    Role Summary:
    The Child Care Worker ensures the basic needs, health, and safety of residents are met and supports youth through a nurturing environment during their placement.

    Essential Functions

    I. Maintain Health and Safety of Residents

    Actively supervise youth at all times, on and off site.Apply behavior management and crisis intervention techniques in alignment with TCI training.Use a trauma-informed approach and remain sensitive in high-stress situations.Provide timely written and oral reports (e.g., incident reports, Life Space Interviews).Coordinate with clinical and medical staff as necessary.Maintain a safe and hazard-free environment in the cottage.Ensure facility security across buildings and grounds.

    II. Program Delivery

    Guide and assist youth with daily living activities (self-care, chores, etc.).Participate in and facilitate recreational and therapeutic programming.Implement PBIS practices consistently, modeling ROAR values: Respect, On-task, Act Safely, Be Responsible.Support youth through personal challenges and refer as appropriate.Communicate significant concerns to relevant staff members.

    III. Professional Practice

    Report to shifts on time and follow appropriate dress and conduct standards.Represent the agency professionally with all stakeholders.Maintain strict confidentiality in accordance with laws and agency policies.Responsibly manage agency funds when authorized.Collaborate with treatment teams and follow all agency policies and procedures.Attend mandatory and recommended training sessions.Uphold and support the mission of Rising Ground.

    Job Summary: Under the supervision of the Lead Social Worker, the social worker will be responsible for conducting Intakes, mental health assessments, screenings, Individual Service Plans (ISP), case management and therapeutic services to program participants who access the Runaway and Homeless youth programs (RHY). The social worker is accountable for overseeing and implementing care and resources to meet the physical, emotional, educational, and social needs of the residents within the program.


    Job Responsibilities:

    • Ensure quality counseling, case management and support is provided to all active participants.

    • Provide weekly documented case notes for sessions held.

    • Review treatment and Individual service plans (ISP) to ensure they meet the needs of the youth.

    • Ensure paperwork for youth is completed in a timely fashion including but not limited to intakes, case notes, Bio Psycho Socials /assessments, and referral material.

    • Provide clinical counseling for those youth that display a need for more intense counseling or are assessed to have possible mental health needs on an as needed basis.

    • Complete all necessary housing applications for participants as they transition from RHY programs.

    • Participate in weekly Huddle and Personal Development meetings (PDMs) with Program managers and participants.

    • Facilitate/ co-facilitate workshops/ groups on a weekly basis.

    • Enter data accurately and timely in PTS and other designated systems.

    • Liaison with existing service providers that provide support to youth.

    • Track services and referrals for monthly and quarterly reports.

    • All other relevant duties as delegated Conduct outreach and presentations to recruit youth to the Drop -In Centers or TIL programs.


    Qualifications:

    • MSW degree required.

    • LMSW or LCSW licensing preferred.

    About the Role

    The Social Work Supervisor will supervise and support the case management activities for their assigned RHY programs. The Social Work Supervisor will report to the Director of Social Services who oversees the assessment, planning, family engagement, and evaluation process for case planners, ensuring compliance with all guidelines, quality assurance standards, and performance indicators according to DYCD and OCFS standards and requirements. The Social Work Supervisor will be the lead coordinator for guiding referrals, intakes and discharges. The supervisor will also support the case management team with audits and other case management related activities.

    What You’ll Do

    · Provide effective ongoing supervision feedback to Case Planners who report to Social Work Supervisor.

    · Maintain documentation of supervision through the completion of supervisory logs, and progress notes.

    · Regularly review all progress notes, court documentation, and other relevant documentation to ensure full compliance and best practices in services provided to RHY youth.

    · Develop and maintain a case planning team in each RHY facility who works in collaboration with all milieu staff.

    · Set high expectations and enforce strict deadlines for staff to ensure excellence in all areas.

    · Monitor all components of Treatment Team meetings including challenges, goals, assessments, treatment plan tool compliance.

    · Attend transition meetings, Service Plan Review meetings, staff meetings, admin meetings, treatment team meetings, red flag meetings and discharge meetings.

    · Support the principles of each facility’s Evidence Based Model with the end goal teaching the young people to make positive, lasting changes in their behavior by preparing them to be self-sufficient through education and work experience.

    · Collaborate with agency departments to meet program goals and objectives including the HR, Training and QI Departments in the areas of performance management, interviewing, mandated training, professional development, PAMS Score card, Evolve needs, and QI goals.

    · Ensure that records are accurate and complete in compliance with city, state, & agency regulations

    · Coordinate Intakes for youth entering the RHY programs

    · Cover for Director of Social Services when needed.

    · In the absence of case planner, cover case planner workload as needed.

    · Other related task that may be assigned as needed

    Your Qualifications

    · Master’s degree in human services or related field required / LMSW preferred.

    Minimum of two (2) years of experience, with at least one (1) year of experience, in a supervisory capacity in the field of child welfare, or a minimum of three (3) years of experience, with at least one (1) year of experience working with at-risk youth, under qualified supervision.

    · Excellent writing and organizational skills

    · Experience with juvenile justice system involved youth preferred

    · Proficient with Microsoft Office suite

    · Commitment to progressive social justice and anti-racist practices

    · Belief in an individual’s capacity to change.

    · Bi-lingual (Spanish) a plus

    Work Environment:

    The Social Work Supervisor position will be based at one of the Bronx RHY facilities. Travel to other sites in Queens and Brooklyn may be required for coverage and audit purposes.

    One remote workday per week is permitted upon Director Approval.

    The Social Work Supervisor will provide their own transportation and work-related travel expenses outside of the facilities will be reimbursed upon review. Ability to travel to all five boroughs.



    Position Type/Expected Hours of Work:


    This is a full-time position of 35 hours per week. Daily schedule and hours will fluctuate to meet the needs of the team and clients. The Social Work Supervisor will be responsible for maintaining their own schedule appropriate to the needs of the team and youth served, which include both during the day, evenings, and weekends on occasion. The Social Work Supervisor will be expected to participate in facility treatment team meetings, family engagement, and other events to help to engage youth in placement and streamline services. The schedule requires flexibility to meet the participation expectations. The Social Work Supervisor will be available to respond to program needs after hours or weekend on-call by phone.

    Additional Requirements:

    Authorized to work in the U.S.Ability to work in-person in New York City (NY) for at least 3 days or more per week.Ability to travel to other Rising Ground sites (if required).

    About the Role

    Just Us is an ACS-funded Gender Responsive Diversion Program facilitated by Rising Gound. This program is a holistic, gender-responsive program for girls, gender-nonconforming, and nonbinary youth (TGNBNC) youth who are involved, or at high risk of involvement, in the youth justice system. The project - which is grounded in the tenets of healing-centered, anti-oppressive practice - brings together highly skilled individual coaching; evidence-based clinical interventions; opportunities for young people to engage in group-based activities; economic empowerment skills building; and space where young people can come together to build community. All elements of the project are intentionally designed to support young people in cultivating their leadership skills; building their self-reliance; forging healthy social relationships; developing diverse strategies to avoid engaging in criminalized behavior/activities; and greatly enhancing their educational experiences and work opportunities. The full-time Counselor will provide youth-driven, healing-centered, anti-oppressive, clinical and emotional support and advocacy to young people engaged in the project. This is an exciting and demanding opportunity for an emerging therapist looking to participate in the expansion of a program that is designed to become a national model.


    What You’ll Do


    Connect with 15-20 young people within the project and serve as their primary point of contact throughout their engagement in the program. Intentionally and regularly engage all young people in weekly hour-long individual sessions.With each young person, co-construct individualized engagement plans and assess and revise the throughout the length of the young person’s engagement with the program.On a rotating schedule, be available to help staff at the Teen Hub on the weekend.Engage in collaboration and case coordination with all program staff and a vast network of stakeholders.Participate in weekly supervision, team meetings, group supervision, and attend other required meetings as requiredMaintain program records, case files, and funder reports with accuracy and attention to timeliness detail.Other duties are required.
