-Full time schedule in patient homes and at home: Monday through Friday 8:30 - 5:00 pm
-Home care or hospice experience preferred
-Chronic disease management experience preferred
-Must be willing to travel up to one hour from home
-Must be willing to visit patients in their homes 4 days/week, 1 day remote
