Our mission-driven and member-focused company is seeking an experienced and self-motivated Community Guide who has worked with individuals living with health challenges and social issues, and understands the unique needs of this community.
This energetic and adaptable Community Guide is an integral part of Reema, a start-up working to transform how health care and social care work together. The Community Guide works to build genuine, positive relationships with members in an effort to improve member's health and wellbeing. Their work is completed daily through an integrated technology platform to call and text members, schedule community visits, as well as document all interactions. The Guide will be responsible for the full lifecycle of their caseload, including initial outreach to engage members in services, ongoing work with members to achieve their wellbeing goals through connection to health care and community resources, and graduation from the program. A primary strength of the Guide should be excellent verbal and written communication, building strong meaningful relationships, problem solving, and a comfortability in navigating the ambiguous nature of a constantly growing and evolving startup.
This position requires significant time in the field, directly interacting with members. This role is most suitable for someone who is confident in interacting directly with many people, many of whom have experienced trauma and face barriers because of their mental, physical and chemical health.
Outreach and engagement into care:
Engage and work alongside clients from diverse ethnic backgrounds, focusing on those with: complex social needs and chronic health conditions.Reach out to members and share about Reema Health to enroll them in our care model.Provide high quality and meaningful support to members to achieve improved health and wellbeing outcomes:
Assess member needs and support systems, including social determinants of health, to help create a member-centered care plan.Establish trusting relationships with members, reliably provide meaningful and quality support, and follow-through in a timely and appropriate manner to achieve their wellbeing goals.Coordinate referrals and support appointment scheduling and attendance with behavioral health, primary care, and other provide types for members.Through cold outreach create and manage a caseload of 60 - 100 members and provide coverage for other Community Guides during periods of absence or increased workload.Understand overall goals for the month. Independently organize and prioritize workload with effective time and task management skills to achieve monthly goals and outcomes.Actively listen, empathize, and appreciate the vulnerability of members willing to share their challenges. Community Guides will maintain professional boundaries and confidentiality, seeking consultation and support with complex member situations from the multidisciplinary team.Visit members in person as appropriate. This may be in the member's home, community, the hospital or other treatment setting to build trust, maintain connection and meet member's needs.Utilize text, phone, email and face-to-face interactions appropriately to match your communication approach with each members' preference and situation.Effectively identify and connect members to community and health resources to support member needs. This may include transporting members and accompanying them to access other services. Establish relationships with resources to support Reema's members as needed.Prepare members for services ending by keeping self-sufficiency and independence top of mind.Complete accurate and thorough documentation and record keeping in a timely manner:
Utilize the Reema App to thoroughly document all work with members in a timely fashion, including keeping a care plan updated. Understand and comply with confidentiality of protected health information and HIPAA.Support member records to be thorough and accurate in Reema's technology.Provide positive representation of Reema:

Education, Experience and Skills:
Bachelor's degree in Social Work, Human Services, Psychology, Health Sciences or other related fields. Years of related experience and subsequent expertise can be substituted for years of completed education towards a bachelor's degreeThree or more years of direct experience with population or community to be served and knowledge of communityFluent in Spanish - nice to have but not requiredDriven to work with individuals who have complex health needs and believe that recovery is possibleDemonstrated experience with crisis planning, case management, boundaries and personal safety in community workProficiencies in critical thinking, time management, strategizing, and multitaskingStrong propensity towards building and maintaining relationships through initiating conversations and creating connectionsAbility to develop, adapt, and execute outreach and care plansDemonstrated proficiency working with technology, including basic knowledge of all related computer and software applications such as Microsoft Word, Outlook and Excel, and Video Conferencing required; G-Suite preferredQualifications & Position Requirements:

We are seeking an experienced and detailoriented Senior Accountant who can balance hands-on accounting responsibilities with financial analysis and planning. This role will be instrumental in maintaining accurate financial records, supporting month-end close, delivering insightful reporting, building budgets and forecasts, and helping scale our financial processes as we grow.
Reporting to the Head of Finance and Accounting, the Senior Accounting Manager plays a critical role in:
Ensuring accurate and timely accounting entries and financial reportingEstablishing and maintaining scalable, GAAP compliant accounting processes to support rapid growthImplementing and optimizing financial systemsEstablishing and maintaining strong internal controlsResponsibilities:
Manage day-to-day accounting operations, including general ledger, accounts payable/receivable, and reconciliations.Support monthly, quarterly, and annual close processes to ensure timely and accurate reporting.Assist with revenue recognition in accordance with ASC 606, particularly related to medical claims.Partner with the Head of Finance and Accounting on internal and external financial reporting, including KPI tracking and board materials.Prepare, monitor, and update budgets and rolling forecasts in partnership with department leaders.Conduct variance analysis and identify trends to inform strategic and operational decisions.Build and maintain financial models to support scenario planning, cash flow projections, and business cases.Help maintain internal controls, accounting policies, and procedures to ensure compliance and scalability.Support audits, tax filings, and compliance requirements (e.g., HIPAA, SOC2 where applicable).Assist in implementing and optimizing financial systems and tools to improve automation and data integrity.Collaborate cross-functionally to provide financial insights to non-finance stakeholders.Manage relationships with external auditors, tax advisors, and key vendors.Requirements:
Bachelor's Degree in Accounting, Finance, or related field. CPA or CMA preferred6+ years of progressive accounting/finance experience, with a blend of accounting operations and FP&A responsibilities.Proven experience preparing budgets, forecasts, and financial models.Strong technical accounting knowledge and working understanding of US GAAP, ASC 606, and internal controls.Experience conducting variance analysis and presenting findings to leadership.Systems-oriented, with proficiency in QuickBooks Online, NetSuite, or similar, plus strong Excel/Google Sheets skills.Strong business acumen with the ability to connect financial metrics to operational outcomes.Excellent organizational skills, attention to detail, and ability to meet deadlines in a fast-paced environment.Strong communication skills, with the ability to translate complex financial concepts to non-financial audiences.Experience with medical billing, claims adjudication, or revenue cycle management platforms.Preferred Skills and Experiences:
Familiarity with venture-backed companies, investor reporting, and strategic decision-making.Knowledge of equity accounting and experience with tools like Carta.Hands-on experience with system implementations or upgrades (e.g., QuickBooks Online, NetSuite, Bill, Ramp, Brex, RCM tools, or payroll providers).Strategic planning experience, particularly with high-growth companies or startups.Benefits:
