    Location: Riderwood Village by Erickson Senior Living We are hiring Home Care Register Nurse (Flex/PRN) for our gated community. No driving required! We have a highly dedicated and passionate team who believe our residents and their families and deserve the highest quality home care . The RN in Home Care provides nursing care according to established policies and procedures for a Medicare-certified home care agency. Compensation: $46.00 - $48.00 per hour, commensurate with experience plus $5,000* sign-on bonus! * Please note: Candidates sourced by third-party agencies or internal candidates are not eligible for this sign-on bonus. How you will make an impact: Partnering and collaborating in the initial evaluation to assess the residents’ needs to develop and implement a service plan based on the evaluation and in conjunction with the resident, family, physician, and /or authorized practitioner Communicating any changes in the resident’s condition Ensuring that the resident’s individualized plan is implemented and followed by the Home Health and Home Support agency team of providers Delivering nursing care as outlined in the plan of care and according to acceptable nursing standards Participating in care conferences and other resident–related meetings Participating in the orientation and training of other agency nurses as well as training any Aide staff development activities What you will need: Current Registered Nurse license for the State of Maryland Current CPR certification, required Current OASIS experience Home Care experience Medicare-certified home health experience Experience in caring for seniors and/or senior with cognitive impairment Please note that specific state regulations and requirements may be applicable. We are hiring resident-centered Physical Therapist PRN who specifically have experience within Home Care field to join our Certified Home Health team. All of our residents live within our award-winning campus (Home Care without the travel! ), creating an environment where you can be part of their journey throughout their continuum of care. Our Physical Therapists support our residents in all aspects of daily living, providing person-centered care to ensure we meet all of our resident’s needs. What you will need: Must meet current Medicare and state requirements Must possess a valid and current Maryland State Physical Therapy license Experience and the ability to complete documentation to meet reimbursement and regulatory requirements (ie: OASIS , etc.) Previous Home Care Physical Therapy experience required Therapeutic experience in older adults across multiple diagnostic groups is preferred Please note that specific state regulations and requirements may be applicable.
