POSITION SUMMARY:
We are seeking an experienced Licensed Practical Nurse who is energetic and passionate about helping and working with a multi-specialty clinic with diverse patient population. The Licensed Practical Nurse will work under the supervision of the Nurse Manager to evaluate, triage patients for clinical care of the Healthcare providers. Will support our healthcare team, and contribute to the success of our organization.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Patient Care:Provide direct patient care under the supervision of a Registered Nurse (RN) or Physician (MD).Take vital signs, administer medications, and perform various medical procedures.Monitor patients' conditions, report changes to the RN or Provider, and implement care plans.Help Link new and established patients to care services, including case management, partner services, medical care, social programs, and education. Clinical Support: Assist RNs, Nurse Manager and Providers with exams, procedures, and other medical tasks.Carry out intakes and bio-psychosocial assessments for clientsPrepare patients for exams, tests, and procedures.Maintain accurate and up-to-date patient records.Participate in off-site community outreach events.Administer injections and medications onsite or at offsite outreaches as prescribed by the provider.Monitor, coordinate, and administer vaccinations Communication and Collaboration:Communicate effectively with patients, families, and healthcare team members.Collaborate with RNs, MDs, and other healthcare professionals to provide comprehensive care.Participate in multidisciplinary team meetings and contribute to care planning.Assists in the coordination of services with SJH Case Managers, other disciplines and patient's insurance plans.Advocates for patient in arrangement of medically necessary services. Quality Improvement and Safety:Participate in quality improvement initiatives and patient safety efforts.Identify and report potential safety hazards or quality concerns.Implement evidence-based practices and follow organizational policies and procedures.Adhere to SJH safety, policies, procedures and regulationsMaintain current professional certifications and licensures Administrative Tasks:Manage patient flow, prepare exam rooms, and maintain a clean and organized work environment.Answer phone calls in relations to patient care, respond to patient inquiries, and provide general administrative support.Provide crisis intervention as assistance as instructed by the RN, Nurse manager and Provider as needed.Monitor and coordinate medical care and insurance authorizationPrepare data reports for external agencies and clinic managementMaintain accurate clinical documentation in electronic medical records (EMR)Ensure and Maintain adequate patient records, track service, referrals, due dates, and identify potential beneficiaries.Ensure and Maintain adequate patient's vaccine records, track vaccine utilization and storage use and service.OTHER DUTIES:

Responsibilities:
Responsible for scheduling doctors, nurse practitioners and specialists to maximize coverage and appointment volume at all Saint James Health locationsSupervises Case Managers, Community Health Workers, Site Managers, and NavigatorsWorks with Site Manager to oversee Medical Assistants, Medical Receptionists, and Call Center StaffWork with the site managers to ensure support staff coverage for all sites and providers including; Medical Assistants and Medical ReceptionistsResponsible for staff coverage and adjusts staff working schedules as needed to cover PTO, sick and emergency calls. Maximizes the use of available staff resources.Coordinates staff training and retraining to further the professional development of the staff and improvement to patient care.Works with the Chief Medical Officer and site manager to implement any existing, new or revised clinical processes.Works with Human Resources regarding discipline, grievance resolution, and performance evaluations for all clinical support staff.Works with the Chief Medical Officer to design a process to maintain inventory throughout the sites.Oversees staff to ensure the daily AM and PM checklists are completed.Ensures clinic compliance with HRSA, Ambulatory Care and Mental Health licensing regulatoryManages all vaccine, medication, supply ordering, storage, and inventory.Conducts quarterly staff meetings in collaboration with site managers.Interviews candidates and recommends candidates for open positions.Works with human resources regarding orientation of new staff.Completes yearly competency reviews as well as on going reviews and evaluations.Ensures that all staff certifications are up to date and in human resource file.Collaborates with the Chief Medical Officer and other site managers to scheduled training or the staff regarding risk management training to mitigate risk within the organization.Conducts monthly audits to ensure proper documentation regarding vaccines, EUAs, VIS, history of vaccines administered, vaccine documentation, vaccination sites, proper VFC status, and information is correct and entered in the systemConducts monthly audits to ensure demographics are being charted correctlyImplement quality improvement measures approved by the Chief Medical Officer as needed for staff.Oversees clinical operations with respect to appointment scheduling, patient flow, and medical records.Responsible for maintaining efficiency through provider scheduling and identifying gaps in services.Strengthens the intake process for all medical appointments by identifying needs for staff training.Works closely with the Chief Medical Officer to analyze the delivery of patient care services and to identify strategies to fill identified gaps.Monitors the clinical process of appointment scheduling, patient flow, and medical records.Support the implementation of policy and workflow changesPerform other duties as assigned.Qualifications
Hold a current Registered Nurse license in the state of New Jersey.One year supervisor within a healthcare center or hospital.Ability to fluently speak, read and write English and Spanish.
