    Starting Pay: $18.00-$22.00
    Schedule: Monday - Friday 7:00a-3:30p
    Location: Independent Living at Sagewood

    What you'll do:

    Your role is to provide hands on care and assist residents as needed with their care and ADLs along with being a companion to the resident. Assists with helping to keep resident's apartments tidy and other domestic duties as requested by the resident. By a companion and keep our seniors active while encouraging and respecting their independence.
    Ambulation and transfer assistance Toileting, bathing, and grooming assistance Wake up services and dressing assistance Meal preparation Organizational help Ambulation support (pushing a wheel chair or accompanying for balance) Errands (grocery and household goods shopping, etc.) Companionship Schedule and medication reminders Maintain service plan tracking of all services performed Know and understand resident's expectations and meet those expectations within community policy Inform Supervisor of any changing conditions that affect the resident's ability to function independently

What you'll do: • Helps with the maintenance and accuracy of records in regard to patient care for all patients on personal case load. • Responsible for the correct reporting of billing information of Physical Therapy in regard to all patients on personal case load. • Assists the Physical Therapist in carrying out procedures and programs. • Performs routine treatment procedures in accordance with the planned program. • Contributes to program planning and development in collaboration with a PT. • Observes, records and reports to their supervisor conditions, reactions and responses related to their assigned duties. • Reports any pertinent findings to the registered therapist. • Participate in development and presentation of in-services for the facility as requested. • Perform clinical functions: screens, following of treatment plans, and delivery of treatment in accordance with facility, Medicare, and policies and procedures. • Terminates services formulating discontinuation and follow-up plans when maximum benefit is received under the supervision of a PT. • Assist and assess transfers, gait and balance as required by functional mobility of patient level of assistance from minimum to maximum. • Participate in meetings regarding patient care as appropriate. • Assist when appropriate with any community education and public relations activities of the facility. • Maintains a high standard of resident care. • Responds immediately to all crisis and/or calls for assistance. • Actively participates in in-services, department meetings and continuing education. Communicates with patient’s physician/family as necessary regarding patient’s progress, treatments, etc., in consultation with the supervising Physical Therapist. • Responsible for the coordination and cooperation with other staff members in carrying out the treatment plan as established.
