. Develop rapport with program members to understand their housing needs, goals, strengths, and barriers, then create a feasible, sustainable Individualized Housing Support Plan based on the Community Support Assessment. Keep the channels of communication open, and be proactive about problem-solving with individuals, interdisciplinary teams, members of Community Support, potential employers and training programs. Offer personalized case management support to program participants that is individualized, culturally competent, family-focused, goal-driven, strengths-based and appropriate to ensure members are matched with resources considering their needs and income-generating abilities. Provide personalized support in life skills, job preparation, and financial literacy, to guide members towards self-sufficiency by offering essential training in home care, job readiness, and budgeting. Maintain member records, track outcomes in appropriate databases, and submit regular monthly and weekly progress reports to Supervisor. Act as a liaison between landlords and tenants to resolve issues and prevent evictions, conduct post-housing home visits to ensure lease compliance, coordinate case conferences, consult with service teams, social workers and case managers as needed. Monitor member's progress towards agreed-upon goals. Perform additional duties as assigned.

You will manage the entire engagement cycle, from initial contact and building rapport to successful onboarding and securing a "win" for the individual and the community.Generate Leads & Build Your Pipeline: Proactively canvass communities-to identify and engage potential members. You are not waiting for leads; you are on the front lines creating them.Qualify Prospects with Precision: Master the DHCS eligibility criteria to quickly and accurately assess if an individual qualifies for our life-changing Enhanced Care Management and Community Support Services.     Job Summary:

    We are looking for a detail oriented and proactive Housing Deposit Coordinator to manage the submission and facilitation of housing deposit authorization requests to the health plans. This role is crucial for ensuring all processes are executed in a timely and accurate manner, in compliance with relevant guidelines. The Housing Deposit Coordinator will oversee landlord and tenant relationships, process housing authorizations, address any issues or denials, and collaborate with various internal departments.

    Responsibilities:

    Submit housing deposit authorization requests to health plans, ensuring all required documentation is accurate and submitted on time. Act as a liaison between landlords, tenants, and health plans to facilitate smooth processing of housing deposit authorizations and resolve any arising issues. Review and validate all documentation related to housing deposits and authorizations, ensuring correct coding and completeness. Collaborate with internal staff to address discrepancies or missing information. Ensure timely processing of authorizations, monitor the status of pending requests, and follow up as necessary to secure approvals. Investigate authorization denials, working closely with insurance providers to identify reasons for denials. Partner with internal teams (billing, legal, Community Support staff) to ensure compliance with polices and resolve issues related to housing deposit authorizations. Ensure all housing deposit authorizations comply with internal procedures, health plan requirements, and applicable regulations. Maintain accurate records for auditing purposes. Communicate with leasing offices to gather required documentation and confirm lease agreements. Ensure all lease agreements comply with organizational standards and requirements. Submit referrals to the health plan for the housing deposit/housing sustainability services on behalf of approved members. Follow up with the health plan to secure approvals for requested services. Send approval and all applicable documentation to our finance department for processing of housing deposit or billable contact forms. Coordinate with property management/members to assist in facilitating housing deposit payments in a timely manner. Perform additional duties as assigned.

    Education and Experience:

    High school, diploma or GED is required. Minimum of 2 years' experience in an administrative or coordinator role, preferably in a health care or housing related setting. Experience in handling authorizations, insurance submissions, or similar administrative roles.

    Required Skills/Abilities:

    Must have a valid California driver's license and valid vehicle insurance. Strong attention to details with the ability to identify errors in documentation and medical coding. Excellent written and verbal communication skills; comfortable with interactions with landlords, tenants, health plans and internal teams.     Job Summary:

    The official job title is Lead Care Manager (LCM). 

    The Lead Care Manager (LCM) role involves developing personalized care plans, coordinating member services, and collaborating closely with members and families, as well as Primary Care Providers to ensure they receive necessary medical treatment and support. The LCM will consult with members to determine their needs, develop individualized action plans, and work with care teams to manage the member experience effectively. Providing emotional support, resolving administrative issues, and ensuring timely access to care are key aspects of the position. Compassion, healthcare knowledge, and exceptional customer service are essential qualities for assisting members in becoming self-sufficient in health. The LCM will work with a diverse population of members enrolled in the Enhanced Care Management program, which may involve one or multiple members from the population of focus section below.

    Responsibilities:

    Interview members to assess medical and social determinant of healthcare gaps and provide education about their condition and medication, while developing individualized care plans. Respond to member inquiries and concerns, ensuring adherence to hospital and legal requirements. Collaborate with interdisciplinary teams, locate medical and social resources, and coordinate social service plans. Maintain on-going contact with members, via telehealth and in-person visitation. Advocate for members, consult with healthcare providers, arrange appointments and treatment plans, evaluate member progress, and assist with healthcare barriers. Maintain empathy and professionalism while contacting members and families. Supporting behavioral health coordination, Substance Abuse and Community Resources. Perform additional duties as assigned.

    Populations of Focus:

    Individuals experiencing homelessness: Lacking a fixed, regular, and adequate nighttime residence. Individuals at risk for avoidable hospital or emergency department utilization: Five or more emergency room visits in a six-month period that could have been avoided with appropriate outpatient care or improved treatment adherence. Individuals with Serious Mental Health and/ or substance use disorder needs: Adults who meet the eligibility criteria for participation in, or obtaining services through Specialist mental health services or the Drug Medi-Cal organized delivery system or Drug Medi-Cal program. Individuals transitioning from incarceration/Justice Involved: Adults transitioning from a correctional setting or transitioned from a correction setting within the past 12 months, or children and youth who are transitioning from a youth correctional facility or transitioned from being in a youth correctional facility within the past 12 months. Adults living in the community and at risk for long-term care institutionalization: Adults who are living in the community who meet the SNF Level of Care criteria; or who require lower-acuity skilled nursing or equipment for prevention, diagnosis, or treatment of acute illness or injury. Adult nursing facility residents transitioning to the community: Adult nursing residents who are interested in moving out of the institution, and are likely candidates to do so successfully, and are able to reside continuously in the community. Children and youth enrolled in California Children's Services (CCS) or CCS Whole Child Model (WCM) with Additional Needs Beyond the CCS condition: Children and youth enrolled in CCS or CCS WCM and are experiencing at least one complex social factor influencing their health. Children and Youth Involved in Child WelfareIndividuals with Intellectual/ Developmental Disabilities: Adults who have a diagnosed I/ DD and qualify for eligibility in any other adult ECM population of focus. Pregnant and Postpartum individuals; Birth Equity Population of Focus: Adults and youth who are pregnant or postpartum and qualify for eligibility in any other adult or youth ECM POF, or are subject to racial and ethnic disparities.

    Education and Experience:

    High school diploma or GED required. Minimum of 1 year experience in case management, member care, customer
    service, call center, or member care required. Valid California driver's license and valid vehicle insurance required. MA certificate or medical terminology knowledge preferred.

    Required Skills/Abilities:

    Excellent communication, interpersonal, customer service and organizational skills. Bilingual in Spanish (Required).
    Computer skills for documentation, email and chat support. Proficient skills in working independently and collaboratively in a team to provide member care. Proficiency in multitasking, organization, and attention to detail is required for effectively providing care to multiple members simultaneously. Candidates should exhibit the capability to utilize resources effectively for problem-solving while maintaining composure under pressure in a compassionate manner. Applicants must demonstrate proficient speaking, reading, and typing abilities and possess strong proofreading skills.

    Physical Requirements:

    Must be able to travel using personal vehicle to complete outreach visits. Mileage reimbursement for the use of your vehicle is at a standard rate. Must successfully complete and maintain current Tuberculosis (TB) testing and clearance in accordance with company policy and applicable local/state health regulations as a condition of employment. Prolonged periods of sitting at an office desk on the computer. Lifting: Able to lift up to 15lbs.

    Travel Details:

    Candidate needs to be comfortable driving to San Francisco area, most of our member concentration is in San Francisco, CA.First week of employment will report to the San Ramon office.Mileage reimbursement is compensated at the IRS standard rate.
    Commute to the San Ramon office is considered work time. Staff member can leave their home at the start of their scheduled shift.
    First month is weekly visits on Wednesdays to the San Ramon office
    After the first month the staff member will be evaluated on the frequencies of office visits.
    Office visits will decrease to once per month based on the needs of the office.

    Pay range$27—$30 USD
