    RADIAS Health is excited to announce a NEW Youth Assertive Community Treatment (ACT) Team starting this summer!

    The Youth Assertive Community Treatment (ACT) Team is a specialized program designed to provide comprehensive, community-based mental health services to adolescents and young adults aged 14-20 who are living with mental illness and co-occurring substance use disorders and reside in the metro area. The program aims to support youth in achieving their personal recovery goals, enhancing their quality of life, and fostering independence.

    Key Features:

    Starting Salary Range: Full-Time - $92,539 - $102,529 salary + Office Benefit Package (on-call expenses): $90/month subject to change

    **Sign-On Bonus of $875 payable after 6 months in role**

    The Registered Nurse is a fully integrated member of the team and together with the Advanced Practice Registered Nurse (APRN) or Psychiatrist, guides the medication and medical treatment needs of all individuals served. The RN also partners with team members in carrying out the treatment goals and objectives that are collaboratively developed with the individuals served based on their vision of recovery.

    The schedule is 20 hours per week. The position is part of an on-call rotation for providing clinical consultation after hours.

    Duties and Responsibilities:

    Take the lead role and participate in providing medication administration, education and medical services. Develop, maintain and supervise team psychopharmacologic and medical treatment and medication policies and procedures. This includes administering, evaluating and recording psychotropic medications prescribed by the team psychiatrist. Evaluate and chart psychotropic medication effectiveness, complications and side effects, and arrange for required lab work according to protocol.Participate in daily team meetings and treatment planning meetings.Work with the team to provide ACT services for clients, assume primary responsibility for developing, writing, implementing, evaluating and revising overall recovery goals and plans in collaboration with the client and the treatment team.Manage the system of distribution of medication to clients and integrating medication administration into clients’ individual treatment plans.Administer IM medications to clients.Manage pharmaceuticals and medical supplies.Maintain a record of controlled medications and samples and manage the destruction of outstanding medications.Coordinate, schedule, and administer medical assessments of client physical health, make appropriate referrals to community physicians, and coordinate psychiatric treatment with medical treatment providers to ensure continuity of care.Work with the ACT prescriber to coordinate care with ACT team and external providers to encourage consistent and appropriate ongoing care of the client.Assess and establish appropriate medication management plans for clients including obtaining necessary prescriptions, medication administration, and monitoring side effects. Promote skill building for medication self-management.Provide cross-training to ACT team on topics related to the integration of mental health/ physical health and medication.Work with clients on all aspects of psychiatric rehabilitation, support, and psychotherapeutic interventions identified in the treatment plan. File reports with Vulnerable Adult, the Ombudsman’s Office, Child Protection, and other mandated/required reports when appropriate.Participate in evening, weekend, and holiday coverage and be on-call as needed by the team.Must transport clients in the community.Complete all paperwork, reports, and charting contemporaneously and in an organized manner. Participate actively in on-going professional growth and development; maintain appropriate professional behavior and participate in appropriate supervision.Maintain a pattern of regular work hours.Other duties as assigned.

    Requirements

    Current license as a Registered Nurse in the State of Minnesota. Preferred: bachelor's degree and experience in inpatient, outpatient, or community psychiatric setting.

RADIAS Healthis seeking full-time and part-time Mental Health Counselors to join the Residential Support Services Program. You'll be working in a home-like setting, supporting adults who have experienced significant barriers in living due to symptoms of mental illness and substance use disorders.

    Mental Health Counselor daily support-related responsibilities include spending time interacting with persons served to socialize and build rapport, assisting individuals in meeting daily needs and ensuring ongoing connections with other community supports. You will be a key part of a person's care team, helping to implement treatment.

    We have 14 locations within Anoka, Dakota, Ramsey, and Washington counties.

    Hours:

    Full-time: Evenings (Mon - Fri, Sun - Wed, Wed - Sat, or Mon/Tue/Thur/Fri 4:00 pm - 12:00 am)Full-time: Weekends (Sat & Sun 8:00 am - 11:00 pm or 9 am - 12 am)Full-time: Days (Mon - Fri 9:00 am - 5:00 pm)Full-time: Overnights (Wed - Sat 11:45 pm - 8:30 am), 35 hours per week

    Duties

    Provide courteous and respectful care to all clients. Spend the majority of each shift interacting with clients, developing relationships and building rapport with clients, and conducting tasks that are directly related to client care Monitor clients regularly as stipulated in each client’s care plans. Responsible for knowledge and implementation of service and behavior plans. Remain current on client’s plans and case notes completed by other staff. Encourage and monitor treatment plan compliance. Utilizes crisis assessment tools for suicidal and aggressive behavior. Identify individuals prone to having behavior emergencies and recommends additional programming appropriate for their needs. Help defuse and de-escalate any potential behavior emergencies. Assist residents with their room cleaning, personal hygiene and laundry. Assist clients in independent living skills; hygiene checks, housekeeping, budgeting, etc. Administer medications according to RSS policy and procedures under the supervision of the facility’s nurse. Inform other staff and Lead Clinical Mental Health Counselor when quantity of client’s medications running low. Order client medication, as needed. Update medication sheets when medication changes occur. Develop weekly menus and shopping list and does grocery shopping for the facility, encouraging participation by clients. Plan and coordinates recreational/social activities. Schedule client appointments, as needed. Plan holiday activities and special events. Provide input in staff meetings and treatment planning. Prepare meals, serve and clean up according to Health Department standards. Assist in training new staff. Responsible for providing staff coverage and ensuring the security of the facility and general supervision of residents. Responsible for securing coverage of any missed shifts, planned or unplanned. Participate in routine cleaning duties. Perform light house maintenance or request maintenance assistance as needed. Must be able to transport clients in the community. Produce and distribute incident reports within (8) hours of the relevant event, as needed.

    The Mental Health Practitioner Supervisor will oversee a group home with four residents and provide supervision and clinical direction to staff.     Client Related:
