    RN Supervisor - Charge Nurse

    STATUS:                                  Non-Exempt, hourly, fulltime, more than 30 hours

    REPORTS TO:                         Director of Health Services/Administrator 

    GENERAL SUMMARY:

    The Charge Nurse is an RN Supervisor who is responsible for the overall nursing care/service on his/her shift, including the accurate and timely administration of medications, treatments, documentation and emergency calls, as indicated.  All job duties are to be performed in accordance with established standards as described in the Quality Assurance Manual for the department.

     PRINCIPAL DUTIES:

     Primary Job Duties:

     -Report – read 24 hour report and communication book, initial and date communication book

    -Make rounds – Make visual rounds of unit – See any sick residents first, check all O2 for correct rate, look for safety – alarms, medications in room, call lights in place etc, infection control – tubing labeled, briefs not in trash cans, bags in trash cans, resident care-positioning, room cleanliness. Address any concerns with staff.

    -Check medication room for complete QA audits, expired meds, dated opened vials.

    -Conduct documented review of any admission from the day before. Complete all outstanding documents. Notify DON of any concerns with admission process.

    -Follow up with any family concerns – Investigate resident/family concerns and implement corrective action plans as needed to resolve issues.  Follow up with DON/Social Work.

    -Oversees implementation of resident care plans and conducts rounds to monitor critical or high-acuity residents. 

    -Ensure any admission that come in during the week or weekend are to be completed by the charge nurse (LPN/RN)r.

    -Conduct audit of all skin assessment sheets that should have been completed during the week. Complete any that were not done. Leave written list for DON of any that you had to complete.

    -Audit monthly summary list for needed completion. Leave list of any omissions for DON.

    -Audit for completion of Quarterly assessments per calendar. Leave list of any omissions for DON.

    -Conduct staff education and lead training for new hires, provide feedback and assess competencies.

    GENERAL SUMMARY:

    The Nurses Aide assists the licensed nursing staff by performing routine nursing duties and activities of daily living.

     

    Primary Job Duties:

    -Assists residents with dressing, grooming, eating, bathing, positioning, turning, toileting and exercising.

    -Gives treatments, performs tests, carries out procedures and collects specimens as instructed.

    -Changes bed linens, makes beds and keeps rooms clean and orderly.

    -Transfers, transports and escorts residents as needed.

    PHYSICAL REQUIREMENTS/WORKING CONDITIONS:

     

    Frequent standing, stooping, bending, stretching, squatting; may be exposed to blood and body fluids which may contain HIV and/or HBV; must be able to transport residents via wheelchair, geri-chair or shower chair, pushing up to 150 lbs. unassisted. Must be able to transfer, lift, turn and position a resident weighing up to 125 lbs. unassisted, or with the assistance of one for a resident weighing over 125 lbs.. Must be able to stoop, bend, stretch, squat, stand, and walk for up to 90% of the work day. May be subjected to offensive odors and combative behavior. Must be able to reach up to a level of six feet; must be able to push a dietary cart up to 40 lbs unassisted. Must be able to lift and carry up to 20 lbs. frequently, and 40 lbs. occasionally.

    RN Supervisor - Charge Nurse

    STATUS:                                  Non-Exempt, hourly, fulltime, more than 30 hours

    REPORTS TO:                         Director of Health Services/Administrator 

    GENERAL SUMMARY:

    The Charge Nurse is an RN Supervisor who is responsible for the overall nursing care/service on his/her shift, including the accurate and timely administration of medications, treatments, documentation and emergency calls, as indicated.  All job duties are to be performed in accordance with established standards as described in the Quality Assurance Manual for the department.

     PRINCIPAL DUTIES:

     Primary Job Duties:

     -Report – read 24 hour report and communication book, initial and date communication book

    -Make rounds – Make visual rounds of unit – See any sick residents first, check all O2 for correct rate, look for safety – alarms, medications in room, call lights in place etc, infection control – tubing labeled, briefs not in trash cans, bags in trash cans, resident care-positioning, room cleanliness. Address any concerns with staff.

    -Check medication room for complete QA audits, expired meds, dated opened vials.

    -Conduct documented review of any admission from the day before. Complete all outstanding documents. Notify DON of any concerns with admission process.

    -Follow up with any family concerns – Investigate resident/family concerns and implement corrective action plans as needed to resolve issues.  Follow up with DON/Social Work.

    -Oversees implementation of resident care plans and conducts rounds to monitor critical or high-acuity residents. 

    -Ensure any admission that come in during the week or weekend are to be completed by the charge nurse (LPN/RN)r.

    -Conduct audit of all skin assessment sheets that should have been completed during the week. Complete any that were not done. Leave written list for DON of any that you had to complete.

    -Audit monthly summary list for needed completion. Leave list of any omissions for DON.

    -Audit for completion of Quarterly assessments per calendar. Leave list of any omissions for DON.

    -Conduct staff education and lead training for new hires, provide feedback and assess competencies.

    Licensed Practical Nurse

    GENERAL SUMMARY:

    The Charge Nurse is an RN/LPN who is responsible for the overall nursing care/service on his/her shift, including the accurate and timely administration of medications, treatments, documentation and emergency calls, as indicated. All job duties are to be performed in accordance with established standards as described in the Quality Assurance Manual for the department.

    Primary Job Duties:

     

    -Supervises direct care of residents and nursing personnel on his/her shift.

    -Administers direct care as necessary.

    -Gives and receives shift reports.

    -Rounds twice a shift to assure proper care is rendered in a safe and clean environment.

    -Keeps physicians and families aware of changes in resident’s condition.

    -Assists in developing comprehensive care plans.

    -Maintains the self respect, personal dignity, and physical safety of each resident.

    -Maintains accurate documentation of all care rendered.

    -Responds to resident emergency call system.

    -Interfaces with other disciplines to assure continuity of care and services.

    -Receives, transcribes and checks physician orders in the past 48 hours.

    -Accompanies physicians when making rounds.

    -Encourages restorative care in order to obtain maximum resident independence.

    -Participates in family/resident counseling as necessary.

    -Assists DOHS with GNA employee Evaluations.

    -Assists with EMR education and checks EMR documentation from GNA’s twice a shift.

    -Graduate of an accredited school of nursing. Licensed Practical Nurse with a current license in Maryland.

    -Ability to document care given accurately.

    -Ability to communicate effectively in English and perform simple arithmetic.

    -Wears appropriate non-slip footwear.

    PHYSICAL REQUIREMENTS/WORKING CONDITIONS:

    May be exposed to blood and body fluids which may contain HIV and/or HBV. Must be able to transport residents via wheelchair or shower chair, pushing up to 150 lbs. unassisted. Must be able to transfer, lift, turn and position a resident weighing up to 125 lbs. unassisted. Must be able to stoop, bend, stretch, squat, stand, and walk for up to 90% of the work day. May be subjected to offensive odors and combative behavior. Must be able to reach up to a level of six feet. Must be able to lift and carry up to 20 lbs. frequently, and 50 lbs. occasionally.

     

    Salary Range: $24/hr - $28/hr PRN: $28.50 -$29
