    Program Overview: RHD Montgomery County Homeless Services offers a safe place to stay and assistance for individuals experiencing homelessness, helping them transition towards independent living. This is a new site located in Lansdale, PA, and the program specifically caters to single adults without housing. Clients have access to dedicated caseworkers who help them find permanent housing and apply for necessary benefits. Additionally, we provide practical support such as laundry facilities, transportation, meals, clothing, and essential life skills training.

    This program provides a safe environment and comprehensive support services to help individuals transition to stable, independent living. Services include case management, housing support, life skills training, and access to essential resources.
    Position Overview: The Case Manager plays a critical role in supporting participants at Genny’s Place by coordinating services, connecting individuals to community resources, and helping them achieve their recovery and housing goals.This role requires a compassionate, proactive professional who can advocate for participants, manage multiple responsibilities, and work collaboratively within a multidisciplinary team.
    Key Responsibilities: Team-Based Planning: Collaborate with a multidisciplinary team to develop and implement person-centered recovery plans.Partner with participants, clinical staff, and community stakeholders to support goal achievement.Build and maintain relationships with community agencies and service providers.Direct Client Support: Provide community-based case management services focused on housing stability, recovery, and independence.Assist participants with accessing benefits, housing, and community resources.Transport participants to appointments and support engagement in services.Administrative & Documentation: Complete assessments, service plans, and required documentation in compliance with organizational and regulatory standards.Maintain accurate and timely records in accordance with program and funder requirements.Identify service gaps and recommend improvements to enhance program effectiveness.
    Qualifications:Bachelor’s degree in Human Services or related field preferred (or equivalent experience).Minimum of 2 years of experience supporting individuals in behavioral health, substance use, or IDD services.Case management experience preferred.Valid driver’s license and reliable transportation required.Intermediate computer skills and ability to use electronic systems.

The team supports individuals in encampments, livening outside, or staying in places unfit for human habitation.
    The Case Manager will determine resources needed to meet the individualized needs of the participant. The Case Manager will take all necessary actions in assuring the implementation of the service treatment plan including serving as an advocate for the participant to enable him/her to receive needed services; identification of program needs, service gaps and recommending strategies. A commitment to the APIS and RHD values should be demonstrated as job duties are performed. Case Managers will work business hours 3 days a week (some flexibility in those hours) and 12-8 two days per week. The case manager will first focus on preventing individuals from entering the homeless services system by providing person-centered problem solving to stabilize housing situations or find alternative solutions.
    DIRECT DUTIES:Team-Based Planning: Act as part of a multidisciplinary team designed to provide services from a holistic and Values based perspective. Assist in the development of recovery plans and reviews in collaboration with the program participant, clinical and medical staff and other stakeholders as determined in collaboration with program participant.Develop and maintain relationships with community agencies in order to assist program participants in connecting with all available resources.Direct Client Care: Provide community-based services to program participants focusing on education, connection with resources, community interaction and achievement of recovery plan goals. Working out in community within the service delivery area of the program. Required to drive program participants to appointments.Outreach and Engagement: Engage and verify any single adults experiencing street homelessness in Montgomery County’s Central and Eastern regions.Facilitate problem-solving conversations to identify safe housing alternatives.Provide referrals to community resources and build community connections.Deliver Outreach services primarily in the field where the clients are located as well as maintain and deliver basic and lifesaving supplies for clients.Utilize an outreach vehicle to visit encampments and homeless individuals. Coordinate outreach to encampments and distribute cleaning materials, hygiene kits, and meals as needed.Respond to client determined priority need or emergency situation – food, health, income, transportation, etc.Case Management and Connection to Resources: Provide Coordinated Entry intake, assessment, and referral via HMIS to housing resources.Connect individuals experiencing homelessness to shelter beds; weather related code blue and code red resources, and other emergency related resources.Provide transportation to vulnerable households in times of high need or emergency, or as requested on an emergency basis by the County.Provide case management supports including housing navigation services, linkage to public benefits, physical health services, legal services, and other service resources as needed.Monthly face-to-face engagement/interactions, no less than one time during a 90-day period will constitute an active client.Support move-ins to housing.Encampments/Clean-Up: Provide cleaning materials (bags, instructions, encouragement) for residents to maintain clean sites, dispose of garbage.Engage residents in Harm Reduction Outreach, including providing needed items, as available, as needed.Responsible for participating in community-oriented encampment resolution efforts and promoting healthy street conditions.
    Job Qualifications : Education and Experience: A bachelor’s Degree in Human Services or a related field is preferred and/or equivalent experience. Intermediate computer skills.One year of case management experience preferred.Two years' general work experience providing support to individuals in behavioral health, Addiction Recovery or IDD services required.Valid driver’s license.

    Position Purpose: The Program Manager provides leadership to program staff in collaboration with the Program Director. The program manager is responsible for ensuring the creation of a safe and healthy environment that fosters communication, a sense of well-being, empowerment and support to the consumers supported by the program. The Program Manager is responsible for overall management of the street outreach program and team they supervise. The Program Manager will act as a role model for employees regarding the expectations of how the job duties should be performed, the attitude to maintain while at work and appropriate interactions with consumers and other professionals. A commitment to the RHD values should be demonstrated as job duties are performed. The Program Manager reports directly to the Program Director.
    The Program Manager will provide supervision and support to the street outreach Case Managers. The Program Manager will provide coordination between the Community Connections office and Street Outreach teams. They will also outreach and connect with all Code Blue providers to understand their programs, make sure communication is effective, and provide support with system navigation. The program manager will also support with direct client contact.

    What You'll Do

    As a Community Inclusion Specialist, you will provide assistance to adults with developmental disabilities. The Community Inclusion Specialist creates and facilitates daily outings designed to promote community integration, build cultural identity, and connect participants with relevant opportunities to help meet their larger goals within the community.     At Resources for Human Development, we are dedicated to empowering individuals with disabilities to live fulfilling and independent lives. Our team is passionate about providing compassionate care and support tailored to each person's unique needs. We are currently seeking motivated and caring individuals to join us as Per Diem Direct Support Professionals. Per diem DSPs are eligible to work different shifts at different locations throughout the area. Shifts are typically 7am to 3pm, 3pm to 11pm, and awake overnights 11pm to 7am. You only need to work one 8 hour per pay period in order to stay in our per diem relief pool.

    Community Passages is a community-based residential program designed to provide a homelike environment for individuals with developmental disabilities and dual diagnoses, many of whom present significantly challenging behaviors and most of whom previously resided at state centers and hospitals. Residents develop life skills, increase their self-esteem, and are empowered to make choices for themselves. Person-centered planning, self-determination, and personalization foster individualized programs in an open and safe environment that encourages consumer empowerment and personal growth.
