    POSITION OVERVIEW:

    Under the supervision of the Program Director, the Case Worker will be responsible for implementing, monitoring, and evaluating case management activities for the program. The Case Worker works directly with individuals dealing with homelessness, health care referrals, processing paperwork, etc. The Case Worker is responsible for assessing the needs of the individuals and their families and helping them secure permanent housing, benefits, and access to other available social service incentives.

    KEY ESSENTIAL FUNCTIONS:

    Conduct intake interviews and assessments.Counsel clients with confronting issues adversely affecting their lives (i.e.: mental illness, addiction, abuse).Ensure clients have appropriate medical insurance and are referred to medical provider.Assist clients in finding housing and employment.Maintain updated case files with necessary documentation and client face sheets are always up to date.Network with community partners and organizations.Conduct biweekly group intakes. Schedule weekly meetings with families in caseload. Create case plans and monitor plans for effectiveness. Must conduct weekly follow-ups to close the Case/Stars database.Maintain physical files and/or electronic files within DFTA. Evaluate new and existing client's needs to determine their immediate needs, functional capabilities and assistance needed to stabilize housing, healthcare, finances, etc. Establish and maintain working relationships with public agencies concerning public benefits, employment services, supportive services, housing resources, etc. Attend and participate in all required agency meetings: staff meetings, house meetings, in-service training and weekly case review.Other duties in keeping with the scope and nature of the position which may include DFTA, DOH and continue education training covering the center, and attending community board meetings.May be asked to conduct peer trainings and peer reviews on occasion.Follows clearly established policies and procedures.Makes decisions within well-defined parameters for completion of own work.

    REQUIREMENTS:

    Associate’s degree from an accredited college is required or High school diploma/GED with four (4) years of applicable/relevant experience.Baccalaureate degree from an accredited college is preferred.2-year experience in relevant field: case management, social services, human services and substance abuse experience preferred.Bilingual (English and Spanish) (English and Mandarin/Cantonese) preferred.

    POSITION OVERVIEW:

    Under supervision of the Program Director, the Case Manager is responsible for providing case management, housing support to participants and monthly reporting requirements that will ensure compliance with the parameters of the program. The requirements listed below represent the knowledge, skill, and/or ability required.     KEY ESSENTIAL FUNCTIONS:

    Housing Liaison

    Acts as liaison between participants, superintendents and building managers to ensure all repairs are completed to either the participant’s apartment unit or the building.Maintains a Building and Apartment Log, which includes date, and condition of the apartments and buildings.Reports all non-compliance issues with respect to the apartment and building to the Supportive Housing Manager, including incidents requiring immediate attention.

    Case Management and Record Keeping

    Provides case management and on site crisis intervention Completes required service plans.Facilitates monthly CAB tenant meetings. Educates participants with respect to good housekeeping and home-safety practices, budgeting and landlord/tenant relations, as necessaryMaintains appropriate documentation of all tenant contacts.Documents monthly progress notes in the participants’ chartConducts home visits to help evaluate participants’ compliance with the rules and regulations of the program and to assess the need for referral to supportive servicesTakes random urine samples for drug screening toxicologyMay escort participants to social services agencies, as requestedReports any alleged child neglect or abuse to the Supportive Housing Manager

    Housing Reporting Requirements

    Responsible for completing annual tenant rent calculation forms.Responsible for completing annual apartment inspection forms.Updates monthly program data base.Ensures annual leases are updated and filed

    EDUCATION AND EXPERIENCE:

    Associate’s Degree or minimum of 60 verifiable college credits completed, Bachelor’s Degree PreferredMinimum of one (1) year previous relevant experience in a Mental or Behavioral Health environment required.Ability to use business acumen to exercise good judgment and decision-making.Ability to deal with sensitive issues with diplomacy and discretion.Must be able to multi task with strong organizational skills.Excellent interpersonal skills and able to communicate both verbally and in written form.Commitment to cultural diversity and sensitivity.Ability to maintain a professional demeanor under pressure, and operate with a keen sense of urgency and commitment to quality.
