    What will I do as an employee with Redstone?

    As a RN Case Manager for Home Health, supporting the Redstone mission will include the following responsibilities:

    Planning, coordination and delivery of quality home health care and wellness services, assuring compliance with corporate standards and federal, state and local regulations.Following current nursing professional standards of practice and agency approved protocols for home health.Providing ongoing assessment of clients, assuring an accurate evaluation of client needs.

    What will I do as an employee with Redstone?

    As the Director of Home Health, supporting the Redstone mission will include the following responsibilities:

    Clinical and Team Leadership: Provide effective leadership to the Home Health team, offering clinical expertise and guidance. This includes supporting core values, managing staff communication, and addressing behavioral issues.

    Care Management and Coordination: Oversee all aspects of client care coordination, ensuring the timely and accurate development, implementation, and assessment of individualized plans of care (POC). This includes integrating and gaining approval for physician orders, coordinating with interdisciplinary teams, ensuring client satisfaction, and managing medical supply inventory.

    Quality and Regulatory Compliance: Maintain full oversight of the program's clinical operations to ensure compliance with regulatory requirements, accrediting bodies, and payer rules (reimbursement). This involves monitoring clinical documentation, auditing the Electronic Medical Record (EMR), participating in the Professional Advisory Committee, and completing clinical records reviews.

    Operations and Administration: Direct, supervise, and consult with clinical staff, serving as a resource for clinical activities. Responsibilities include supervising and evaluating the physician's plan of treatment, collaborating on referrals, coordinating 24/7 department schedules (including on-call), managing census in line with the budget, and reviewing client intake processes.

    People Management and Development: Be responsible for the full scope of staff management, including interviewing, selection, setting performance expectations, coaching, counseling, and performance evaluation. This role also focuses on fostering a positive work environment and leading staff training and orientation programs.


    What do I need to qualify for this role with Redstone?Registered Nurse (BSN preferred), and hold an unencumbered license to practice in Pennsylvania.One (1) year experience in home health care/hospice/long term care.One (1) year experience in a Medicare-certified home health agency is preferred.Prior supervisory experience is preferred.Experience in health care accounting is preferred.Must possess a valid Pennsylvania driver's license and acceptable driving record.

    What will I do as an employee with Redstone?

    As an Assistant Building Services Director, supporting the Redstone mission will include the following responsibilities:

    Effective leadership that includes supporting core values, addressing zero tolerance behaviors and maintaining active communication with employees supervised.

    Receives written work orders or verbal instructions and completes repairs.

    Conducts visual inspection of boilers, pumps, pipes, and mechanical equipment throughout the campus several times per day. Checks pressure and temperature meters (several times per day) and records data on paper log in binder (one time per day). Reports any leakage, extremes in temperature in the equipment rooms, or anything unusual to the supervisor.

    Conducts monthly fire drills. To be conducted per regulations requires and documents results. Confirms that automatic locks and alarms are working properly on a daily basis.

    What do I need for this role with Redstone?Associate's degree (A. A.) in RHVAC or equivalent from two-year college or technical school.At least 2 years of supervisory experience.A valid PA driver's license free of disciplinary action.

    Key Responsibilities:

    Leadership & Staff Development: Collaborate with nursing leadership to provide clear direction and support to nursing staff. Uphold Redstone's core values, address performance issues, and maintain open communication with all employees.

    Clinical Coordination: Actively participate in daily interdisciplinary team meetings to ensure seamless care coordination. Support accurate and comprehensive documentation of resident conditions and ensure timely communication with physicians and families regarding changes in status.

    Resident Care Management: Coordinate all aspects of resident admissions, care conferences, hospital transfers, and discharges. Attend and actively contribute to care conference meetings.

    Physician & Provider Liaison: Assist with processing physician orders and participate in rounds with physicians and other supportive services.

    Quality Assurance & Compliance: Review outstanding documentation and tasks to ensure completion. Serve as the primary contact for the contracted wound care group, coordinating weekly rounds and ongoing collaboration. Participate in Quality Assurance and Infection Control committees and assist with the development and implementation of corrective action plans for regulatory agencies.

    Staff Education: Assist in developing and implementing staff education programs through huddles, one-on-one sessions, and virtual training.

    Patient & Family Advocacy: Intervene in suspected abuse situations and assist in resident investigations. Serve as a resource for families, addressing concerns promptly and providing exceptional customer service.

    Team Support: Provide hands-on support for medication passes during staff shortages or vacancies. Foster a culture of inclusivity and teamwork across the department.

    Qualifications:

    Active Registered Nurse (RN) license in the state of Pennsylvania.

    Experience working in a long-term care or skilled nursing facility.

    Strong knowledge of nursing principles, practices, and procedures.

    Excellent leadership, communication, and interpersonal skills.

    Proficiency in electronic health records (EHR) and documentation systems.

    Commitment to providing compassionate, high-quality care.

    Ability to work effectively in a fast-paced, interdisciplinary team environment.

The base rate is $27.50/hour (no shift requirement)Bronze, Silver and Gold program level pay ranges $27.50-33.75/hr. The position must work the minimum hours in a four (4) week period master schedule as outlined in the program levels to remain eligible for associated wage rate in that level.Orientation must be initiated within two (2) weeks and completed within six (6) weeks of the employment date.     Providing total nursing care to residents in the facilityMaintaining a safe abuse free environment for all residents living in the facilityAdministration of medications and treatments as prescribed by the physician.Accurate documentation regarding residents' conditions, medications, treatments, and other mandated recordsAccurate completion of admission process to include assessments, interim care plans, service plans obtain physician orders, and timely documentation as per admission process.

    What We're Looking For:

    An active RN license in Pennsylvania.Experience working with the geriatric population.Flexibility to work different shifts, and at different locations.Excellent communication, interpersonal, and collaboration skills.Compassionate and patient demeanor with a genuine interest in senior care.High degree of flexibility and adaptability to thrive in a fast-paced environment.

    What You'll Do:

    As an LPN at Redstone, you will:

    Provide comprehensive nursing care to residents, ensuring their comfort and well-being.Maintain a safe and abuse-free environment for all residents.Administer medications and treatments as prescribed by physicians.Accurately document resident conditions, medications, treatments, and other required records.Complete the admission process efficiently, including assessments, care plans, and obtaining physician orders.
