    Position Summary

    Accura Health is seeking a dynamic and results-driven Sales Liaison / Sales Executive to develop and manage referral relationships across the DFW and Houston markets. This individual will represent Accura Health to physicians, hospitals, case managers, post-acute facilities, and community partners to drive patient referrals and revenue growth.

    This is a field-based position requiring strong relationship-building skills and a deep understanding of healthcare services.

    Key Responsibilities

    Develop and maintain referral relationships with PCPs, specialists, hospitals, SNFs, ALFs, home health agencies, and case managers

    Promote Accura Health’s primary and post-acute care management services

    Meet and exceed referral and revenue goals

    Conduct in-person meetings, presentations, and follow-ups within assigned territory

    Identify new business opportunities and growth strategies

    Track referral activity, pipeline, and outcomes

    Collaborate with intake, clinical, and operations teams to ensure smooth patient transitions

    Maintain compliance with HIPAA, Anti-Kickback, and Stark regulations

    Qualifications

    Required:

    2+ years of healthcare sales or referral liaison experience

    Knowledge of primary care, post-acute care, or care management services

    Strong relationship-building, presentation, and communication skills

    Self-motivated and goal-oriented

    Valid Texas driver’s license and reliable transportation

    Preferred:

    Experience in home health, physician services, or care management

    Established referral relationships in DFW or Houston

    CRM experience

    Job description:

    Accura Health is seeking an experienced PRN Field Nurse Practitioner in South, Central & North Dallas to provide high-quality primary and post-acute care services to complex patients in the home setting. This role focuses on Transitional Care Management (TCM) and House Calls, with an emphasis on preventing avoidable hospitalizations and improving patient outcomes. The Nurse Practitioner will manage an assigned coverage area and work collaboratively with physicians, care teams, and internal clinical staff.

    Key Responsibilities
    Clinical Care & Patient Management

    Conduct in-home evaluations and follow-up visits for complex post-acute patientsPerform comprehensive routine and urgent assessmentsDocument findings, treatments, and care plans accurately and timelyProvide physical and emotional support to patients and families during periods of illness or crisisPrevent unnecessary emergency room visits and hospital readmissions through proactive care

    Care Coordination & Communication

    Communicate clinical findings and care recommendations with the Medical Director, ordering physicians, care team, and internal staffCoordinate all aspects of patient care across disciplinesCollaborate with primary and specialty providers to ensure continuity of care

    Diagnostics, Treatment & Prescribing

    Order, perform, and interpret diagnostic tests (laboratory and radiology) within scope of practicePrescribe medications, including controlled substances, per licensure and collaborative agreementsOrder treatments and durable medical equipment (DME) as clinically indicatedImplement therapeutic and corrective interventions as appropriate

    Documentation & Compliance

    Maintain accurate, compliant patient records to support quality care and reimbursementEnsure compliance with all applicable laws, regulations, and collaborative practice agreementsInitiate and support reimbursement processes for services renderedAdhere strictly to HIPAA and confidentiality standards

    Professional Development & Leadership

    Assist with training, mentoring, and supervision of nursing trainees and advanced practice studentsParticipate in development and refinement of clinical practice guidelinesEngage in ongoing professional development and continuing educationSupport a culture of quality, accountability, and clinical excellence

    Supervisory Responsibilities

    Supervise nursing trainees, graduate nurses, and advanced practice students during field placementsParticipate in interdisciplinary supervision of medical, social work, and allied health trainees as assigned

    Qualifications
    Required

    Certified Nurse Practitioner (NP)Active Texas Nurse Practitioner license in good standingMinimum 2 years of Nurse Practitioner experienceMinimum 2 years of Primary Care experienceValid CPR certificationValid Texas driver’s license, auto insurance, and reliable transportationAuthorized to work legally in the United States

    Preferred

    Experience in post-acute care, geriatrics, home health, or transitional careStrong care coordination and documentation skills
