    We are seeking a dedicated Nurse Practitioner to provide high-quality care in a primary care setting. The ideal candidate will perform clinical assessments, develop treatment plans, and coordinate with physician to ensure comprehensive patient management. This role focuses on delivering patient-centered care with an emphasis on chronic disease management and effective communication within the healthcare team.

     

    Responsibilities

    Conduct patient assessments and diagnose health conditionsDevelop and implement treatment plansManage medication and monitor patient outcomesEducate patients on health management and preventive careCoordinate care with physicians Maintain accurate clinical documentation in (EHR) EPICManage chronic diseases and follow-up careCollaborate effectively across clinical staff to optimize patient care

     

    Required Qualifications

    Minimum of 2 years’ experience as a nurse practitionerProficient in clinical assessment and patient careStrong knowledge of pharmacologyExperienced with electronic health records (EHR) EPICExcellent communication and critical thinking skillsEffective time management abilities
